Iohexol was introduced by lumbar puncture in a series of 148 consecutive children aged between 5 days and 16 years referred for myelography; no patient was excluded. Initially, iohexol 180 mgI/ml was used in dosage proportional to body weight varying between 5 ml and 15 ml. Pressure resistance of an experimental epidural blood patch was studied in vitro. Pieces of canine lumbar dura were perforated with a 19-gauge needle (n = 12) or a 25-gauge needle (n = 6) and kept between the intercommunicating chambers of a plexiglass apparatus. A review of the most recent 23 cases of benign intracranial hypertension (BIH), admitted to the War Memorial Children's Hospital in London, Ontario, provided a compendium of the clinical manifestations of this disorder in children and adolescents. Fast deterioration of the neurological status of patients with raised intracranial pressure after lumbar puncture is not rare. Cerebral coning or rebleeding occur in consequence of cerebrospinal fluid leak. An experimental model correlates leakage with needle gauge. Computed tomography is indicated for diagnosis at the time of the initial examination. Puncture with needles of small diameter (less than 0.5 mm) is recommended.

Within 20 months 145 ascending thoracocervical myelographies and 155 lumbar myelographies with the nonionic water-soluble contrast medium iopamidol were performed. The iodine concentration given was 250 mg I/ml or 200 mg I/ml respectively. The total iodine never exceeded 2.5 g (8-10 ml). A prospective investigation of the adverse effects of myelography with metrizamide was performed in a series of 250 examinations in infants, children and adolescents. Adverse reactions occurred in 58 per cent of all cases. The majority of these were mild, and seemed to be due to the lumbar puncture alone. The frequency of the moderate and marked reactions was directly related to the total amount of metrizamide given and to the cranial extension of the examination in the spinal canal. No other circumstances around the procedure had any impact on the occurrence of adverse reactions.

A series of 605 myelographies with metrizamide was reviewed to assess the risk of local injury caused by the puncture and injection of metrizamide. A lesion below the normal level of the medullary conus was found in 139 patients, and in 10 of these the needle went into or through the lesion. One patient developed paraplegia due to the puncture and injection, no other patient had any symptoms. It is concluded that in spite of the risk, lumbar puncture should be the routine procedure for myelography with metrizamide in children.

Ketamine, a cataleptic analgesic substance which theoretically may block pain receptors in the spinal cord, providing pain relief without respiratory depression, was injected intrathecally with or without benzethonium chloride 0.1 mg/ml (a preservative) into 8 monkeys under anaesthesia. A population of 28 patients underwent the percutaneous placement of a shunt in the lumbar region with a single tube, the other end of which was placed in the peritoneal cavity by laparotomy. A patient with eosinophilic meningitis in Shizuoka Prefecture, Honshu, Japan had nausea, vomiting and headache on admission; laboratory examinations revealed leukocytosis with eosinophilia. Continuous spinal drainage of cerebrospinal fluid was used in 50 patients with CSF fistulas both traumatic and postoperative. The method seemed to be helpful in the healing of leaks, thereby rendering surgery unnecessary. It also helped the surgical sealing of long standing tears.

The experiences of more than 200 cisternographic examination with 111Indium-DTPA are reported. The radiopharmaceutical was tolerated without adverse reactions and the suboccipital intrathecal injection more appropriate than the lumbar. With 111In-DTPA excellent scans were obtained up to 48 h after application. 111In is compared with other radiopharmaceuticals for cisternography. The cisternography remains valuable as an additional examination of pneumencephalography and cranial computerized tomography.

The current frequency of road accidents leads to the necessity to seek a possible head injury in the past history of patients suffereing from acute purulent meningitis. This investigation must be all the more assiduous in the presence of a meningitis which is pneumococcal in origin and recurrent. Rhinorrhoea is a sign of primary importance. Tomography of the frontal and sagittal views of the skul remains of value. Radio-isotope studies of the CSF happile make up for the deficiencies of radiological examination, without providing an infallible means for the detection of osteomeningitis defects.

Following inadvertent spinal anesthesia for delivery, a patient developed incapacitating post-lumbar puncture headache that persisted for 9 weeks. Scintigrams of the lumbar region, obtained after injection of 99mTC-human serum albumin into the cisterna magna, showed the cerebrospinal fluid leak. In these two patients, as in 13 others described in the literature, a typical meningitic syndrome with excitement, confusion and a fever of 104degrees F (40degrees C) appeared 4-6 hours following the intra-thecal injection.
