Impact of decentralised, community-led molecular STI point-of-care testing on testing practices in remote First Nations communities in Australia.

OBJECTIVE: In remote Australia, where there is a substantial burden of sexually transmitted infections (STIs) among First Nations peoples, testing and timely treatment are key to reducing prevalence. We evaluated the impact of introducing molecular point-of-care (POC) testing for chlamydia/gonorrhoea and later trichomonas on STI testing (including syphilis) and positive tests. METHODS: We conducted a retrospective interrupted time-series analysis of routinely collected data from First Nations peoples aged 15-54 years attending 20 remote/regional clinics participating in the Test, Treat and GO

Evaluation of the Hologic Aptima Vaginitis assays for microbiological diagnosis of vaginitis in women with abnormal vaginal discharge attending primary care.

Introduction. Infectious vaginitis, caused mostly by bacterial vaginosis (BV), Candida vaginitis (CV) and Trichomonas vaginitis (TV), is a common reason for primary care visits. Recent studies demonstrate the potential utility of molecular assays in replacing conventional diagnostic methods.Hypothesis/Gap Statement. Different diagnostic standards are in use for BV; published studies have evaluated molecular diagnostic tests against Amsel criteria, Nugent score or a combination of both. In the UK, Hay/Ison criteria are commonly used, but there are limited studies evaluating molecular diagnostic

Correlates of Sexually Transmitted Infection Positivity and Implementation of Point-of-Care Sexually Transmitted Infection Testing at a Mobile Clinic for Women Who Inject Drugs in Seattle, WA.

BACKGROUND: Women who inject drugs (WWID) face complex barriers to engagement in sexual healthcare. We used point-of-care (POC) testing to assess sexually transmitted infection (STI) prevalence, correlates, and treatment delivery, as well as testing acceptability and feasibility, at a mobile clinic for WWID in Seattle, Washington. METHODS: We implemented a mobile clinic offering substance use and sexual health services for women ≥18 years with a history of injecting drugs and negative or unknown HIV status. We offered POC testing for trichomonas, syphilis, chlamydia, gonorrhea, and HIV at enr

Cervicovaginal cytokine profiles and their associations with colposcopic findings in adolescent girls and adult women in South Africa: an exploratory cross-sectional study.

BACKGROUND: Colposcopy is used to evaluate cervical inflammation and detect precancerous and other cervical lesions; however, access remains limited in many low-resource settings. Identification of cervicovaginal immune biomarkers associated with specific colposcopic findings may provide insight into mucosal immune alterations linked to increased susceptibility to sexually transmitted infections (STIs) and human immunodeficiency virus (HIV), although these associations do not imply diagnostic or predictive utility in a cross-sectional context. METHODS: In this exploratory cross-sectional stud

High prevalence of sexually transmitted infections among adolescent girls and young women seeking HIV testing in Entebbe, Uganda.

BACKGROUND: Adolescent girls and young women (AGYW) in Uganda experience a high burden of sexually transmitted infections (STIs), increasing vulnerability to HIV acquisition. Despite the WHO and Uganda's Ministry of Health recommendations, routine integration of STI screening into HIV testing services (HTS) remains limited. We assessed the prevalence of laboratory-confirmed STIs, associated factors and Neisseria gonorrhoeae antimicrobial resistance among AGYW seeking HTS in Entebbe, Uganda. METHODS: We conducted a cross-sectional study among 321 AGYW aged 15-24 years attending HTS at the Ugan

Genital tract infections, antiretroviral treatment and HIV-1 viral load in early postpartum women: : implications for sexual and breastfeeding HIV transmission.

BACKGROUND: Maintaining viral suppression in the postpartum period is critical for preventing HIV transmission to sexual partners and breastfed infants. Despite systemic viral suppression, genital HIV shedding may still occur in some women. We examined the association between bacterial vaginosis (BV) and sexually transmitted infections (STIs) and genital HIV shedding in postpartum women. METHODS: In this cross-sectional postpartum study, we hypothesised that BV and other genital tract infections would modulate HIV viral load (VL) in the genital tract. Postpartum women with HIV, aged < 25 years

Imaging characteristics and comparative evaluation of dual-fluorescence assay in various types of vaginitis: a cross-sectional study.

BACKGROUND: Rapid and accurate differentiation of common vaginitis remains challenging in routine clinical practice. Dual-fluorescence assay, which simultaneously visualizes microorganisms and host cells under blue light excitation, may serve as an adjunctive tool to conventional microscopy. This study explored the imaging characteristics of dual-fluorescence staining in cultured vaginal epithelial cells and standard reference microbial strains, and further evaluated its diagnostic performance in a clinical population of 758 women undergoing vaginal secretion testing. RESULTS: (1) Under 450-5

Harnessing the Vaginal Microbiome: A New Frontier in the Prevention of Sexually Transmitted Infections.

BACKGROUND: The vaginal microbiome plays a vital role in maintaining reproductive and urogenital health in females. In healthy women, it is dominated by Lactobacillus species, including L. crispatus, L. gasseri, L. iners, and L. jensenii. These species provide strong protection against sexually transmitted infections (STIs) and other gynecological diseases. They produce lactic acid, which helps maintain a low vaginal pH, supporting epithelial integrity, modulating the immune system, and preventing colonization by bacterial pathogens. Dysbiosis, or an imbalance in microbial communities, is link

Point-of-care screening for curable sexually transmitted infections in antenatal care in Zimbabwe: a costing study.

BACKGROUND: Sexually transmitted infections (STIs) including Chlamydia trachomatis, Neisseria gonorrhoeae and Trichomonas vaginalis during pregnancy are associated with adverse birth outcomes. However, there are limited published data available on cost estimates for implementing such screening. The aim of this study was to estimate the cost of integrating point-of-care STI screening into antenatal care in Harare, Zimbabwe, compared with a modelled standard of care (syndromic management). METHODS: We implemented screening for C. trachomatis, N. gonorrhoeae and T. vaginalis in antenatal care at

Epidemiology of Treponema pallidum, Chlamydia trachomatis, Neisseria gonorrhoeae and Trichomonas vaginalis among forcibly displaced populations: a global systematic review and meta-analysis.

BACKGROUND: Curable sexually transmitted infections (STIs), including Treponema pallidum (syphilis), Chlamydia trachomatis, Neisseria gonorrhoeae and Trichomonas vaginalis, remain a major global health concern; yet their epidemiology among forcibly displaced populations is poorly understood. Limited access to healthcare, high-risk exposures and structural vulnerabilities may contribute to a heightened burden of STIs in these populations. METHODS: A systematic review conducted up to 12 July 2024 assessed the prevalence of syphilis, C. trachomatis, N. gonorrhoeae and T. vaginalis infections amo

Implementing STI point-of-care testing using a whole-system programme model: the ProSPiRO implementation research protocol.

OBJECTIVES: Sexually transmitted infections (STIs) continue to cause substantial global morbidity and mortality, compounded by reliance on syndromic management and limited access to laboratory diagnostics. Although point-of-care tests (POCTs) can enable timely diagnosis and treatment, their adoption within health systems remains inconsistent. Here we present an implementation research protocol that aims to generate evidence on the acceptability, feasibility and suitability of a collaborative whole-system programme model to support implementation of STI POCTs and improve access to STI testing a

High Burden of Sexually Transmitted Infections Among Pre-exposure Prophylaxis (PrEP) Users: Testing and Prevention Needs.

Pre-exposure prophylaxis (PrEP) effectively prevents HIV acquisition but does not protect against other sexually transmitted infections (STIs). Rising STI rates among PrEP users have raised concerns about risk compensation and highlight the need for integrated sexual health strategies. We assessed the prevalence, anatomical distribution, and behavioural determinants of STIs among PrEP users in Spain. This retrospective observational study included 724 individuals receiving daily tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) at a tertiary hospital in Madrid between March and October 202
