Ocular Immune Toxicity in Cancer Immunotherapy, From Immune Privilege Breakdown to Treatment Rechallenge: A Review.

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy but can disrupt ocular immune privilege and produce heterogeneous immune-related adverse events (Eye-irAEs). This structured narrative review with evidence mapping integrates clinical, epidemiologic, mechanistic, pharmacovigilance, and management data to define translational pathways from checkpoint blockade to ocular toxicity and treatment decisions. In a real-world cohort of 41,020 ICI-treated patients, ophthalmic irAEs occurred in 4.18% within 1 year, including dry eye disease in 2.00%, conjunctivitis in 0.87%, blepharitis

Treatment of Mixed Dry Eye Disease with Topical Azithromycin: A Prospective Controlled Pilot Study.

Background: This study evaluated the efficacy of topical azithromycin ophthalmic solution (Azyter®, 15 mg/g) as second-line treatment for mixed dry eye disease (DED), given the drug's known non-antimicrobial anti-inflammatory and immunomodulatory properties. Methods: In this pilot, prospective, controlled, non-randomized interventional study, fifteen patients with mixed DED refractory to first-line treatment received topical azithromycin for 30 days (twice daily for 3 days, then once daily for 27 days); ten healthy individuals served as controls. Clinical evaluation was performed before and af

An interdisciplinary scoping review of the emerging impact of periocular skincare products on the ocular surface microbiome and tear film stability.

BACKGROUND: The healthy ocular surface microbiome consists of Actinobacteria, Proteobacteria, and Firmicutes phyla, with Corynebacterium, Propionibacterium, and Staphylococcus as dominant genera. This microbial environment is essential for maintaining immune homeostasis and barrier integrity. OBJECTIVE: To synthesize existing evidence on how periocular skincare and cosmetic products affect the ocular surface microbiome, tear film stability, and downstream ocular surface disease. METHODS: A systematic literature search was conducted across PubMed, Cochrane Library, Embase, Web of Science, and

Intense pulsed light as a multi-target intervention paradigm for refractory ocular surface diseases: a review.

Ocular surface diseases (OSDs) are a heterogeneous group of disorders characterized by overlapping inflammatory, vascular, microbial, neuroimmune, and meibomian gland abnormalities that contribute to the onset and persistence of ocular surface symptoms and signs. Conventional symptom-directed therapies do not adequately alleviate refractory meibomian gland dysfunction, Demodex-associated blepharitis, ocular rosacea, recurrent chalazion, and allergic conjunctivitis. Recent evidence suggests that these conditions may share convergent pathological features, including chronic inflammation, abnorma

Acquired Distichiasis in Dermatomyositis: A Possible Delayed Sequela of Chronic Periocular Inflammation.

Acquired distichiasis is an uncommon eyelid disorder characterized by an accessory row of eyelashes arising from the meibomian gland orifices, most often in the setting of chronic inflammatory or cicatricial disease. We report a case of a 65-year-old woman with longstanding dermatomyositis, recurrent periocular inflammation, and episodes of blepharitis who developed bilateral upper-eyelid distichiasis. Review of clinical photographs from 2020 and from a severe periocular flare in 2023 showed no clearly visible accessory row. At follow-up in January 2026, clinical and dermoscopic examination de

Demodex blepharitis treatment with a selenium sulfide-containing ointment: a case report.

Demodex blepharitis is a chronic, progressive condition characterized by inflammation, ocular irritation, and erythema due to an overgrowth of mites in hair follicles and oil glands of the eyelids. AZR-MD-001(AZR) is a selenium sulfide-containing ophthalmic ointment that is a potential treatment option for Demodex blepharitis. We wished to complete a case study to identify the potential of this agent to treat Demodex blepharitis. A patient with chronic Demodex blepharitis applied AZR 0.5% to the right lower eyelid every other day at bedtime for 6 weeks, discontinued treatment for 6 weeks, then

Phthiriasis palpebrarum in a 7-year-old girl: a case report.

This case report presents a rare instance of phthiriasis palpebrarum in a 7-year-old female patient. The patient complained of bilateral eyelid pruritus and erythema lasting for 3 weeks. Slit-lamp examination revealed adult lice and nits adhering to the palpebral margins, and microscopic assessment confirmed the diagnosis of Pthirus pubis infestation. Following multiple treatment attempts, all lice and nits were successfully eliminated. With the application of tobramycin eye ointment, the patient achieved full recovery. Phthiriasis palpebrarum is an uncommon condition, especially in pediatric

The Use of Free-Living Amoeba in Treating Bacterial Infections: Thermotolerant Dictyostelids Are Avirulent and Do Not Cause Keratitis in Mice.

PURPOSE: Antimicrobial drug resistance is a significant threat to human health and additional drugs and therapeutic approaches are desperately needed. Amoebozoa is a large and diverse group whose primary mode of nutrition is phagocytosis of bacteria. This feeding behavior raises the possibility that dictyostelids could be used to topically treat bacterial infections. Dictyostelids are generally considered avirulent; however, there are two case reports of keratitis owing to Dictyostelium polycephalum, raising concerns about their therapeutic use. Our goal was to determine whether thermotolerant

[Ocular rosacea : Clinical aspects, diagnostics, management and treatment].

Ocular rosacea is a chronic inflammatory disease that affects the surface of the eye and the eyelids. It often occurs in conjunction with rosacea in the facial area but can also occur independently. It is characterized by bilateral chronic posterior blepharitis and meibomitis, which can involve the entire surface of the eye, including the cornea, during the course of the disease. The diagnosis is largely based on clinical findings. The main symptoms include reddened, burning and itchy eyes, dryness, sensitivity to light and blurred vision. The exact etiology of ocular rosacea is not fully unde

Complications of corneal crosslinking: What can go wrong?

Corneal cross-linking (CXL) is a widely used treatment that is effective in halting the progression of corneal ectasias. Although its therapeutic efficacy is well established, various short- and long-term complications following CXL have been reported. We evaluate the potential complications of the CXL procedure and summarize their management. The most common post-CXL complications include corneal haze, sterile corneal infiltrates, microbial infection, endothelial dysfunction, limbal damage, Herpes simplex reactivation, and treatment failure. These complications may arise from alterations in t

The Role of Prophylactic Topical Administration of Antibiotics in Uncomplicated Cataract Surgery in the Era of Intracameral Antibiotics.

INTRODUCTION: Cataract surgery is one of the most commonly performed procedures in human medicine. Although the risk of postoperative inflammatory complications is low, their consequences can be devastating. The current global standard for preventing such complications is intracameral administration of antibiotics, while the role of prophylactic topical antibiotics in uncomplicated cases remains controversial. AIM: The aim of this paper is to evaluate the available evidence regarding the effectiveness of topical antibiotic prophylaxis in uncomplicated cataract surgery in the era of routine in

The Hidden Culprit Unraveled by Dermoscopy: A Rare Case of Pediatric Facial Demodicosis.

We report a rare case of pediatric facial and ocular demodicosis in an immunocompetent seven-year-old with chronic centrofacial papulopustular lesions and recurrent blepharitis misdiagnosed over several years. Dermoscopic examination revealed a high-density Demodex infestation on facial skin and eyelashes, leading to a definitive diagnosis. A combined treatment approach using topical and systemic metronidazole, along with targeted ocular therapy, achieved complete remission. This case highlights the diagnostic power of dermoscopy and underscores the importance of considering demodicosis in chi

Topical Tacrolimus as a Novel Treatment for Refractory Idiopathic Facial Aseptic Granuloma in a Pediatric Patient.

This report describes the case of a four-year-old girl with idiopathic facial aseptic granuloma (IFAG) who developed persistent, treatment-resistant eyelid and facial inflammation despite multiple courses of antibiotics and corticosteroids. Her presentation included recurrent hordeola, severe blepharitis, and a cheek nodule, with a clinical picture more consistent with IFAG than pilomatrixoma despite histologic evidence of the latter. Standard therapies provided only transient benefit and were limited by systemic side effects. Initiation of topical tacrolimus 0.03% ointment led to rapid and su

Systemic isotretinoin-associated Salzmann-like nodular degeneration in a young adult.

A 19-year-old male undergoing systemic isotretinoin therapy presented with bilateral corneal nodules resembling Salzmann nodular degeneration (SND). He had no prior ocular comorbidities and reported mild itching. Slitlamp examination revealed inflammatory nodules with pannus and blepharitis; optical coherence tomography (OCT) confirmed no stromal involvement. Systemic retinoid therapy was discontinued, and topical antiinflammatory treatment initiated. Despite transient visual decline, the patient improved over six months, with resolution of symptoms and stabilization of the corneal lesions. Th
