Nanoparticles hijack calvarial immune cells for CNS drug delivery and stroke therapy.

The rapid accessibility of calvarial immune cells to the brain, in principle, may offer transformative opportunities for overcoming drug delivery barriers in central nervous system (CNS) disorders. Here, we hijacked calvarial immune cells using drug-loaded nanoparticles (NPs) and leveraged their unique migration mechanism through skull-meninges microchannels to bypass the blood-brain barrier (BBB) for CNS drug delivery. We constructed NP-loaded immune cells in situ via intracalvariosseous (ICO) injection, validated their prompt migration in response to CNS perturbation, and targeted therapeuti

Dural Amyloidoma Located in the Falx Cerebri and Spinal Dura: Two Case Reports.

Amyloidosis is a progressive disorder marked by the aggregation of insoluble fibrillar proteins in various tissues, leading to tissue damage. Localised amyloidosis, known as amyloidoma, is particularly rare in the central nervous system and may be mistaken for a neoplastic lesion due to similar radiological features. Consequently, the general treatment approach often involves total excision. However, it is important to consider that a biopsy of the amyloidoma, rather than total excision, may suffice for complete recovery when paired with appropriate systemic treatment. This report presents two

Spontaneous vocalisation as a presenting clinical sign in dogs: An association with cervical localisation and intervertebral disc disease.

BACKGROUND: Spontaneous vocalisation is a common reason for dogs to be presented to veterinary practices. However, the cause can be unclear, making prioritisation of investigations challenging. Although vocalisation has been associated with syringomyelia, and anecdotally with cervical disc disease, the most common causes are not reported. METHODS: The medical records of dogs presented at a single institution with spontaneous vocalisation between 2016 and 2022 were retrospectively analysed. RESULTS: A total of 115 dogs were included. The most common breeds were crossbreeds (n = 23), Cavalier

Investigator-led clinical trial of boron neutron capture therapy system for recurrent high-grade meningiomas after radiation therapy: Randomized phase II study.

BACKGROUND: High-grade meningiomas (HGMs) recurring after X-ray treatment show a poor prognosis. We assessed the effectiveness and safety of boron neutron capture therapy (BNCT) in patients with refractory recurrent HGMs. METHODS: This phase II investigator-led randomized controlled trial utilized an accelerator-based BNCT system to treat refractory recurrent HGMs. Patients were randomly assigned in a 2:1 ratio to the BNCT (12 patients) and control (6 patients) arms. Progression-free survival (PFS) judged by an independent third-party committee was the primary endpoint and PFS judged by the i

Assessing the Role of Delta Copeptin in the Evaluation of SIAD and CSW.

OBJECTIVES: The current diagnostic methods for syndrome of inappropriate antidiuresis (SIAD) and cerebral salt wasting (CSW), are imprecise. Gold standard radioisotope methodologies are currently confined to research settings. Therefore, this study evaluated whether the change in copeptin levels could be an early diagnostic tool. METHODS: A study was conducted on hyponatremic (≤ 130 meq/L) children. Subjects with endocrine disorders, systemic diseases, diarrhea, and shock were excluded. At baseline, copeptin and fractional excretion of uric acid (FEUA) were measured. Subjects then received ma

Convergent evolution of scavenger cell development at brain borders.

The vertebrate central nervous system is protected by the blood-brain barrier and meningeal membranes, which ensure immune privilege1. In the mammalian brain, microglia and barrier-associated or border-associated macrophages (BAMs) provide immune surveillance and scavenge wastes2, yet how evolution shaped immune-cell diversity and function is not understood. In zebrafish, a vascular-derived mural lymphatic endothelial cell (muLEC) lineage fulfils scavenger cell functions at central nervous system borders3-5. Here we identify the transcription factor odd-skipped related 2 (osr2) as a specific m

Infection as a potential trigger in glial fibrillary acidic protein astrocytopathy: a case report and review of immunotherapeutic strategies.

PURPOSE: Glial fibrillary acidic protein astrocytopathy (GFAP-A) is a form of autoimmune encephalitis (AE) that commonly affects the central nervous system (CNS). The pathogenesis of GFAP-A remains unclear, with infection proposed as a potential trigger, often leading to a misdiagnosis as meningoencephalitis due to overlapping clinical manifestations. This study aims to reports a case of GFAP-A associated with Enterococcus faecium infection, summarize the clinical characteristics, treatment details, and prognostic outcomes of GFAP-IgG-positive patients with concurrent infections, and provide e

Meningeal metastatic tumor with bone destruction from follicular thyroid carcinoma: a case report and literature review.

BACKGROUND: Follicular thyroid carcinoma (ftc) is a malignant neoplasm arising from thyroid follicular epithelial cells and is the second most common thyroid cancer after papillary thyroid carcinoma. meningeal metastasis from ftc with calvarial destruction is exceedingly rare and is often misdiagnosed clinically as meningioma. we reviewed previously reported cases in the literature and, in addition, report and include one case of meningeal metastasis from ftc treated at our center. METHODS: Using the keywords "meningioma," "follicular thyroid carcinoma," and "cerebral metastatic tumor," we se

Investigation of the rimegepant effect on cerebral and extracerebral arteries during migraine attacks: a longitudinal magnetic resonance angiography study.

Migraine is a leading cause of disability worldwide, and triptans, the most widely used acute treatment, act through vasoconstriction and are contraindicated in patients with vascular disease. Rimegepant, a calcitonin gene-related peptide receptor antagonist, is proposed as a non-vasoconstrictive alternative, but its direct vascular effects during spontaneous migraine attacks have not been examined. This was a prospective, longitudinal study conducted at a single academic imaging centre between 12 January 2024 and 10 June 2025. Eighteen women aged 18-40 years with menstrually related migraine

Survival of lung cancer patients with central nervous system metastases: from clinical trials to mouse models.

BACKGROUND: Central nervous system (CNS) metastasis is common in patients with non-small cell lung cancer (NSCLC) and is associated with poor prognosis. Evidence guiding optimal clinical management remains limited, and comprehensive studies on animal models that recapitulate different CNS metastatic patterns are lacking. METHODS: A total of 316 NSCLC patients with CNS metastases were retrospectively enrolled between May 2019 and February 2024. Patients were categorized into four groups: brain metastases only (BM, n = 145), leptomeningeal metastases only (LM, n = 43), concurrent brain and lept

Physiological Effects of Photobiomodulation Therapy in the Treatment of Multiple Concussions.

Repeated concussion traumatic brain injury (TBI) results in long-term brain damage and cognitive dysfunctions, leading to neurodegenerative diseases. The brain clearance system plays a crucial role in TBI recovery and neurodegenerative disease amelioration by draining waste macromolecules from the brain. Pharmacological therapeutics have failed to demonstrate benefits in human TBI. Photobiomodulation (PBM) has gained interest in neuroscience and has been shown to improve brain drainage. Here, we evaluated the efficiency of PBM in the treatment of multiple concussions in mice and the augmentati

Deficiency of SMARCB1 drives an immunosuppressive microenvironment in meningioma.

BACKGROUND: Meningiomas are the most common primary intracranial tumor in adults and systemic therapy is urgently needed for high-grade fatal tumors and those cannot be completely removed by surgery. Multiomics studies have established a molecular classification system in addition to the grading system by the World Health Organization, and SWI/SNF-related BAF chromatin remodeling complex subunit B1 (SMARCB1) mutation was enriched in the immunogenic subgroup. Meningiomas are myeloid-dominant tumors with abundant and unevenly distributed CD163+ macrophages, a feature linked to intratumoral heter

Local metastatic expansion versus secondary intra-organ dissemination: two causes of neurological death explained by fundamentally different metastatic colonization patterns.

BACKGROUND: Neurological failure contributes to 15-50% of deaths in patients with brain metastases, yet the underlying mechanisms remain poorly understood. Clinical causes range from local compression to meningeal metastasis. In this context, a link between infiltrative histopathological growth patterns (HGPs) and meningeal metastasis was recently described and prompted this reverse translation study. METHODS: We conducted a retrospective postmortem histological assessment and a prospective MRI-based proof-of-concept study to explore neurological decline mechanisms in two experimental brain m

Preliminary assessment of an MRI-based grading system for leptomeningeal disease: an exploratory prognostic framework.

BACKGROUND: Current diagnostic approaches of leptomeningeal disease (LMD) rely heavily on cerebrospinal fluid (CSF) cytology, which shows significant limitations and the requirement for invasive procedures. We aim to develop an MRI-based grading scores for LMD diagnosis and prognosis that address current diagnostic limitations and provide standardized, reproducible assessment criteria. METHODS: We conducted a retrospective analysis of 32 adult cancer patients evaluated for suspected LMD. Two experienced neuroradiologists independently assessed MRI studies using our novel grading system, which

Plasma cells in and around the central nervous system.

Secreting a continuous, and sometimes life-long antibody supply, plasma cells are the effector arm of humoral immune system. With an incredibly diverse array of binding specificities, antibodies play critical roles in homeostasis and disease. Traditional views of plasma cells have them function at a distance, relying on circulation to ferry their antibodies to peripheral tissues. However, this review focuses on plasma cells that operate locally within tissues that lack ready access to circulating antibody, specifically, we explore plasma cells that accumulate within the central nervous system
