Characteristics of patients with dropped head syndrome responsive to serotonin-norepinephrine reuptake inhibitors.

BACKGROUND: Dropped head syndrome (DHS) is a heterogeneous disorder. DHS without any causative diseases has no established pharmacological therapy, thus current treatment is generally limited to pain management. Recently, the potential effect of duloxetine, a serotonin-norepinephrine reuptake inhibitor, has been reported in selected DHS patients. This study aimed to identify the characteristics associated with responsiveness to duloxetine in idiopathic DHS. METHODS: 38 patients with idiopathic DHS who received duloxetine primarily for chronic pain were enrolled. Patients were classified as re

Myeloid sarcoma of the sternum with regional lymph node involvement in a 6-year-old child: a case report.

BACKGROUND: Myeloid sarcoma is a rare extramedullary tumor composed of myeloid progenitor cells, often associated with acute myeloid leukemia or other hematologic malignancies. Primary involvement of the sternum in children is extremely uncommon and may pose substantial diagnostic challenges. CASE PRESENTATION: A 6-year-old boy presented with recurrent sternal pain lasting over four months, along with low back pain, bilateral breast pain, and fever. Imaging showed a destructive sternal lesion with an adjacent soft tissue mass, initially raising suspicion for Ewing sarcoma. Bone marrow aspirat

A structured list of laboratory tests for screening the possible causes of small fiber neuropathy in clinical practice.

Small fiber neuropathies (SFN) are increasingly recognized as the cause of various sensory and autonomic disorders. Different tests exist to enable the objective diagnosis of SFN, but these tests generally do not identify a possible etiology. However, finding the cause of SFN is the best way to implement effective treatment. Thus, the etiological assessment must be as exhaustive as possible so as not to miss a curable cause of SFN. This search is based primarily on patient's history and clinical examination but may also require additional laboratory investigations. The objective of this articl

Clinical and functional characteristics of the KLF11 (p.R29Q) variant associated with MODY7.

OBJECTIVE: The KLF11 gene can cause maturity-onset diabetes of the young type 7 (MODY7). Currently, there are few reports on MODY7 cases and their clinical and functional characteristics. This study aimed to analyze the clinical and functional features of the KLF11 R29Q variant to provide a basis for the diagnosis and treatment of this disease. METHODS: Clinical baseline data of patients were collected, and high-throughput sequencing (NGS) was used to screen probands clinically suspected of harboring KLF11 variants, with validation performed in their family members. A luciferase reporter assa

Autonomic dysfunction in fibromyalgia syndrome: the role of small fiber damage.

OBJECTIVE: Cardiovascular autonomic symptoms, particularly orthostatic intolerance, are frequent in fibromyalgia syndrome (FMS) and markedly contribute to disability, yet their underlying mechanisms remain unclear. Small fiber pathology (SFP), detected in 30-50% of patients with FMS, may also involve postganglionic sympathetic fibers. This study examined the contribution of SFP to autonomic dysfunction and cardiovascular autonomic symptoms in FMS. METHODS: We prospectively enrolled 43 patients with FMS reporting orthostatic intolerance and 20 age- and sex-matched healthy controls. All partici

Cardiac Autonomic Neuropathy is Associated with the Incidence and Progression of Cardiovascular-Kidney-Metabolic Syndrome.

AIMS: To investigate the role of cardiac autonomic neuropathy (CAN) in the development and progression of cardiovascular-kidney-metabolic (CKM) syndrome. METHODS: A three-stage analytical framework was applied. Cross-sectional associations between CAN (assessed by cardiovascular autonomic reflex tests) and CKM stages were examined in a discovery cohort and validated in the UK Biobank cohort. Longitudinal analyses evaluated the association between CAN and incident CKM syndrome in the UK Biobank. Multi-state models were used to assess the influence of CAN on CKM progression. RESULTS: The preva

Diabetic gastroparesis: pathophysiology and impact on insulin timing choices.

Diabetic gastroparesis (DGP) is a chronic and frequently underrecognized complication of diabetes mellitus, characterized by delayed gastric emptying in the absence of mechanical obstruction. It represents a clinically significant manifestation of diabetic autonomic neuropathy and arises from a complex interplay of sustained hyperglycemia, oxidative and nitrosative stress, advanced glycation end-product accumulation, impaired insulin/IGF-1 signaling, lipid dysmetabolism, and inflammatory activation. These mechanisms converge to damage autonomic and enteric neurons, reduce interstitial cells of

Repeated Ultrasound-Guided Lumbar Sympathetic Block for Refractory Painful Diabetic Peripheral Neuropathy: The First Case from Mzuzu Central Hospital, Malawi.

BACKGROUND: Painful diabetic peripheral neuropathy (PDPN) is a common complication of diabetes mellitus characterized by chronic neuropathic pain, sleep disturbance, and impaired quality of life. Although pharmacological therapies remain the cornerstone of treatment, many patients experience inadequate symptom control. Access to advanced interventional pain management techniques, including radiofrequency ablation and neuromodulation, remains limited in many healthcare settings. We report the first case of ultrasound-guided lumbar sympathetic block (LSB) performed for refractory PDPN at Mzuzu C

Behavioral variant frontotemporal dementia associated with a NEK1 missense variant: exploring a possible phenotypic association.

NEK1 variants are recognized genetic contributors to amyotrophic lateral sclerosis (ALS) and have occasionally been reported within the ALS-frontotemporal dementia (FTD) spectrum. However, their association with isolated behavioral variant frontotemporal dementia (bvFTD) remains unclear. Here, we describe a 69-year-old man who developed progressive behavioral symptoms beginning in his early 60s. Cognitive evaluation demonstrated reduced verbal fluency with relative preservation of memory functions. Structural and functional neuroimaging demonstrated right-predominant frontotemporal atrophy and

Treatment-induced neuropathy of diabetes (TIND): a hidden complication in diabetes management.

Treatment-induced neuropathy of diabetes (TIND) is an under-recognised complication that arises after rapid glycaemic control in diabetic patients. It presents with acute autonomic dysfunction and/or neuropathic pain resulting from small fibre nerve damage. This case involves a woman in her 50s with type 2 diabetes who transitioned from oral hypoglycaemic agents to insulin therapy, leading to a significant drop in HbA1c from 10% to 7.2% within the last 3 months. She presented with painful sensations, orthostatic hypotension and syncope. A diagnostic workup revealed peripheral nerve and autonom

Neuroinflammation and osteomyelitis in adults with Type 2 diabetes mellitus and peripheral neuropathy without and with foot lesions. What comes first?

AIMS: Diabetic foot is the leading cause of both major and minor non-traumatic amputations yet a truly understanding of the phenomenon is still lacking. The updated definition for diabetes-related foot disease from the International Working Group on the Diabetic Foot (IWGDF 2023 update) is "disease of the foot of a person with current or previously diagnosed diabetes mellitus that includes one or more of the following: peripheral neuropathy, peripheral artery disease, infection, ulcer(s), neuro-osteoarthropathy, gangrene, or amputation", but what comes first? Our hypothesis is that distal sens

Diabetic distal symmetric polyneuropathy: More than just "tingling in the feet".

Diabetic distal symmetric polyneuropathy (DDSP) is the most common form of diabetic neuropathy. DDSP, foot ulcerations, and lower limb amputation greatly increase mortality, which is 23% 2 years after a foot ulceration and rises to 71% after 10 years. DDSP is seen in patients with both type 1 and type 2 diabetes, but the clinical features and pathophysiology are distinctly different. DDSP also presents in patients with pre-diabetes and/or obesity alone, indicating that glycaemic control is not the sole contributor to the pathophysiology of DDSP. In the BARI 2D trial, 51% of those with type 2 d

Evolution and transmission dynamics of wild poliovirus in Pakistan and Afghanistan (2012-2023).

Despite concerted global vaccination efforts, wild poliovirus remains endemic in two countries in 2024, Pakistan and Afghanistan. This study uses phylogeographic analysis of poliovirus genetic and epidemiological data from clinical and wastewater surveillance to identify the causes of poliovirus persistence and routes of spread over the last decade (2012 to 2023). Poliovirus genetic diversity declined after 2020, with one of two major genetic clusters dying out, and recent detections are now closely related genetically. High-risk and hard-to-access regions have sustained polio transmission ove

Perioperative changes of serum orphanin in diabetic patients and its relationship with sympathetic nervous system.

The occurrence of cardiovascular events in diabetic patients during the perioperative period is related to the activation of sympathetic nerves. Basic research shows that serum nociceptin/orphanin FQ (N/OFQ) levels in diabetic neuropathy rats increased, and N/OFQ reduces the release of norepinephrine (NE). We hypothesize that N/OFQ will affect the sympathetic nervous system during perioperative myocardium of diabetic patients. 66 patients with unilateral knee arthroplasty were divided into diabetes group (D group) and non-diabetes group (N group). Measured blood glucose, serum NE, N/OFQ concen
