Epigenetic mechanisms in brain development and in neurodegeneration.

Epigenetic mechanisms are important heritable modifications beyond genetic mechanisms. Interindividual epigenetic variants can lead to phenotypic variations even in identical genotypes. Studies on the epigenetic contributions to Alzheimer disease and other neurodegenerative diseases could provide more comprehensive and precise insights into the heritable and nonheritable mechanisms. This knowledge will enable the development of individually tailored preventive and therapeutic pharmacologic interventions. This chapter is divided into three sections. First, we provide a detailed introduction to

Real-World Health Care Resource Utilization, Adherence, and Persistence Following Subcutaneous IgPro20 Initiation Among US Patients with Chronic Inflammatory Demyelinating Polyneuropathy.

INTRODUCTION: Chronic inflammatory demyelinating polyneuropathy (CIDP) is a rare, inflammatory peripheral nervous system disorder. Intravenous immunoglobulin (IVIG) has been the standard maintenance therapy, but since 2018, Immune Globulin Subcutaneous (Human), 20% liquid (IgPro20), a subcutaneous immunoglobulin (SCIG) therapy, has offered an alternative, demonstrating benefit in maintaining CIDP improvement and remission. This study aimed to evaluate real-world healthcare resource utilization (HCRU), adherence, and persistence following IgPro20 initiation in patients with CIDP, given limited

Skull Bone Marrow as a Cranial Neuroimmune Reservoir in Alzheimer's Disease: Amyloid, Osteoimmune Remodeling, and Translational Imaging Evidence.

Alzheimer's disease (AD) has traditionally been conceptualized as a disorder driven primarily by cerebral amyloid-β accumulation, tau pathology, synaptic failure, vascular dysfunction, and chronic neuroinflammation. However, emerging evidence suggests that AD pathobiology may also involve extracerebral cranial compartments positioned at the interface between the brain, meninges, cerebrospinal fluid, and skull bone marrow. Recent anatomical and experimental studies have identified direct vascular and osseous channels connecting skull bone marrow with the dura mater, allowing bidirectional excha

Placental Insulin-like Growth Factor Signaling and Neurodevelopment: Emerging Evidence from the Placenta-Brain Axis.

The field of neuroplacentology is a growing area that focuses on the influence of placental function on brain development. This field has expanded our understanding of the placenta's crucial role in proper neurodevelopment as well as placental anomalies that increase the risk of adverse neurodevelopmental outcomes. Neuroplacentology studies have revealed distinct roles of placentally provided hormones and nutrients in neurodevelopment whose disruption by perinatal problems, including inflammation, can increase the risk of neurodevelopmental disorders such as autism spectrum disorder and ADHD.

The Landscape of Tumor Dissemination in Leptomeningeal Disease: Mapping CTC Plasticity and Regional Heterogeneity Across Blood and CSF Compartments.

Background/Objectives: Leptomeningeal disease (LMD) is among the most severe complications of metastatic breast cancer, with median survival measured in weeks. Definitive diagnosis relies on neuroimaging and cerebrospinal fluid (CSF) cytology, which carries a false-negative rate of 40-50% at a single lumbar puncture. Improved tools for detecting and characterizing tumor cells across systemic and central nervous system (CNS) compartments are urgently needed. Methods: We applied an enrichment-free, fluorescence whole-slide imaging (fWSI)-based liquid biopsy platform to ante-mortem and post-morte

Ameliorative Effects of Citrus Unshiu Peel Extract in Cellular and Mouse Models of Pelizaeus-Merzbacher Disease.

Hypomyelinating leukodystrophy 1 (HLD1), also known as Pelizaeus-Merzbacher disease (PMD), is a severe X-linked hereditary hypomyelinating/demyelinating disorder caused primarily by genomic duplications or multiplications that lead to the increased expression of proteolipid protein 1 (PLP1). Aberrant PLP1 expression impairs oligodendrocyte differentiation and disrupts myelin formation in the central nervous system (CNS). Despite decades of research, effective disease-modifying therapies remain unavailable. In the present study, we investigated the potential of Chinpi, the Citrus Unshiu Peel ex

Functional hypothalamic amenorrhea: A call for global awareness and research.

Functional hypothalamic amenorrhea (FHA) is a common yet under-recognized cause of secondary amenorrhea resulting from reversible suppression of the hypothalamic-pituitary-ovarian axis. Triggered by chronic stress, low energy availability, disordered eating, and/or excessive exercise, FHA leads not only to anovulation and hypoestrogenism but also to broader neuroendocrine disruption involving the hypothalamic-pituitary-adrenal and thyroid axes. Although historically associated with extreme trauma and the "female athlete triad," FHA occurs worldwide and remains significantly underdiagnosed acro

Phenotype, treatment, and determinants of prognosis in Erdheim-Chester disease: an international, retrospective, cohort study.

BACKGROUND: Erdheim-Chester disease is a rare histiocytosis driven by MAPK pathway gene mutations. Its clinical presentation is heterogeneous, and there is a shortage of data on natural history and prognosis. We aimed to characterise clinical presentation, associated conditions, treatments, outcomes, and determinants of prognosis in a large international Erdheim-Chester disease cohort. METHODS: We conducted an international, retrospective, cohort study of patients with biopsy-proven Erdheim-Chester disease diagnosed between Jan 1, 1985, and Dec 31, 2024. Data were collected in ten countries b

Epithelial-mesenchymal Wnt crosstalk directs planar cell polarity in the developing cochlea.

Cochlear hair cells are precisely oriented by planar cell polarity. Wnt proteins direct planar cell polarity in vertebrates, but their roles in regulating cochlear planar cell polarity remain unclear. Here, we inhibit Wnt secretion by ablating epithelial Wntless and find shortened cochlea, mild hair cell misorientation and mislocalization of core planar cell polarity proteins, Fzd6 and Dvl2. Computational analysis predicts that epithelial Wnts (Wnt5a, Wnt7a, Wnt7b) and Wnt5a from surrounding periotic mesenchyme act on developing hair cells. Deletion of epithelial Wnt5a, Wnt7a, and Wnt7b additi

A trans-therapeutic signature of recovery in internet gaming disorder: From shared network reorganization to molecular underpinnings.

BACKGROUND AND AIMS: A variety of self-regulation interventions have shown efficacy in treating Internet Gaming Disorder (IGD), but their shared and unique neurobiological mechanisms remain unclear. This study aims to identify a common, trans-therapeutic neural signature of recovery and to delineate the specific connectivity fingerprints of five different self-regulation interventions for IGD. METHODS: This secondary analysis pooled resting-state fMRI data from 269 individuals with IGD who participated in one of five randomized controlled trials of self-regulation-based interventions: Approac

The role of epigenetic DNA modifications in the brain: Focus on cell specification, neuropathology, and environmental influences.

Epigenetics plays a fundamental role in the regulation of transcriptional landscapes that define cellular identity and the ability of cells and tissues, and ultimately organisms, to adapt to a changing environment. DNA methylation is a stable and heritable epigenetic mark that is involved in cellular differentiation and homeostasis; nevertheless, methylation marks can be reversed by oxidation of 5-methylcytosine into 5-hydroxymethylcytosine and other derivatives, through the action of TET (ten-eleven translocation) dioxygenase enzymes. This provides a mechanism for the active DNA demethylation

Neuroimmune-oligovascular signaling shapes oligodendrocyte precursor cell fate and repair after ischemic stroke.

BACKGROUND: Ischemic stroke causes neuronal and oligodendroglial injury, axonal and myelin damage, and neurovascular unit dysfunction. Oligodendrocyte precursor cells (OPCs) are lifelong progenitors that generate oligodendrocytes while receiving and transmitting signals within neural, immune, and vascular microenvironments. This review examines how neuroimmune-oligovascular communication and intracellular signaling shape OPC survival, recruitment, state transitions, differentiation, and remyelination after ischemic stroke. MAIN BODY: We first define OPC lineage identity, functional heterogene

Comparative Analysis of Impulse Control Symptoms Among Individuals Receiving GPi-DBS or STN-DBS for Parkinson's Disease.

OBJECTIVE: Impulse control disorders (ICDs) are a complication of Parkinson's disease (PD) treatment, but whether deep brain stimulation (DBS) affects ICDs remains unknown. Studies have focused on de novo ICDs among individuals receiving subthalamic nucleus DBS (STN-DBS), but the effects of DBS of the globus pallidus pars internus (GPi-DBS) are less understood. Rates and severities of ICDs among individuals receiving GPi-DBS or STN-DBS were compared. METHODS: Twenty-seven individuals with PD receiving bilateral DBS (17 STN, 10 GPi-DBS) were prospectively enrolled. Before surgery, participants

The effect of concurrent application of the deep brain stimulation of the nucleus accumbens and cannabidiol injection during extinction period on reinstatement of METH-induced conditioned place preference in extinguished rats.

Methamphetamine (METH) is a potent stimulant drug that significantly alters the function of the central nervous system (CNS). METHS's abuse may be linked with a variety of psychiatric disorders, primarily by affecting the reward system. Cannabidiol (CBD), a primary cannabinoid component of the Cannabis plant, has been shown to affect the reward system, suggesting it for modulating addictive behaviors. On the other hand, deep brain stimulation (DBS) of the nucleus accumbens (NAc), one of the most significant brain regions implicated in mediating rewards, is effective in alleviating drug-seeking

Incident versus prevalent progressive supranuclear palsy: analyses of U.S. Alzheimer's Disease Research Centers data indicate clinical and neuropathological features of a relatively common and underrecognized tauopathy in the aging brain.

Progressive supranuclear palsy (PSP) is an adult-onset neurodegenerative disease with a heterogeneous spectrum of clinical and pathological features. The epidemiology of PSP-as defined by the gold standard of autopsy diagnosis-remains unresolved, but prevalence estimates are typically ~1:10,000. Here, we analyzed the National Alzheimer's Coordinating Center (NACC) Neuropathology Data Set, aggregated from 38 U.S. Alzheimer's Disease Research Centers (ADRCs). We compared clinical and neuropathologic features of autopsied participants, stratifying on cognitive status at recruitment into the cohor

Cortisol and cognition in psychotic disorders: A systematic review and multi-level meta-analysis.

Emerging evidence suggests that dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, particularly cortisol, may contribute to the cognitive deficits commonly observed in psychotic disorders. We conducted the first combined systematic review and quantitative synthesis of associations between cortisol and cognition in psychosis. Twelve studies met inclusion criteria (940 participants; 74.68% cases; 39.57% female; mean age = 31.83 years). Of these, nine studies (822 participants; 71.04% cases; 35.96% female; mean age = 32.72 years) contributed data to three-level meta-analyses examinin

Endometriosis and chronic pain syndrome: The role of stress mechanisms.

Patients with endometriosis report increased levels of perceived stress, which correlates with disease severity and pain intensity, creating feedback loops that perpetuate the cycle of endometriosis inflammation, chronic pain syndrome, and pathological stress response. Stress and chronic pain form an integrated biopsychosocial system in which neuroendocrine dysregulation, central sensitization, neuroinflammation, large-scale network reorganization, and maladaptive threat learning mutually reinforce each other. Sex differences, developmental experiences, and resilience factors modulate this int
