Diagnostic Yield of CT Myelography for the Identification of CSF Leaks in Patients with Post Dural Puncture Headache.

BACKGROUND AND PURPOSE: Post dural puncture headache (PDPH) refractory to conservative measures can be clinically debilitating, and diagnostic evaluation with spinal imaging including CT myelography (CTM) may be performed to localize a CSF leak and guide treatment. The purpose of this study is to determine the diagnostic yield of CTM for detecting a CSF leak in patients with persistent PDPH. MATERIALS AND METHODS: This is a single-center, retrospective cohort study of patients referred for CTM between September 2013 and August 2025 to localize and/or identify a CSF leak in patients with PDPH.

Fluoroscopic guidance decreases the risk of post-lumbar puncture headache in patients with idiopathic intracranial hypertension.

BACKGROUND: Information derived from lumbar puncture (LP) remains fundamental in the diagnosis of idiopathic intracranial hypertension (IIH). However, almost all patients with IIH are obese, rendering LPs technically difficult. The number of attempts required to achieve success and the likelihood of post-LP headache due to cerebrospinal fluid (CSF) leak has not been well characterized in this specific patient population or by LP modality. METHODS: We conducted a retrospective chart review of IIH patients seen at a single neuro-ophthalmology clinic who underwent LP, either guided by convention

Post-dural Puncture Headache Complicated by Subdural Hematoma in an Elderly Patient Following a Pencil-Point Spinal Needle Anesthesia: A Case Report.

Spinal anesthesia using a pencil-point needle in older patients is a commonly performed procedure. Post-dural puncture headache (PDPH) is a complication that can happen following spinal anesthesia due to cerebrospinal fluid (CSF) leakage, but its incidence in elderly patients is rare. While PDPH presents with typical symptoms and resolves with conservative management, in older patients, it can happen along with intracranial subdural hematoma (SDH), a rare but potentially life-threatening complication due to the persistent CSF leak. We report a rare case of a 73-year-old woman who presented wit

Post-dural Puncture Headache: Pathophysiology, Risk Stratification, Prevention, and Evidence-Based Management for Practicing Anesthesiologists.

Post-dural puncture headache (PDPH) is a well-recognized secondary headache disorder resulting from persistent cerebrospinal fluid (CSF) leakage following iatrogenic dural disruption during neuraxial procedures, including spinal anesthesia, diagnostic lumbar puncture, dural puncture epidural (DPE), and unintentional dural puncture (UDP). The syndrome reflects a complex interaction between CSF hydrodynamics, craniospinal compliance, vascular compensation, and nociceptive sensitization. Reduction in CSF volume produces intracranial hypotension and compensatory venous dilation in accordance with

Persistent headache burden after surgical treatment of post-dural puncture headache: a cross-sectional study.

BACKGROUND: Post-dural puncture headache (PDPH) may follow intended or unintended spinal dural puncture and is usually self-limiting. In some patients, however, symptoms persist despite epidural blood patches, while postoperative patient-reported data are largely absent. We assessed headache disability, functional status, and psychosocial burden among respondents self-reporting prior surgery for PDPH or dural-puncture-related CSF leak. METHODS: An online cross-sectional survey was conducted between February and April 2025. Adults who self-reported prior surgical treatment for PDPH or dural-pu

Post-Dural Puncture Headache with Abducens Nerve Palsy after Epidural Labor Analgesia: Successful Treatment with CT-Guided Epidural Blood Patch - A Case Report.

INTRODUCTION: Post-dural puncture headache is a well-recognized complication of neuraxial anesthesia caused by cerebrospinal fluid leakage and intracranial hypotension. Although usually self-limiting, persistent post-dural puncture headache may rarely be complicated by cranial nerve palsy. CASE PRESENTATION: We report the case of a 33-year-old postpartum woman who developed severe orthostatic headache after apparently uncomplicated epidural labor analgesia, followed by neck stiffness, auditory pressure, and horizontal diplopia due to abducens nerve palsy. Initial cranial computed tomography w

Successful venous thrombectomy for extensive cerebral venous and sinus thrombosis after failed diagnostic lumbar puncture.

With an incidence of 2-5 per million adults, cerebral venous and sinus thrombosis (CVST) is a rarity in the spectrum of cerebrovascular diseases. The etiology and symptomatic presentation are heterogeneous and diverse. CSVT is, therefore, often underdiagnosed. In addition to therapeutic anticoagulation, venous thrombectomy is a last-resort therapy in individual cases.A 42-year-old woman was admitted to the hospital with postural headache following an unsuccessful lumbar puncture. On suspicion of post-puncture syndrome, analgesic therapy with adjuvant caffeine tablets was initially chosen, lead

Trends in Medicare procedural and reimbursement rates for spinal CSF leak repair (2000-2021).

OBJECTIVE: CSF leaks are a common complication of spinal surgery, occurring in 3%-16% of elective cases and up to 12% of operatively managed traumatic spinal injuries. They can also occur following lumbar puncture. However, there are limited data on utilization and reimbursement trends within Medicare for CSF leak repairs. Characterizing the economic burden and market of spinal CSF leak repair has implications for optimizing effective management strategies. This study evaluates reimbursement and utilization trends for both surgical repair and epidural blood patch (EBP) management of spinal CSF

Effect on the size of optic nerve sheath diameter in patients undergoing surgeries under spinal anaesthesia versus peripheral nerve blocks - A randomised controlled study.

BACKGROUND AND AIMS: Post-dural puncture headache is a complication of spinal anaesthesia, theorised to be triggered by a lowering in intracranial pressure due to the cerebrospinal fluid leak through the dural puncture. Our objective was to evaluate whether there is a decrease in optic nerve sheath diameter (ONSD) with a reduction in intracranial pressure after spinal anaesthesia. METHODS: Patients were randomised by a computer-generated randomisation table to receive spinal anaesthesia (Group S) or peripheral nerve block (Group P) after assessing their eligibility for the anaesthesia procedu

Intravenous Contrast-Enhanced MR Myelography in CSF Leakage for the Detection of Spinal CSF Lamellae.

BACKGROUND AND PURPOSE: Intracranial hypotension (IH) results from cerebrospinal fluid (CSF) leakage from the dural sac, occurring spontaneously or iatrogenically (e.g., post-lumbar puncture), and may cause a wide range of symptoms with significant functional impairment. Accurate detection of the epidural CSF lamella is key to diagnosis. This study evaluated the diagnostic value of intravenous contrast-enhanced MRI using heavily T2-weighted FLAIR (HT2-FLAIR) spine imaging compared to nonenhanced MR myelography at 3 Tesla. METHODS: Ten consecutive patients with IH symptoms were prospectively e

Case Report of a Rare but Serious Complication in Anesthesia: Arachnoiditis Following an Epidural Blood Patch.

The epidural blood patch (EBP) is an invasive technique used to plug a leak of cerebrospinal fluid, usually following an iatrogenic lesion of the dura mater. This injection of autologous blood into the epidural space is associated with a number of rare but potentially serious complications. We present a case of arachnoiditis in a young parturient who presented with symptoms compatible with intracranial hypotension due to an accidental dural puncture during an obstetric epidural, and who was treated with two epidural blood patches. The diagnosis of arachnoiditis was suspected based on the neuro

Surgical success of subcutaneous systems for intrathecal nusinersen delivery in spinal muscular atrophy: A systematic review and meta-analysis.

BACKGROUND: The intrathecal administration of nusinersen to treat spinal muscular atrophy (SMA) has demonstrated therapeutic effect, however, repeat lumbar punctures confer multiple disadvantages. As such, subcutaneous systems have been described to provide an alternative, less invasive strategy for repeat administration. Correspondingly this study aimed to aggregate metadata to better define the surgical success of these subcutaneous systems. METHODS: Searches of multiple electronic databases from inception to March 2025 were conducted following PRISMA guidelines. Articles were screened agai

CSF Pressure and Dynamics in Patients With Chronic Postdural Puncture Headache: A Single-Center Cohort Study.

BACKGROUND AND OBJECTIVES: Chronic orthostatic headache after dural puncture is a serious condition lacking defined diagnostic criteria and pathophysiologic understanding. Prevailing opinion suggests CSF outflow through an unsealed dural hole after a lumbar puncture. We aimed to systematically search for a leak with MRI and perform dynamic CSF infusion testing in patients with chronic orthostatic headache after a lumbar puncture. METHODS: In a retrospective, single-center cohort study at the Medical Center-University Hospital Freiburg, patients with chronic postdural puncture headache (cPDPH)
