[Emerging Therapeutic Strategies for Central Nervous System Regeneration].

Reperfusion therapy has markedly improved outcomes after acute ischemic stroke; however, many patients remain ineligible for treatment or sustain irreversible brain injury despite successful recanalization. Regenerative medicine therefore represents an important strategy for restoring damaged neural tissue and promoting functional recovery. Stem cell therapies may exert therapeutic effects through multiple mechanisms, including cell replacement, secretion of neurotrophic factors, modulation of inflammation and immune responses, angiogenesis, and intercellular transfer of mitochondria and other

The impact of creatine on brain functions.

As an endogenous compound essential for energy homeostasis, creatine holds a fundamental role in the bioenergetics of the brain, an organ with high metabolic requirements. This review provides a synthesis of current knowledge on the impact of creatine on the functions of the central nervous system (CNS). The brain, which is capable of de novo creatine synthesis, demonstrates an increase in its internal concentrations following supplementation; however, this effect is less significant than in muscle tissue and is constrained by the blood-brain barrier, among other factors. The development of ef

Mapping the hepatic neural niche: from three-dimensional innervation architecture to periportal interoception and human translation.

The liver has traditionally been viewed as an organ regulated by circulating hormones and long-range autonomic reflexes, with its intrahepatic neural architecture receiving comparatively little systematic attention. However, recent advances in tissue clearing, three-dimensional (3D) volumetric imaging, neural tracing, and spatial omics are reshaping this view. Here we synthesize emerging evidence that the hepatic nervous system is organized around spatially defined neurovascular-cellular niches, and propose the periportal region as a dominant anatomical site for interoceptive integration, whil

Central nervous system, cranial, and cerebrovascular conditions beyond the Kimonis criteria in patients with Gorlin syndrome: a retrospective cohort study.

BACKGROUND: Gorlin syndrome is a multisystem developmental and cancer-predisposing disorder commonly caused by germline PTCH1 variants. The Kimonis clinical criteria capture the canonical phenotype; however, central nervous system (CNS), cranial, and cerebrovascular conditions documented beyond these criteria remain poorly defined. METHODS: We conducted a retrospective descriptive cohort study of 32 patients who fulfilled the Kimonis clinical criteria and received care at either or both hospitals in Japan. Clinical information through March 31, 2026, including sufficiently documented care els

Effect of Different Cortical Targets of Transcranial Magnetic Stimulation on Freezing of Gait in Parkinsonism: A Systematic Review.

BACKGROUND AND PURPOSE: Freezing of gait (FOG) is a disabling and episodic gait disturbance in patients with Parkinson's, with varying responses to noninvasive brain stimulation. Transcranial magnetic stimulation (TMS) has been explored as a therapeutic option; however, the optimal cortical target for reducing FOG remains unclear. This systematic review synthesizes and critically evaluates the available evidence regarding the effects of TMS applied to different cortical targets on FOG-related outcomes, with emphasis on the indirect nature of comparisons between targets. METHODS: A systematic

H3K9me3 inhibition reverses Alzheimer's progression by restoring synaptic and immune proteostasis across the brain-retina axis.

INTRODUCTION: Epigenetic dysregulation is increasingly linked to aging and neurodegeneration, yet its contribution to retinal and brain pathology in Alzheimer's disease (AD) remains unclear. METHODS: We quantified the repressive epigenetic mark histone H3 lysine 9 trimethylation (H3K9me3) in post mortem retinas and brains from donors spanning normal cognition, mild cognitive impairment, and AD dementia. Using a multi-enzyme methyltransferase panel, we characterized the in vitro inhibitory activity of ETP69, an epidithiodiketopiperazine, against suppressor of variegation 3-9 homologue 1 (SUV39

Functional and Structural Imaging Features of Cognitive Profiles in Multiple Sclerosis.

BACKGROUND AND OBJECTIVES: Cognitive impairment affects 40%-70% of patients with multiple sclerosis (MS). Although recent hypothesis-driven and data-driven efforts have identified cognitive phenotypes, functional substrates underlying impairment in such phenotypes are still not clear. Our objective was to identify distinct cognitive phenotypes using clustering in a large cohort of patients with MS and characterize their associated clinical, structural MRI, and resting-state (RS) functional connectivity (FC) features. METHODS: We conducted an observational, cross-sectional study with right-han

Caspase-1 and Inflammasome Signaling in Spinal Cord Injury: Mechanistic Insights and Translational Gaps.

Caspase-1 is a central mediator of canonical inflammasome signaling and drives neuroinflammatory injury through interleukin-1β (IL-1β) and interleukin-18 (IL-18) maturation, gasdermin D (GSDMD)-mediated pyroptosis, and secondary inflammatory cascades. In spinal cord injury (SCI) and other neurologic disorders, experimental evidence links caspase-1 activation to blood-spinal cord barrier (BSCB) or blood-brain barrier (BBB) disruption, glial activation, neuronal and oligodendroglial injury, and worse functional outcomes. However, despite mechanistic rationale, the clinical role of caspase-1 as a

Cerebrospinal fluid outflow pathways in neurodegeneration: nasopharyngeal lymphatics and the ocular glymphatic-like axis.

The glymphatic system supports cerebrospinal fluid (CSF)-interstitial fluid (ISF) exchange and has been implicated, to different extents, in the transport or clearance of amyloid-β (Aβ), tau, lactate and other solutes. Although most studies have focused on CSF influx, astrocytic aquaporin 4 (AQP4) expression, perivascular localisation and polarisation, AQP4-mediated transmembrane water transport, and perivascular exchange, brain clearance also depends on efficient extracranial drainage of waste-containing fluid. In this narrative Review, we examine this 'outflow end', including meningeal lymph

Effects of Finasteride exposure on lifespan and locomotor phenotypes.

Finasteride is one of the core medications for treating androgen-associated diseases in men. It exhibits multiple therapeutic functions, including inhibiting 5α-reductase, reducing dihydrotestosterone levels, and improving conditions, including prostate enlargement and hair loss. Using Drosophila as the primary model and a mouse behavioral assay as a complementary approach, we combined developmental and behavioral assessments, morphological analyses of motor and sensory neurons, transcriptomics, and genetic methods to investigate developmental, neuronal, and behavioral changes associated with

Predictive factors associated with interval changes in STN-DBS lead-tip position between immediate and delayed postoperative CT.

BACKGROUND: Brain shift during deep brain stimulation (DBS) surgery may contribute to changes in lead position. Although intracranial air volume is considered an important factor, other intraoperative factors may also influence lead displacement. This study evaluated interval changes in subthalamic nucleus (STN)-DBS lead-tip position between immediate and delayed postoperative computed tomography (CT) and investigated factors associated with these changes, including intracranial air volume and intraoperative head position. METHODS: We retrospectively analyzed 42 DBS leads implanted bilaterall

Peripheral and cerebrospinal fluid inflammatory markers in sporadic amyotrophic lateral sclerosis an exploratory cross-sectional association study.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder with an ambiguous etiology, whose onset may be linked to innate immune system activation outside the central nervous system (CNS). We investigated the association between peripheral blood and CNS inflammatory parameters in 45 sporadic ALS patients compared to 30 controls, analyzing peripheral blood tests (C-reactive protein [CRP], blood count, fibrinogen) and cerebrospinal fluid (CSF) immunological measures (Tumor Necrosis Factor alpha [TNF-α], TNF receptor type 1 [TNFR-1], CRP) alongside clinical characteristics.

Circadian rhythms and sleep disturbances: a conceptual perspective with implications for autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by cyst formation, kidney enlargement, and extrarenal complications. While the role of genetic mutations in PKD1, PKD2, and other minor genes is well established, the influence of circadian rhythms and sleep disturbances on disease progression remains underexplored. The circadian timing system, coordinated by the central clock in the suprachiasmatic nucleus and synchronized with peripheral clocks in the kidneys, regulates key physiological processes, including glomerular filtration rate (GFR), sodium handl

Development of Italian centile curves for the North Star Ambulatory Assessment and timed items.

Centile curves may improve the interpretation of motor outcomes in specific populations. This study aimed to develop Italian age-specific centile curves for NSAA, rise-from-floor velocity (RFFV), and 10-meter walk/run velocity (10MWRV) in boys with Duchenne muscular dystrophy (DMD) and to assess their agreement with existing UK reference curves. Data from 627 patients (5080 assessments) with an age between 3-18 years were extracted from the Italian DMD Registry. GAMLSS models were used to estimate centile curves. Agreement with UK data (restricted to ages 5-16 years and who are actively on ste

Thymoquinone-Loaded Hexosomal Nanoparticles Attenuate Neuroinflammation and Improve Cognitive Function in an Alzheimer's Disease Mouse Model.

INTRODUCTION: Alzheimer's disease (AD) involves neuroinflammation that worsens cognitive decline. Thymoquinone (TQ) has potent anti-inflammatory activity, but its therapeutic application is limited by poor solubility, stability, and systemic bioavailability, which may restrict effective brain exposure. This study developed TQ-loaded hexosomal nanoparticles (TQ-HNP) to enhance TQ delivery and therapeutic efficacy in a dizocilpine (MK-801)- induced mouse model of AD. METHODS: TQ-HNP were prepared using glyceryl monooleate, oleic acid, and poloxamer 407, optimized to ~104 nm size, -31 mV zeta po

The brain of the female Anopheles gambiae mosquito at spatial and single-cell resolution.

Blood feeding in Anopheles mosquitoes is at the root of all malaria infections in humans and is supported by a range of physiological and behavioral adaptations. Since behavior is orchestrated at the level of the central nervous system, we surveyed the cellular diversity and spatial organization of the female Anopheles gambiae brain and compared it to other insect species. We then describe transcriptional changes associated with mosquito life history events with single brain and single-cell RNA sequencing. We did not identify expansion of specific cell populations, but a modulation of neurotra

Acute Psychosis as the First Manifestation of Neuro-Behçet's Disease.

Behçet's disease is a chronic, recurring systemic inflammatory vasculitis of unknown etiology. If the central nervous system is involved, the condition is referred to as neuro-Behçet's disease. In addition to classical neurological involvement, patients with Behçet's disease may present with complex neurobehavioural and mental disorders, which are termed neuro-psycho-Behçet's disease. We describe a rare case of a 28-year-old man presenting with acute psychosis as the initial presentation of neuro-Behçet's disease.

Maladaptive sensory coding in the enteric nervous system playing a role in gastrointestinal disorders.

Maladaptive sensory coding within the enteric nervous system (ENS) is increasingly implicated in gastrointestinal dysmotility, abnormal secretion, neuroimmune dysfunction, and visceral hypersensitivity. Intrinsic primary afferent neurons (IPANs), classically corresponding to Dogiel type II/AH neurons, constitute a major sensory arm of intrinsic enteric reflex circuits. They detect mechanical, chemical, epithelial, microbial, and immune-derived signals and relay this information to interneurons, motor neurons, and secretomotor pathways, thereby coordinating gastrointestinal function independent

Takotsubo Cardiomyopathy Associated With Severe Medetomidine Withdrawal: A Case Report.

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a transient syndrome of cardiac dysfunction with no occlusive coronary arterial disease, which may mimic an ischemic cardiac event. The pathogenesis of TCM is thought to be primarily driven by the effects of a temporary catecholamine surge on cardiac and vascular tissue. Medetomidine (an a2 agonist commonly found in the Philadelphia fentanyl supply) withdrawal is characterized by agitation and sympathetic hyperactivity that can precipitate a catecholamine surge. Though withdrawal from other Central Nervous System (CNS)-depressing substances such as
