Precision treatment of chalazion based on AI-driven risk stratification: a prospective study of triamcinolone acetonide combined with intense pulsed light therapy.

PURPOSE: To evaluate the rates of chalazion recurrence following intralesional triamcinolone acetonide (TA), intense pulsed light (IPL), or combined TA + IPL therapy and to validate an AI-enhanced risk stratification strategy. METHODS: This prospective, observational study enrolled 366 patients. To eliminate immortal time bias and circular reasoning, LASSO-Cox regression with 10-fold cross-validation was employed to identify purely baseline predictors: age, Demodex infection, chronicity, and meibomian gland dysfunction (MGD). These baseline predictors were integrated into a clinical Cox propo

Lower eyelid schwannoma mimicking chalazion in an adolescent.

To highlight a rare case of eyelid schwannoma in a teenager, clinically mimicking a common chalazion, and confirmed by histopathology and immunohistochemistry. A 15-year-old boy presented with a slowly enlarging, painless right lower eyelid nodule. Initially diagnosed as a chalazion or sebaceous cyst, the lesion was surgically excised. Histopathological examination revealed interwoven fascicles of spindle cells with Antoni A and B patterns and Verocay bodies. Immunohistochemistry revealed strong S-100 protein positivity, confirming the diagnosis of schwannoma. Eyelid schwannomas are extremely

Diagnostic challenge of a lower eyelid swelling: A vascular thrombus mimicking a dermoid cyst.

Eyelid and periorbital lesions encompass a wide range of etiologies and clinical presentations, often posing diagnostic challenges. We report a case of a right lower eyelid mass that was initially referred to as a nonresolving chalazion. Clinical suspicion suggested a vascular lesion; however, magnetic resonance imaging indicated a dermoid or epidermoid cyst. Definitive diagnosis was achieved through histopathological examination of the excised specimen, which revealed a vascular thrombus originating from a true aneurysm of an artery. In the absence of a clear history of trauma, infection, or

Recurrent Inflammatory Intratarsal Keratinous Cyst Following an Incomplete Excision: A Case Report.

Intratarsal keratinous cyst (IKC) is a rare benign lesion arising within the tarsal plate that is frequently misdiagnosed as a chalazion. An IKC's incomplete excision often leads to multiple recurrences, but cases accompanied by acute inflammation or pain remain uncommon. We report a rare case of an IKC presenting with recurrent painful inflammatory episodes following incomplete surgical removal. An 80-year-old male presented with a firm, painless mass in his left upper eyelid. Eyelid eversion revealed a whitish lesion through the palpebral conjunctiva. Under the clinical diagnosis of a chalaz

Visibility-Graph Analysis of Raman Spectra in Chalazion and Lipid Dysregulation: An Exploratory Study.

A chalazion is an inflammatory lesion of the eyelid caused by a blockage in the sebaceous glands. To identify subtle biochemical differences in chalazion secretions and patient serum, we apply a graph-theory-based visibility-graph approach to Raman spectra, focusing on its exploratory potential for detecting band-specific spectral differences. As a classical baseline, we also employ Principal Component Analysis (PCA); however, the visibility-graph approach provides complementary, band-localized information that may support interpretation of subtle spectral differences. The study included sampl

Pyoderma Gangrenosum of the Eyelid: A Challenging Case.

BACKGROUND Pyoderma gangrenosum (PG) involving the eyelid is rare and can closely mimic other differential diagnoses, posing a diagnostic challenge. This case highlights the diagnostic approach and management of pyoderma gangrenosum-related eyelid lesions. CASE REPORT A 29-year-old man with a history of ulcerative colitis developed a persistent ulcerative lesion on his left upper eyelid, which failed to respond to conventional treatment for chalazion. Examination revealed a pinkish-yellow ulcerative lesion with ill-defined borders, causing distortion of the eyelid margin contour. Due to the po

Topical Tacrolimus as a Novel Treatment for Refractory Idiopathic Facial Aseptic Granuloma in a Pediatric Patient.

This report describes the case of a four-year-old girl with idiopathic facial aseptic granuloma (IFAG) who developed persistent, treatment-resistant eyelid and facial inflammation despite multiple courses of antibiotics and corticosteroids. Her presentation included recurrent hordeola, severe blepharitis, and a cheek nodule, with a clinical picture more consistent with IFAG than pilomatrixoma despite histologic evidence of the latter. Standard therapies provided only transient benefit and were limited by systemic side effects. Initiation of topical tacrolimus 0.03% ointment led to rapid and su

Blepharitis driven by microbiome dysbiosis and Demodex infestation: possible pathogenic mechanisms.

Blepharitis is a chronic inflammation of the eyelid margin that is mediated by the immune system. It is one of the common ocular surface diseases and often leads to serious sequelae that threaten vision, such as dry eye syndrome due to insufficient tear secretion, corneal neovascularization, and stubborn chalazion. Elucidating its precise etiology is therefore imperative. Emerging high-throughput sequencing and metagenomic analyses have unveiled a quantitative and qualitative disruption of the periocular microbiome (dysbiosis), characterized by the expansion of specific bacterial species such

Simultaneous and recurrent hordeola and chalazia associated with tirzepatide therapy: a case series.

Glucagon-like peptide-1 (GLP-1) and dual GLP-1/glucose-dependent insulinotropic polypeptide (GIP) receptor agonists are increasingly prescribed for obesity and type 2 diabetes, but eyelid gland pathology has not previously been reported with these agents. We describe what we believe to be the first reported case series of 2 patients who developed multiple simultaneous internal hordeola after starting tirzepatide, a dual GLP-1/GIP receptor agonist. Patient 1, a 44-year-old woman, developed 6 simultaneous hordeola 4 weeks after dose escalation, 5 of which progressed to chalazia; lesions lessened

Photobiomodulation therapy for children's eye and vision conditions.

BACKGROUND: Photobiomodulation (PBM) is a non-invasive, light-based technology that stimulates cellular mechanisms, with therapeutic effect on a variety of medical conditions. Wavelengths of light are absorbed by intracellular photoreceptors, resulting in the activation of signalling pathways that culminate in biological changes within the cell. PBM treatment has been reported for a range of eye/vision conditions, mostly in adults. OBJECTIVES: We carried out a scoping review of the published literature and collated reported uses of PBM for eye/vision/lid/orbital conditions in children and you

Anterior Segment Eye Diseases and Quality of Life: An Updated Comprehensive Narrative Review.

Anterior segment eye diseases significantly affect patients' quality of life (QoL) by impairing physical health, psychological well-being, social interactions, and economic stability. Conditions such as dry eye disease, blepharitis, conjunctivitis, keratoconus, keratitis, and corneal dystrophies cause chronic discomfort, visual disturbance, and emotional distress, leading to reduced daily functioning and work productivity. Other disorders like chalazion, trichiasis, ptosis, strabismus, and thyroid eye disease entail a psychological burden by affecting appearance and social confidence. This nar
