Application-Dependent Early-Phase Stability of Autologous Fibrin Glue in Endoscopic Transsphenoidal Surgery.

BACKGROUND: In endoscopic transsphenoidal surgery (eTSS), fibrin sealants are applied in a confined and frequently inclined operative field, where early-phase material behavior may influence sealant retention and procedural controllability before complete polymerization. Autologous fibrin glue is characterized by slower polymerization kinetics, which may influence its early-phase coagulation behavior immediately after application. However, the clinical relevance of this early-phase coagulation behavior and its association with application techniques remain unclear. METHODS: We retrospectively

Comprehensive radiological assessment of ethmoid skull-base anatomy: beyond the Keros classification.

The Keros classification has been the traditional standard for stratifying olfactory fossa depth and predicting skull-base risk during endoscopic sinus surgery (ESS). However, accumulating evidence from multicenter CT studies across different populations demonstrates that Keros type alone does not correlate with other clinically important skull-base parameters, including ethmoid roof asymmetry, orbit-referenced skull-base height indices, and the position of the anterior ethmoidal artery (AEA) in relation to supraorbital ethmoid cells (SOECs). This narrative review synthesizes contemporary radi

Trigeminal Neuralgia After a Cerebrospinal Fluid Leak: A Case Report.

Cerebrospinal fluid (CSF) leakage can be difficult to localize and may result in unexplained intracranial hypotension with intractable positional headache. Loss of CSF cushioning can cause brain sagging, leading to compression and traction of cranial nerves within the brain cisterns and mimicking Chiari malformation. This case report describes a patient presenting with trigeminal facial pain and positional headache who was ultimately diagnosed with a thoracic spinal CSF-venous fistula. Both headache and facial pain resolved following surgical intervention. This report highlights the importance

Microvascular decompression of the VIII cranial nerve for the treatment of refractory tinnitus and paroxysmal vertigo - A case series.

BACKGROUND: Tinnitus is a common otologic complaint, affecting 10-15% of the general population, with 5% of patients developing disabling symptoms refractory to pharmacological therapy. Neurovascular compression of the VIII cranial nerve has been proposed as a potential etiology, and microvascular decompression (MVD) may represent a therapeutic alternative. METHODS: We present a case series of seven patients with disabling tinnitus, vertigo, and, in some cases, hypoacusis, all of whom were refractory to at least 12 months of pharmacological treatment. Preoperative assessment included audiomet

Optic Tract Edema on MRI in Spontaneous Intracranial Hypotension with Sagging Brain.

Spontaneous intracranial hypotension is caused by a spinal cerebrospinal fluid leak. It is typically associated with orthostatic headache and a broad range of additional neurological symptoms. Brain MRI features include pachymeningeal enhancement, subdural collections, pituitary enlargement, venous sinus engorgement and downward displacement of the brain. Ocular involvement has been described as optic neuropathy and changes in the optic nerve sheath fluid, but signal changes in the optic tracts have not been systematically reported. We retrospectively reviewed our patients with spontaneous int

Pedicled lateral nasal wall flap for endoscopic skull base reconstruction: how I do it.

BACKGROUND: The pedicled nasoseptal flap (Hadad-Bassagasteguy flap, HBF) has been considered as the workhorse for reconstruction of high-flow cerebrospinal fluid (CSF) leak following endoscopic endonasal skull base surgery. However, if the septal mucosa is affected by a tumor or the vascular pedicle is injured due to prior surgery, the use of HBF is precluded. METHOD: We illustrate the harvest techniques of the lateral nasal wall flap (LNWF) based on the posterior lateral nasal artery (PLNA). CONCLUSION: The LNWF is a robust intranasal regional flap with a consistent blood supply and compara

Clinical Presentation and Epidemiology of Spontaneous Cerebrospinal Fluid Leaks.

Spontaneous cerebrospinal fluid (CSF) leaks are increasingly common for otolaryngologists to identify and treat. Patients may present with clinical symptoms of headache, pulsatile tinnitus, aural fullness, muffled hearing, vertigo, clear otorrhea, or salty, metallic-tasting rhinorrhea. Spontaneous CSF leaks are strongly associated with idiopathic intracranial hypertension (IIH) and share common risk factors and demographics with patients with IIH. Otolaryngologists should be aware and skilled at the diagnosis of spontaneous CSF leaks, given its negative impact on quality of life for these pati

Audiovestibular manifestations of intracranial hypotension: a descriptive clinical series.

BACKGROUND: Intracranial hypotension (IH) results from decreased cerebrospinal fluid (CSF) volume or pressure and typically presents with orthostatic headache. IH may also manifest with audiovestibular symptoms with variable association to endolymphatic hydrops (EH). However, the nature of this relationship remains unclear. OBJECTIVES: To describe the clinical, audiometric, vestibular, and imaging characteristics of patients with IH presenting with cochleovestibular dysfunction. METHODS: We conducted a retrospective observational study including 12 patients diagnosed with IH based on MRI findi

Are there any potential risk factors for infection following an incidental durotomy during spine surgery?

Incidental durotomy is a complication of spinal surgery that carries a risk of persistent CSF leak. One major concern with persistent CSF leakage is the increased risk of infection. Fifty-six consecutive patients who underwent spine surgery complicated by incidental durotomy between 2017 and 2022. We compared 28 patients who developed a postoperative infection (IG) and 28 patients who did not (CG). The median age was 40.6 years in the IG and 57.7 years in the CG (p = 0.04). The index procedures were decompression for stenosis or discectomy for herniated disc; 18 patients had associated instrum

Endoscopic endonasal surgery for cholesteatomas of the posterior cranial fossa and parasellar region. A challenging pathology for experts.

Endoscopic endonasal surgery (EES) offers a direct ventral corridor to the skull base, potentially enabling maximal resection of extensive cholesteatomas (epidermoid cysts) in the posterior cranial fossa and parasellar region. However, outcomes and complications specific to this approach require detailed analysis. A retrospective review was conducted on 34 patients who underwent EES for cholesteatomas of the posterior fossa and parasellar region at a single center from 2011 to 2025 year. Extent of resection (near-total > 95%, subtotal 90-95%), complications, and neurological outcomes were anal

Complete recovery and resolution of neurological symptoms following a penetrating gunshot in the left frontal lobe: a case report.

BACKGROUND: Traumatic brain injury is a major global health concern, with penetrating brain trauma representing an uncommon but highly lethal subset, especially in conflict-affected regions. Outcomes in penetrating brain trauma are strongly influenced by admission Glasgow Coma Scale (GCS), extent of structural injury, projectile characteristics, and imaging findings. Early expert assessment, rapid neuroimaging, and timely surgical intervention are critical for improving survival in selected patients although long‑term recovery often requires multidisciplinary rehabilitation. We report a rare c

Early lumbar microdiscectomy and quality-of-life outcomes: methodological considerations regarding the pilot study by Horan et al.

Horan and colleagues recently reported a pilot study examining quality-of-life outcomes following early versus late elective lumbar microdiscectomy, concluding that surgery within twelve months is associated with superior patient-reported outcomes and proposing a six-month maximum waiting time. While the clinical question is both relevant and timely, we believe several methodological and conceptual issues merit discussion before such a recommendation is translated into practice. First, the study conflates waitlist time with symptom duration, yet its Discussion and proposed threshold are predom

Parallel Postauricular Incisions in Transmastoid CSF Leak Repair: Wound Outcomes.

BACKGROUND: Cerebrospinal fluid (CSF) leak after retrosigmoid vestibular schwannoma resection remains a challenging complication. Historically, surgeons reopened the prior retrosigmoid incision for transmastoid repair because the safety of creating a separate postauricular incision was uncertain, with concerns regarding wound breakdown and infection. However, limited data support this practice, and a focused mastoid incision may offer improved access and patient tolerance. This study evaluated wound healing outcomes following transmastoid repair using a new postauricular incision for persisten

Anterior Lumbar Interbody Fusion Offers Safer Inpatient Profiles But at Greater Cost Compared With Posterolateral Fusion: National Outcomes for Spondylolisthesis From 2016 to 2022.

INTRODUCTION: Degenerative lumbar spine disease represents a leading global source of disability, with spondylolisthesis contributing substantially to the burden of low back pain and impaired function. Lumbar fusion remains a commonly performed surgical strategy for degenerative spondylolisthesis, although decompression alone versus decompression with fusion continues to be an area of active debate, particularly in select low-grade cases. This study aimed to compare inpatient complications, discharge disposition, mortality, and costs between anterior lumbar interbody fusion (ALIF) and posterol

Risk factors for cerebrospinal fluid leakage after spinal tumor surgery: a systematic review and meta-analysis.

BACKGROUND: Previous studies on risk factors for cerebrospinal fluid (CSF) leakage after spinal tumor surgery have reported inconsistent findings, particularly for age, revision surgery, and surgery-related factors such as operative time and blood loss. We therefore performed a systematic review and meta-analysis to identify the most robust risk factors for postoperative CSF leakage in this population. METHODS: PubMed, Embase, Web of Science, Cochrane Library, ProQuest, China National Knowledge Infrastructure (CNKI), China Biology Medicine disc (CBMdisc), Wanfang, and VIP were systematically

Spontaneous spinal CSF leaks: a rare variant exome sequencing study and functional analysis.

BACKGROUND: Spontaneous spinal CSF leaks are associated with connective tissue diseases including Marfan syndrome and Loeys-Dietz syndrome, which are caused by mutations in genes that influence the content and integrity of the extracellular matrix. Patients with spontaneous spinal CSF leaks without a defined connective tissue disease diagnosis can show subtle or non-specific connective tissue disease manifestations, suggesting that mutations in extracellular matrix proteins might contribute to more common presentations of this condition. By doing a whole-exome sequencing study, we aimed to elu
