[How to Proceed with Diagnosing Memory Loss].

When diagnosing dementia disorders, obtaining a detailed medical history and identifying relevant findings on physical and neurological examinations are fundamental. It is essential to assess cognitive function, neurological symptoms and signs, and dementia severity; conduct additional tests; and establish a differential diagnosis. With the widespread use of anti-amyloid antibody therapies, mild cognitive impairment (MCI) due to Alzheimer's disease (AD) and early-stage AD must not be overlooked. When evaluating patients with dementia who present with diverse symptoms, clinicians must consider

Spinal Muscular Atrophy in Adult Neurology Services in India.

Spinal muscular atrophy (SMA) research has focused predominantly on paediatric populations, with limited adult data from low- and middle-income countries (LMICs). We prospectively recruited suspected SMA patients (age ≥ 12 years) into a neuromuscular disease cohort at a tertiary centre in northern India, using phenotyping, creatine kinase, electrophysiology and genetic testing. Forty genetically confirmed patients were included (median age 20.5 years; mean symptom duration 14.7 ± 10.9 years). Proximal lower-limb weakness was the commonest presentation (82.5%). Forty-five percent were misdiagno

Neural Stem Cell-Derived Extracellular Vesicles Improve CNS Delivery and Therapeutic Efficacy of Antisense Oligonucleotide in Amyotrophic Lateral Sclerosis.

Antisense oligonucleotides (ASOs) hold clinical promise for the treatment of neurodegenerative diseases such as amyotrophic lateral sclerosis (ALS). Yet, their efficacy is constrained by the need for invasive delivery, poor central nervous system (CNS) penetration, inefficient cellular uptake and rapid clearance. Here, we developed neural stem cell-derived extracellular vesicles (NSC-EVs) as natural carriers to enhance CNS targeting and therapeutic efficacy of ASO-based treatments for ALS. Using a passive loading method, therapeutic ASOs were efficiently associated with NSC-EVs (NSC-EV-ASO). C

Severe adenylosuccinate lyase deficiency with early autonomic dysfunction: functional characterization of a novel ADSL variant and exploratory treatment with disulfiram.

Adenylosuccinate lyase (ADSL) deficiency (MIM# 103050) is a rare inherited disorder of purine metabolism characterized by neurodevelopmental impairment, epilepsy, and accumulation of succinylpurines. The clinical spectrum ranges from severe prenatal manifestations with congenital anomalies to mild intellectual disability. We report a child with a severe form of ADSL deficiency presenting with early autonomic dysfunction, including recurrent apnea and bradycardia beginning in the neonatal period, followed by pharmacoresistant epilepsy and profound developmental delay. Exome sequencing identifie

Consecutive Emergency Carotid Artery Stenting and Transcatheter Aortic Valve Replacement in a Patient With Cardiogenic Shock: A Case Report.

Severe aortic stenosis (AS) carries a poor prognosis when symptomatic, particularly in cardiogenic shock (CS). Emergency transcatheter aortic valve replacement (TAVR) is rarely performed but may represent a rescue therapy. Coexistent carotid artery stenosis (CAS) further increases procedural risk. We report the case of an 83-year-old female patient with severe AS and beginning CS. Imaging revealed severe right internal CAS and occlusion of the left internal carotid artery. Despite the use of norepinephrine, the patient progressed to deteriorating CS with acute kidney injury and impaired mental

Biomarkers for myelin oligodendrocyte glycoprotein-associated disease: recent advances and future.

MOGAD is a rare autoimmune encephalomyelitis that shares similar clinical manifestations with other central nervous system demyelinating diseases. However, they must be distinguished due to their distinct pathogenesis and treatment methods. Currently, the clinical diagnosis of MOGAD primarily relies on patients' clinical manifestations and imaging features, with biomarker application being relatively limited. Meanwhile, research on MOGAD biomarkers has advanced rapidly, with findings mainly focusing on imaging features, antibody titers, nerve cell markers, cytokines, and inflammatory cell rati

Effects of Different Numbers of Stimulations in Single-Session Radial Extracorporeal Shock Wave Therapy on Spasticity Improvement in Individuals With Subacute Hemiparetic Stroke: Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Radial extracorporeal shock wave therapy (rESWT) improves lower-limb spasticity after stroke. However, the efficacy of rESWT in individuals with hemiparetic stroke in the subacute phase, in whom neural components of spasticity are thought to predominate, has not been sufficiently investigated. In addition, the effects of different numbers of stimulations on the magnitude and duration of spasticity improvement remain unclear. OBJECTIVE: We aim to explore the effects of different numbers of stimulations during a single rESWT session on lower-limb spasticity in individuals with subac

Integrated single-cell and Mendelian randomization analyses identify aging-induced brain endothelial SPARCL1 deficiency as a key driver of vascular dementia.

BACKGROUND: Vascular dementia (VaD) is a prevalent age-related neurocognitive disorder characterized by progressive cognitive impairment and a lack of disease-modifying therapies. Although age-related brain endothelial dysfunction has been implicated as a central contributor to the pathogenesis of VaD, the molecular mechanisms underlying this process remain incompletely elucidated. METHODS: We employed an integrative analytical framework combining Mendelian randomization (MR) and single-cell RNA sequencing (scRNA-seq). MR analysis was performed using plasma protein quantitative trait loci (pQ

Quantitative Relationship Between White Matter Hyperintensity Volume and Fazekas Score on Brain MRI.

BACKGROUND: The Fazekas score is widely used to grade white matter hyperintensities (WMHs) in cerebral small vessel disease, yet the equivalent volume of each grade is unclear. We quantified the correspondence between Fazekas scores and WMH volume, normalized ratios, and derived conversion equations across populations. METHODS: We analyzed 1220 participants from 4 mostly United Kingdom-based cohorts representing different cerebral small vessel disease severities: community-dwelling (LBC1936 [Lothian Birth Cohort 1936]), stroke (MSS2 [Mild Stroke Study 2] and MSS3 [Mild Stroke Study 3]), and a

A robust HPLC-HILIC enrichment system for O-glycoproteomics profiling of plasma and cerebrospinal fluid across Alzheimer's disease progression.

BACKGROUND: Alzheimer's disease (AD) pathogenesis involves widespread molecular alterations in both brain tissue and peripheral biofluids, yet the role of protein O-linked glycosylation in disease progression remains poorly characterized due to technical limitations in comprehensive O-glycoproteome profiling. RESULTS: To address this gap, we developed a robust high-performance liquid chromatography-hydrophilic interaction chromatography (HPLC-HILIC) system optimized for efficient enrichment of intact O-glycopeptides from protein digests of complex biological matrices. This system was applied

Microglia deploy TAM receptors to kill motor neurons in a mouse model of amyotrophic lateral sclerosis.

Activation of microglia is a prominent feature of amyotrophic lateral sclerosis (ALS), a neurodegenerative disease that leads to the death of motor neurons. A key component of this activation is elevated expression of the TAM receptor tyrosine kinases Axl and Mer (gene name Mertk). Here we show that germline and microglial-restricted inactivation of the Axl and Mertk genes in the SOD1G93A mouse model of ALS leads to an extension of lifespan, which is tied to the preservation of cholinergic motor neurons and neuromuscular synapses. Also elevated on SOD1G93A neuronal surfaces is the essential TA

Multimodal characterization of age-dependent neurovascular phenotypes in Notch3R170C CADASIL mice by ultra-high-field MRI and histological analyses.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a hereditary small vessel disease caused by mutations in NOTCH3 and characterized by progressive neurovascular alterations. Although magnetic resonance imaging (MRI) can detect vascular and white matter abnormalities in CADASIL, the relationship between imaging findings and underlying cellular and molecular changes remains incompletely understood. Here, we established an integrative, multi-scale framework combining ultra-high-field MRI, quantitative histology, and single-cell RNA sequencing

Alzheimer's disease burden in China (ABC) study: protocol for a nationwide multicentre cross-sectional and prospective cohort study.

INTRODUCTION: Alzheimer's disease (AD) is imposing an increasing public health and socioeconomic burden. In China, rapid population ageing is sharply increasing disease burden. Previous studies have shown that AD-related costs are mainly driven by long-term informal care. However, evidence in China remains limited by an incomplete cost framework and insufficient consideration of caregivers' burden and indirect costs. Notably, the National Dementia Action Plan (2024-2030), issued by the Chinese government, marks a major shift to early detection and comprehensive care of AD, highlighting the urg

Retinal Embolization after Carotid Artery Stenting in a Monocular Patient: Clinical and Ethical Implications.

Severe carotid artery narrowing is a well-recognized cause of ischemic stroke, and carotid artery stenting (CAS) is frequently used to restore blood flow and prevent recurrence. Although embolic protection devices are designed to reduce the risk of distal embolization, ocular complications can still occur. When treating patients with vision in only one eye, any compromise in ocular circulation can have devastating consequences. A 63-year-old male presented with an acute ischemic stroke caused by severe narrowing of the Right internal carotid artery. He underwent CAS. Shortly after the procedur
