Electrospun polycarbonate urethane nanofibrous scaffold for dural repair: physico-chemical characterization and in vivo biocompatibility assessment.

Dural repair of the native dura following trauma or surgery is often challenging due to limitations such as the poor extensibility, regenerative capacity, and the structural integrity of the dura mater. This has led to the rise of dura substitutes, both biological and synthetic, that aim to match the native dura in its strength and elasticity. While the biological dura substitutes are met with immune rejection, scarring or disease transmission risks, scar tissue formation is a major concern in synthetic dura substitutes. In revision surgeries, the chaotic extraction of the scar tissue-encapsul

Insufficient Dural Thickness Recovery in Acute Treatment Predicts Subsequent Relapses in Idiopathic Hypertrophic Pachymeningitis.

Objective Idiopathic hypertrophic pachymeningitis (IHP) is an inflammatory disease of the dura mater of an unknown underlying cause. Although corticosteroids are commonly used as a first-line treatment, mixed responses have been observed. Furthermore, biomarkers for predicting relapse independent of various symptoms and lesion morphology have not yet been identified. We aimed to identify the prognostic factors of relapse in the first episode of IHP by objectively measuring the thickness of hypertrophic lesions of the dura mater. Methods In this retrospective observational study, we enrolled pa

Temporal dynamics of the cerebrospinal fluid proteome in tick-borne encephalitis and enteroviral meningitis.

BACKGROUND: Tick-borne encephalitis (TBE) is a severe viral infection of the central nervous system (CNS), causing acute meningoencephalitis and often long-term neurocognitive sequelae. The lack of specific treatment for TBE highlights the need to improve understanding of disease mechanisms and identify novel biomarkers. Enteroviral meningitis is typically a milder infection of the CNS and rarely causes encephalitis. OBJECTIVES: In this hypothesis-generating study, we aimed to characterise temporal changes in the cerebrospinal fluid (CSF) proteome in TBE and identify candidate biomarkers asso

AcNR1F orchestrates epidermal remodeling to enable host adaptation in Angiostrongylus cantonensis.

Nuclear receptor NR1F regulates circadian rhythms in mammals, metamorphosis in insects, and molting in free-living Caenorhabditis elegans; however, its role in parasitic nematode development remains elusive. Angiostrongylus cantonensis, a zoonotic pathogen that causes eosinophilic meningitis in humans, exhibits distinct host adaptation, with mice and most mammals serving as non-permissive hosts and rats as the permissive host. We combined multi-omic analyses, lentivirus-mediated RNAi, LC-MS/MS, and dual-luciferase assays to elucidate AcNR1F functions in development and host adaptation. Acnr1f

Long-term Outcomes in Previously Healthy non-HIV Individuals with Cryptococcal Meningitis Treated with Pulse-taper Corticosteroids for Post-infectious Inflammatory Syndrome.

BACKGROUND: Post-infectious inflammatory response syndrome (PIIRS) is an increasingly recognized complication of cryptococcal meningoencephalitis (CM) in non-HIV infected hosts with significant morbidity and mortality. Data describing long-term outcomes following immunomodulatory therapy are limited. METHODS: We conducted a prospective observational study of 15 immunocompetent adults with CM-PIIRS treated with a pulse corticosteroid taper (PCT) regimen. Performance status, neurocognitive, ophthalmologic, audiologic, and radiologic assessments were performed at greater than one year following

A comprehensive single-cell transcriptomic atlas of hippocampal microglia in a mouse model of traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) triggers microglial activation and disrupts the inflammatory microenvironment in the hippocampus, which are key contributors to neuroinflammation and subsequent secondary brain damage. Nevertheless, the cellular heterogeneity of microglia, their pro-inflammatory activation mechanisms, and microenvironmental regulatory networks have not been fully characterized. METHODS: By integrating multi-source single-cell transcriptomic datasets and establishing a mouse TBI validation model, we constructed a high-resolution cellular atlas of hippocampal microglia.

Immunogenicity and Safety of Single-Dose MenACYW Conjugate Vaccine in Vietnamese Participants Aged ≥ 12 Months: A Phase III, Open-Label Study.

INTRODUCTION: Invasive meningococcal disease (IMD) is a progressive condition commonly presenting as septicemia and meningitis, with significant mortality and morbidity. Vietnam reports high incidence rates among military populations and young infants. Despite this burden and unpredictable outbreaks, meningococcal vaccination in Vietnam is offered primarily to high-risk individuals and travelers rather than as routine childhood immunization. MenACYW-TT, a quadrivalent meningococcal polysaccharide tetanus toxoid-conjugate vaccine with licensure in > 80 countries, is approved for toddlers aged ≥

Erdheim-Chester Disease Mimicking as Multiple Meningioma-A Case Report with Review of Literature.

Erdheim-Chester disease (ECD) is a rare non-Langerhans cell systemic histiocytosis affecting multiple organs, more commonly long bones, lungs, heart and less commonly brain. Meningeal involvement is extremely rare, and till now only six cases have been detected. Our patient presented with multiple supra- and infratentorial enhancing meningeal mass lesions disguised as multiple meningiomas. The mass lesion causing the mass effect was excised, and its histopathology showed histiocytosis. Multisystemic evaluation and molecular studies confirmed the diagnosis of ECD, and the patient was started on

Innate immune response to Streptococcus suis infection: advances and future challenges.

Streptococcus suis is a pathogenic bacterium primarily found in pigs, where it causes sudden death and meningitis, but it is also capable of causing septic shock and meningitis in humans, being the most important cause of adult bacterial meningitis in Southeast Asia. As the host's innate immune system serves as the first line of defense against microbial invaders, it is crucial to understand the intricate relationship between the bacterium and this system. Despite an increasing number of studies performed in the last 20 years, the pathogenesis of S. suis infection is not fully understood, and

Serotype-independent inhibition of S. pneumoniae by SMiTE, a commensal-derived bacteriocin.

Streptococcus pneumoniae remains a leading cause of disease despite widespread vaccination, highlighting the need for serotype-independent strategies. We recently identified commensal streptococci that produce bacteriocins with anti-pneumococcal activity. Here, we evaluate these bacteriocins as candidates for pneumococcal control. Using cell-free protein synthesis, we screened 58 bacteriocins, the majority of which absent in available pneumococcal genomes, and identified SMiTE as the most potent. Purified SMiTE disrupted pneumococcal membrane integrity, as shown by confocal, transmission, and

Severe strongyloidiasis and Gram-negative bacterial superinfections: epidemiology, clinical implications, and therapeutic strategies.

PURPOSE OF REVIEW: Strongyloidiasis is a neglected tropical disease affecting over 600 million people worldwide. Its severe forms carry a fatality rate exceeding 60%, yet death is rarely attributable to the parasite alone: the terminal event is often bacterial sepsis or meningitis of enteric origin. We review recent evidence on Gram-negative and other bacterial superinfections in severe strongyloidiasis, and their diagnostic and therapeutic implications. RECENT FINDINGS: Cohort studies show that bacteraemia is the commonest complication of severe disease, affecting 38.3% of cases against 4.1%

Human angiostrongyliasis: the past, present, and future.

Human angiostrongyliasis is an emerging foodborne zoonosis caused predominantly by Angiostrongylus cantonensis and Angiostrongylus costaricensis, which produce distinct neurological and abdominal forms of disease, respectively. Human infection occurs primarily through ingestion of raw or undercooked gastropod intermediate hosts, paratenic hosts such as freshwater prawns and crabs, or fresh produce contaminated with infective third-stage larvae (L3). A. cantonensis is the principal cause of neuroangiostrongyliasis and eosinophilic meningitis, whereas A. costaricensis causes abdominal angiostron

The glymphatic-lymphatic axis: known pathways, open questions, and emerging roles in neurodegeneration.

The discovery of the glymphatic system and meningeal lymphatic vessels has transformed our understanding of brain fluid homeostasis and central-peripheral communication, highlighting the critical role of cerebrospinal fluid in brain clearance. While substantial progress has been made in defining the structural components and physiological regulators of brain clearance, major uncertainties remain regarding (1) how these systems interact as an integrated physiological network, (2) how they fail in disease, and (3) whether they can be therapeutically manipulated. In this review, we first summariz

Palatal myoclonus in tuberculous meningitis: a case report.

Palatal myoclonus is a clinical manifestation caused by lesions involving the Guillain-Mollaret triangle (dentato-rubro-olivary pathway) and is often associated with hypertrophic olivary degeneration. Reported causes of palatal myoclonus include infectious, inflammatory, autoimmune, demyelinating, neoplastic, vascular, and drug-related disorders. We report a case of drug-resistant tuberculous meningitis presenting with palatal myoclonus and describe the patient's clinical course and outcome.

Atypical Lyme Neuroborreliosis Presenting With Ventriculitis and Cerebritis: A Case Report.

Lyme neuroborreliosis is a neurological manifestation of infection by Borrelia burgdorferi sensu lato and most commonly presents with lymphocytic meningitis, cranial neuropathy, or painful radiculitis. Central nervous system parenchymal involvement is uncommon, and ventriculitis is rarely reported. We describe a man in his late 70s who presented with acute confusion, dysarthria, and left-sided weakness following several days of severe lower-limb pain and progressive mobility impairment. His neurological status rapidly deteriorated, with secondary seizures requiring invasive mechanical ventilat

Targeting glymphatic-associated pathways for Alzheimer's disease: a review of non-invasive modulation strategies, mechanisms, and emerging evidence.

The glymphatic system, a cerebral waste clearance pathway identified over a decade ago, has reshaped understanding of Alzheimer's disease (AD) pathogenesis by linking impaired brain homeostasis to amyloid-β and tau accumulation. This review synthesizes the biological mechanisms governing glymphatic function, including arterial pulsation, vasomotion, cerebrospinal fluid turnover, norepinephrine oscillations, aquaporin-4 polarization, and meningeal lymphatic drainage, and evaluates emerging non-invasive modulation strategies. Unlike immunotherapies that directly target pathological proteins with

Echovirus 19 Molecular Epidemiology in Russia and Some Central Asian Countries in 1999-2024.

Echovirus 19 (E19) is a rarely reported non-polio enterovirus (NPEV), although it has been associated with a spectrum of severe syndromes, including encephalitis, meningitis, and multisystem hemorrhagic disease in infants. Moreover, Echovirus 19 has been detected in patients with acute flaccid paralysis (AFP), suggesting a potential association between viral infection and CNS damage. Here we describe molecular epidemiology of Echovirus 19 in Russia and Central Asia from 1999 to 2024 using data on poliomyelitis surveillance. Echovirus 19 accounted for no more than 4% of annual NPEV isolates, ho

Impact of initial disease severity and metabolic stress on mortality in patients with suspected external ventricular drain-associated infection.

External ventricular drainage (EVD) remains indispensable for managing intracranial pressure and cerebrospinal fluid (CSF) disorders. Despite advances in neurosurgical care and infection prevention, EVD-related infections still contribute substantially to morbidity and mortality. This study aimed to identify predictors of mortality and evaluate the clinical relevance of infection-related and metabolic parameters in patients with suspected EVD-associated infection undergoing EVD placement. We retrospectively analyzed all adult patients who underwent EVD insertion and were clinically suspected o
