Clinical characteristics and MRI findings of COVID-19 encephalitis: A retrospective observational study.

The coronavirus disease 2019 (COVID-19) pandemic raised concerns about its impact on the nervous system, particularly the occurrence of encephalitis. However, the clinical features and prognosis of COVID-19-associated encephalitis remain poorly understood. This study aims to provide a comprehensive analysis of the demographic, clinical, radiological, and prognostic characteristics of COVID-19 encephalitis patients treated at our medical center following the relaxation of disease control measures in China. A retrospective analysis was conducted on 11 patients diagnosed with COVID-19 encephaliti

Outcomes Following Proton Radiation for Pediatric Meningiomas.

BACKGROUND: Pediatric meningiomas make up 2% of all pediatric central nervous system tumors and are distinct from adult meningiomas, often coinciding with neurofibromatosis-2 (NF2). However, there is limited useful outcome data regarding pediatric meningiomas treated with radiation therapy (RT). PROCEDURES: We identified 18 pediatric patients with meningiomas treated at our institution between 2006 and 2024 with proton therapy (PT). All patients received a total dose of 50.4-59.4 Gy in 1.8 Gy/fraction daily, based on tumor grade. Nine patients had WHO Grade I meningiomas and nine had WHO Grad

Building a national repository of dural-based lesions: clinical, pathological, and demographic insights from the Indian population.

BACKGROUND: Meningiomas are the most common dural-based intracranial tumors, yet Indian literature is predominantly composed of limited single-center studies, restricting nationwide representation and data-driven decision making. With artificial intelligence (AI) becoming increasingly relevant in neuro-oncology for diagnosis, segmentation, and outcome prediction, the lack of a large, standardized national dataset poses a major barrier. The Medical Imaging Datasets for India (MIDAS) initiative, a collaborative national effort involving ICMR, IISc, and ARTPARK, aims to create high-quality, annot

Primary central nervous system T-cell lymphoma: a single-centre retrospective study and literature review.

BACKGROUND: Primary central nervous system T-cell lymphoma (PCNSTL) is an exceptionally rare central nervous system lymphoma with limited clinical data. We present a large case review series of PCNSTL to summarise the clinical characteristics of this disease. METHODS: This study integrated 4 new cases of PCNSTL from our centre with 132 previously reported cases identified through a systematic search of PubMed, Cochrane Library and Web of Science databases. RESULTS: A total of 136 PCNSTL cases were identified, with a median age of 41 years (range 2-89 years), and a male-to-female ratio of 1.8

Neonatal-Onset Chronic Granulomatous Disease Presenting With Recurrent Culture-Negative Meningitis: A Case Report and Diagnostic Considerations.

Chronic granulomatous disease (CGD) is a primary immunodeficiency caused by defective phagocyte oxidative burst and may present in early life with severe or recurrent infections. We report a female infant born at 38 weeks' gestation (birth weight 2700 g) who developed fever and presumed sepsis at 5 days of life, followed by multiple recurrent hospitalizations for febrile illness with suspected meningitis, diarrhea, dehydration, and failure to thrive. Cerebrospinal fluid (CSF) evaluations across episodes demonstrated pleocytosis with elevated protein and normal-to-low glucose, while Gram stain

Decoding brain border immunity to enable future therapeutic avenues.

The brain is no longer viewed as immunologically isolated but as an organ surrounded by dynamic border compartments that coordinate surveillance, drainage, and communication with the periphery. Key interfaces - including the meninges, blood-brain barrier, choroid plexus (ChP), and skull bone marrow - host specialized immune niches that regulate antigen sampling, leukocyte trafficking, and neuroimmune signaling. Recent advances in imaging and in single-cell and spatial profiling have revealed previously unrecognized cell types, migration routes, and barrier specializations that shape central ne

Vogt-Koyanagi-Harada (VKH)-What Do We Know About the Disease, and Can We Recognize It?

Background: Vogt-Koyanagi-Harada (VKH) is a multisystem autoimmune disease that ophthalmologists often encounter first. The condition is caused by an immune response against tyrosinase-related proteins in pigment cells (melanocytes) of the uvea, inner ear, meninges, and skin, and the process may be triggered by genetic and environmental factors. Although much is known about the disease, establishing an accurate and timely diagnosis still requires a multidisciplinary team and strong clinical expertise. Treatment demands early and aggressive anti-inflammatory therapy with corticosteroids, often

Characterization of focal leptomeningeal extension in patients with brain metastases: A novel entity.

BACKGROUND: Classical leptomeningeal disease (cLMD) impacts the entire craniospinal axis, but isolated parenchymal metastases sometimes display focal leptomeningeal extension (fLME) without diffuse LMD elsewhere. We assessed whether patients with fLME can be managed with stereotactic radiation (SRS/SRT) as opposed to whole brain/craniospinal radiation without excess development of marginal recurrences or subsequent cLMD. METHODS: We identified 796 patients with 2,354 newly-diagnosed brain metastases (BrM) without cLMD at diagnosis managed at a tertiary center between 2007 and 2022. Each metas

The glymphatic system and glioblastoma.

The discovery of the glymphatic system and meningeal lymphatic vessels revolutionized the understanding of brain waste clearance. These findings challenged the long-held notion of CNS immune privilege and provided insights into how the brain maintains homeostasis despite lacking conventional lymphatic drainage. However, the impact of pathological conditions, particularly CNS cancers, on the glymphatic system and vice versa remains underexplored. Glioblastoma (GBM) is the most common and aggressive primary adult CNS cancer. Its presence results in glymphatic dysfunction with significant clinica

Therapeutic progress in leptomeningeal metastasis from EGFR mutant non-small cell lung cancer: a clinical medicine review.

Leptomeningeal metastasis in patients with Epidermal Growth Factor Receptor-mutant non-small cell lung cancer carries a dismal prognosis and is associated with profound neurological morbidity. Historically, therapeutic efficacy has been severely limited, leading to poor overall survival. This review aims to synthesize the recent, albeit incremental, advances in the multidisciplinary management of this devastating complication. We delve into the evolving landscape of treatment modalities, with a particular focus on the latest generation of tyrosine kinase inhibitors and their superior central n

Trojan horses at leptomeningeal cellular gates: Antibody-drug conjugates deliver cytotoxic payloads.

The DEBBRAH study showed promising results for patients with HER2-expressing (including HER2-low) metastatic breast cancer who received the antibody-drug conjugate (ADC) trastuzumab deruxtecan for leptomeningeal disease. ADCs represent a novel tool in the precision biomarker-based armamentarium, including in the tumor-agnostic setting, for targeting highly cytotoxic chemotherapy into cancer cells.

Bilateral anterior sacral meningoceles in pregnancy without sacral anomaly: a case report of a rare clinical entity.

This report describes a 32-year-old primigravida diagnosed with bilateral anterior sacral meningoceles without bony defect of the sacrum during pregnancy. The patient remained asymptomatic throughout the pregnancy, with regular monitoring via transvaginal ultrasound and MRI. An elective cesarean section was planned at 38 weeks. However, the patient presented in obstructed labor at 42 weeks and underwent an emergency cesarean section, resulting in the birth of a healthy infant. This case is unique as it involves bilateral anterior meningoceles without sacral anomalies. Anterior sacral meningoce

Modern concepts of CSF disorders in monosutural craniosynostosis.

INTRODUCTION: Although hydrocephalus is randomly encountered in monosutural craniosynostosis (CS), CSF disorders may present more subtly and accompany these pictures. Indeed, dilation of subarachnoid spaces is frequently reported. The pathogenesis of these disorders is still under debate, as well as their prognostic significance. METHODS: A thorough review of the literature has been performed. Accordingly, pathogenic theories, eventual impact of surgical strategy, and prognostic significance are discussed. RESULTS: Most data come from scaphocephaly and trigonocephaly. Focal dilation of subar

Magnetic Resonance Imaging Reveals Meningeal Lymphatic Impairment in Lung Adenocarcinoma Brain Metastasis Progression.

Meningeal lymphatic vessels (mLVs) contribute to brain immune surveillance; however, their structural and functional alterations in brain metastasis remain incompletely defined. Here, we optimized a magnetic resonance imaging protocol to quantitatively analyze mLV structure and drainage function. Using murine lung adenocarcinoma brain metastasis models and clinical imaging data, we reveal significant mLV disruption surrounding the superior sagittal sinus (SSS), which results in impaired drainage to deep cervical lymph nodes (dCLNs). Among ten cervical lymph-node levels assessed clinically, lev

Running exercise mitigates amyloidosis in 5xFAD mice by improving the structure and function of the meningeal lymphatic system.

BACKGROUND: Alzheimer’s disease (AD) progression is closely linked to the accumulation of amyloid-[Formula: see text] (A[Formula: see text]), with impaired clearance mechanisms playing a key role. The meningeal lymphatic (mLym) system, which drains cerebrospinal fluid (CSF) and waste from the brain to peripheral lymph nodes, has emerged as a critical pathway for A[Formula: see text] removal. While physical exercise is known to improve cognitive function and reduce AD risk, its effect on the mLym system and downstream AD pathology have not been fully elucidated. METHODS: Three-month-old 5xFAD m

Evaluation of [18F]MK-6240 binding to tau protein in postmortem human brains of Down syndrome and Alzheimer's disease and assessment of off-target (non-tau) binding.

Alzheimer's disease (AD) and Down Syndrome (DS) are characterized by the aggregation of tau tangles. As a novel tau PET tracer in AD, [18F]MK-6240 has the potential in DSAD to elucidate pathophysiology and advance diagnostic strategies. Autoradiography of frontal cortex (FCX) and temporal cortex (TCX) postmortem brain slices of DSAD (n = 5), AD (n = 5), and cognitively normal (CN) (n = 5) cases indicated similarly high [18F]MK-6240 binding in DSAD and AD cases. Anti-tau immunostains confirmed total tau presence so there was alignment in anti-tau abundance with quantification of [18F]MK-6240 bi

Abemaciclib in meningiomas with somatic NF2 or CDK pathway alterations: the phase 2 Alliance A071401 trial.

Systemic treatments are limited for patients with meningiomas that have progressed after surgery or radiation. Loss of NF2 and CDKN2A/CDKN2B is common in higher-grade meningiomas and promotes progression in preclinical models. We evaluated the efficacy of abemaciclib, a cyclin-dependent kinase 4/6 inhibitor, as one arm of the Alliance umbrella trial A071401, a genomically driven phase 2 study in recurrent and progressive meningiomas. Eligible patients with grade 2 or 3 tumors and NF2 mutations or CDK pathway alterations were treated with abemaciclib. Two co-primary endpoints were used: progres
