Cerebrospinal Fluid Refill Test.

OBJECTIVE: Cerebrospinal fluid (CSF) hypovolemia is often difficult to diagnose because reduced CSF pressure and radiographic findings are not consistently observed. To address this limitation, we developed the CSF refill test, a novel diagnostic procedure based on temporary CSF replenishment and evaluation of symptom improvement. This study reports preliminary and prospective results of this method. METHODS: We analyzed 27 patients with suspected CSF hypovolemia. Seventeen cases (2012-2019) were retrospectively assessed during procedures such as computed tomography myelography and the overfl

Successful spinal anaesthesia for caesarean birth after epidural blood patch for intracranial hypotension headache.

Headache in pregnancy remains a diagnostic challenge. Neuraxial intervention in the antenatal period can lead to complications, such as postdural puncture headache and subdural haematoma, and may have a significant impact on maternal well-being, labour and birth. Here, we describe a case of headache secondary to intracranial hypotension with onset at 32 weeks of gestation in a 40-year-old woman, 12 weeks following spinal anaesthesia for cervical cerclage. It presented novel problems to consider for the performance of lumbar epidural blood patch and the subsequent neuraxial anaesthesia for the

The contribution of lumbar puncture opening pressure in the diagnosis of spontaneous intracranial hypotension: A systematic literature review and meta-analysis.

OBJECTIVE: The objective of this study was to summarize the available evidence regarding the clinical value and trend over time of lumbar cerebrospinal fluid (CSF) opening pressure utilization to diagnose spontaneous intracranial hypotension (SIH). BACKGROUND: CSF opening pressure obtained via lumbar puncture is one of the diagnostic criteria for SIH based on the International Criteria for Headache Disorders, 3rd Edition (ICHD-3), but it has questionable utility as an initial investigation for diagnosing SIH. METHODS: The authors performed a systematic literature review and meta-analysis. Pu

Cerebrospinal Fluid Cutaneous Fistula Following Neuraxial Anesthesia in a Placenta Previa Case: A Rare Complication.

Cerebrospinal fluid (CSF) cutaneous fistula is a rare complication of neuraxial anesthesia, caused by CSF leakage through the epidural puncture site, usually after catheter removal. It may present with symptoms of intracranial hypotension, but some cases remain asymptomatic, making diagnosis and management less straightforward. Evidence is scarce, mostly from case reports, and no standardized treatment exists. Our case emphasizes that early recognition and a stepwise, minimally invasive approach can effectively manage cerebrospinal fluid fistulas, even in asymptomatic obstetric patients. We pr

CT-guided percutaneous cyanoacrylate injection targeting the spinal cerebrospinal fluid leak: a potential therapeutic option for spontaneous intracranial hypotension.

BACKGROUND: We report the first case series of spontaneous intracranial hypotension (SIH) patients who underwent CT-guided percutaneous cyanoacrylate injection targeting the cerebrospinal fluid (CSF) leak. METHODS: A retrospective analysis was performed for all consecutive cases of SIH patients with CSF leak confirmed on CT myelography, treated by CT-guided percutaneous cyanoacrylate injection at our institution from 2016 to 2022. On pretreatment brain and spine MRIs, we analyzed signs of SIH according to the Bern score, and dichotomized cases into positive/negative for spinal longitudinal ex

Mechanism of chronic iatrogenic CSF leak following dural puncture-ventral dural leak: case report.

BACKGROUND: Postdural puncture headache has been traditionally viewed as benign, self-limited, and highly responsive to epidural blood patching (EBP) when needed. A growing body of data from patients experiencing unintended dural puncture (UDP) in the setting of attempted labor epidural placement suggests a minority of patients will have more severe and persistent symptoms. However, the mechanisms accounting for the failure of EBP following dural puncture remain obscure. An understanding of these potential mechanisms is critical to guide management decisions in the face of severe and persisten

Intrathecal hematoma following epidural blood patch: An alternative mechanism to intrathecal puncture.

Intrathecal blood after a percutaneous epidural blood patch is a known complication, one that has been previously attributed to inadvertent needle tip position in the subarachnoid space. We present two cases with imaging confirmation of an alternative mechanism, one that includes blood transferring from the epidural space to the subarachnoid space through a pre-existing dural defect.

Spontaneous Intracranial Hypotension: Clinical Presentation, Diagnosis, and Treatment Strategies.

Spontaneous intracranial hypotension (SIH) typically presents as an acute orthostatic headache during an upright position, secondary to spinal cerebrospinal fluid leaks. New evidence indicates that a lumbar puncture may not be essential for diagnosing every patient with SIH. Spinal neuroimaging protocols used for diagnosing and localizing spinal cerebrospinal fluid leaks include brain/spinal MRI, computed tomography myelography, digital subtraction myelography, and radionuclide cisternography. Complications of SIH include subdural hematoma, cerebral venous thrombosis, and superficial siderosis

Pediatric post-dural puncture headache and paraplegia.

A cerebrospinal fluid (CSF) leak developed in a 14-year-old girl and a 12-year-old boy following a diagnostic lumbar puncture. Two days and sixteen years later, respectively, paraplegia developed due to a functional disorder. Imaging revealed an extensive extradural CSF collection in both patients and digital subtraction myelography was required to pinpoint the exact site of a ventral dural puncture hole where the lumbar spinal needle had gone "through and through" the dural sac. The CSF leak was complicated by cortical vein thrombosis in one patient. Both patients underwent uneventful surgica

A Novel Makeshift Method to Collect Leaking Epidural Fluid: A Case Report.

A healthy 34-year-old full-term parturient was admitted to the labor suite where a combined spinal-epidural (CSE) was easily placed on the first attempt for labor analgesia. After an uneventful delivery, the epidural catheter was removed. Two days later, the patient experienced a fluid leak from the puncture site. The fluid was analyzed to determine whether it was an interstitial or a cerebrospinal fluid (CSF). We describe a novel technique to collect the leaking fluid without admixing fluid from the surrounding area. No previous reports describe a similar technique to diagnose the source of t

At-Home "Tips" for Headache Management: The Use of Self-Trendelenburg to Help Determine the Cause of Worsening Headaches - A Case Series.

INTRODUCTION: In-office use of the Trendelenburg position has been shown to be a beneficial clinical tool to help decipher if a CSF pressure/volume component is part of the underlying etiologic process for a patient's persistent headache. Utilizing the Trendelenburg position at home could potentially be an additional diagnostic tool for the treating headache physician. CASE SERIES: Our headache practice has been using at-home self-Trendelenburg for the past 2 years and will present the clinical scenarios in which it seems to be the most helpful utilizing a case series of patients. Spontaneous intracranial hypotension mimicking iatrogenic spinal cerebrospinal fluid leaks.

OBJECTIVE: To raise awareness that patients with persistent post-dural puncture headache should be considered for evaluation of spontaneous cerebrospinal fluid (CSF) leak. BACKGROUND: Spontaneous intracranial hypotension (SIH) due to a spinal CSF leak may occur following more-or-less trivial traumatic events. We report our experience with spontaneous spinal CSF leaks that occur following percutaneous or open spine procedures, a potential source of diagnostic confusion. METHODS: In a retrospective cohort study, using a prospectively maintained database of patients with SIH, we identified all

Imaging Anatomy of the Cerebrospinal Fluid Spaces.

The cerebrospinal fluid spaces of the spine and brain are an intricate network of tissues with complex anatomic relationships. Understanding the normal imaging anatomy and variants of these spaces is crucial for accessing the spinal subarachnoid space and evaluating patients with suspected CSF leaks. This article reviews the imaging anatomy of the cerebrospinal fluid spaces in the spine and brain with emphasis on clinically relevant anatomy for percutaneous needle access to the spinal subarachnoid space and management of patients with CSF leak.

Fluoroscopic, Computed Tomographic, and Magnetic Resonance Myelography.

Despite all the advantages of magnetic resonance (MR) imaging, there still exist contraindications or limitations to its use. Thus, MR imaging has not entirely replaced fluoroscopic or computed tomographic (CT) myelography to depict the outline of the spinal cord and its nerve roots after intrathecal injection of contrast medium. The growing recent interest of neuroradiologists to accurately diagnose and treat cerebrospinal fluid leaks has also driven a resurgent need for familiarity with this image-guided procedure. This article reviews the numerous technical and periprocedural aspects of flu

Complications of cerebrospinal fluid drainage in thoracoabdominal aortic procedures.

BACKGROUND: Cerebrospinal fluid (CSF) drainage is used to reduce spinal cord ischemia (SCI) in patients undergoing thoracoabdominal aortic procedures. Recent literature has found high rates of complication associated with CSF drainage, which has led to changes in practice. The aim of this study was to investigate rates of CSF drain-related complications in patients undergoing a thoracoabdominal aortic procedure with perioperative placement of a CSF drain. METHODS: We conducted a single-centre retrospective cohort study. We defined major complications as intracranial hemorrhage, epidural hemat
