Defining Success in Ptosis Surgery: Outcome Heterogeneity and Priorities for Standardization.

Ptosis surgery aims to restore eyelid position, visual function, facial expression, and, in children, visual development. Yet reported success rates reflect how success is defined as much as what surgery achieves. Studies use different eyelid-height thresholds, symmetry criteria, composite definitions, and follow-up intervals, limiting comparison across procedures and patient populations. This narrative review examines outcome assessment after ptosis surgery across six domains: anatomical, functional, aesthetic, patient-reported, safety, and durability outcomes. Margin reflex distance 1 remain

Management of Adult-onset Eyelid Xanthogranuloma: Debulking Surgery Coupled with Intralesional Corticosteroids.

INTRODUCTION: Adult-onset xanthogranuloma (AOX) is a rare, benign inflammatory disorder within the spectrum of non-Langerhans cell histiocytosis, often manifesting as localised skin lesions in middle-aged adults and frequently misdiagnosed due to its nonspecific presentation. CASE: A 43-year-old woman developed a painless, progressively enlarging mass on her right upper eyelid over four months, with histopathology confirming AOX. OBSERVATION: Following surgical debulking and adjunctive intralesional corticosteroid (triamcinolone acetonide) injection, the patient exhibited significant clinica

Extended Superolateral Periosteal Flap for Single-Stage Total Lower Eyelid Reconstruction.

Total lower eyelid reconstruction requires restoration of both lamellae and prevention of postoperative malposition. Traditional two-stage approaches often require a prolonged period of monocular vision. This report describes an extended superolateral periosteal flap harvested along the superior orbital rim to within 2-3 mm of the supraorbital notch, achieving a 35-mm length for single-stage total lower eyelid reconstruction. A 71-year-old woman with basosquamous carcinoma involving the entire lower eyelid underwent excision with 5-mm margins, creating a 30 × 35 mm defect. The periosteal flap

Scleral perforation in a young adult with Burkitt lymphoma.

A male in his 20s with Burkitt lymphoma (BL) treated with chemotherapy was referred for lagophthalmos and left facial nerve palsy and re-evaluated weeks later due to acute scleral perforation. The perforated area was surgically closed, but on the first postoperative day, corneal thinning down to the Descemet membrane was noted, with iris adherence to the endothelium and progressive expansion of the affected sclera. Further globe reconstruction was deemed impossible; therefore, enucleation was performed. Histopathology revealed keratoconjunctivitis and acute perforative scleritis, though the aet

[Xanthelasma palpebrum : mortality risk factor or not ?].

A 44-year old patient presents at the emergencies for a cut above his right eyebrow. Inspection reveals a bilateral bilateral xanthelasma palpebrum (XP). The blood analysis shows a normal cholesterol level: is this enough reassurance as to the patient's cardiovascular and mortality risk? This article reviews evidence showing that XP - a cholesterol papule on the eyelids - is associated with an increased risk for mortality, myocardial infarct and severe atherosclerotic disease, independently from the patient's lipid profile.

Corneal Protection in Facial Synkinesis.

BACKGROUND: Corneal protection is the first priority in acute facial paralysis. Orbicularis oculi paralysis causes paralytic lagophthalmos, ectropion, impaired blink, and exposure keratopathy. However, many patients progress to synkinesis, in which muscle reactivation may improve corneal coverage. This study aimed to characterize the longitudinal trajectory of corneal status as patients transition from acute facial paralysis to synkinesis. METHODS: A retrospective cohort study of synkinetic patients evaluated by ophthalmology between 2009 and 2025 was conducted. Included were patients with at

Comparative evaluation of curcumin and resveratrol in breast cancer: network pharmacology, molecular docking, and in-vitro radio-modulating validation in MDA-MB-231 cells.

INTRODUCTION: There is no distinct classification system to evaluate the bone defect in previously managed acetabular fractures. We propose a new classification system for bone defect evaluation in a previously managed acetabular fracture that will be helpful for total hip arthroplasty (THA). MATERIALS AND METHODS: The preoperative pelvis radiographs of 99 THA patients with previous acetabular fractures with at least 2 years of follow-up were evaluated by 10 experienced surgeons (Paprosky and new classification systems). As per the new classification system, the five types of bone defects are

[Progressive exophthalmos as a manifestation of adult xanthogranulomatous orbital disease].

Adult xanthogranulomatous orbital disease (AXOD) is an uncommon condition belonging to the group of non-Langerhans cell histiocytoses. We present the case of a male with a recent diagnosis of adult-onset asthma, who presented with progressive bilateral eyelid edema of one year's duration. Imaging studies revealed diffuse thickening of orbital fat and extraocular muscles. Skin biopsy confirmed the diagnosis of AXOD. The patient was treated with intraorbital (triamcinolone) and systemic (meprednisone) corticosteroids, with clinical and radiological improvement. The final diagnosis was AXOD in it

Efficacy and safety of surgical excision vs. 70% trichloroacetic acid topical application in xanthelasma palpebrarum: a split-face randomized clinical trial.

BACKGROUND: Despite its frequency, there is a lack of comparative evidence evaluating available treatment modalities for xanthelasma palpebrarum (XP). Although surgical excision and trichloroacetic acid (TCA) are commonly used interventions for XP, no study has compared these two modalities. OBJECTIVES: To compare the efficacy and safety of surgical excision vs. 70% TCA in the management of limited XP. METHODS: A prospective, split-face, single-blinded, single-centre randomized clinical trial conducted from November 2024 to August 2025 with a 12-week follow-up. Adults aged ≥ 18 years with bi

Eyelid Xanthelasma and Cardiovascular Outcomes: A Propensity-Matched Analysis.

PURPOSE: Xanthelasma palpebrarum (XP) is a common periocular lesion often regarded as benign. However, its potential association with systemic lipid dysregulation and cardiovascular disease remains controversial. This study aimed to evaluate the relationship between XP, dyslipidemia, and major cardiovascular outcomes using a large-scale, electronic health record dataset. METHODS: This retrospective propensity score-matched cohort study used de-identified electronic health record data from TriNetX, a global federated research network. Adults aged 20 to 85 years with and without XP were identif

Adnexal necrobiotic xanthogranuloma presenting as a "kissing" eyelid lesion.

Necrobiotic xanthogranuloma (NBX; also termed NXG) is a rare non‑Langerhans cell histiocytosis within the adult orbital xanthogranulomatous disease spectrum. It typically presents with uni‑ or bilateral periocular yellow‑brown infiltrates and can mimic xanthelasma and other inflammatory lesions, with possible systemic associations including paraproteinemia and hematologic malignancy. A 50‑year‑old man presented with an uncommon phenotype limited to the ipsilateral upper and lower eyelids, forming a "kissing" lesion across the lid margin. Three prior debulking procedures performed elsewhere wit

Novel and Simple Punch Treatment for Xanthelasma Palpebrarum: A Preliminary Case Series.

BACKGROUND: Xanthelasma palpebrarum (XP) is a common cutaneous condition of the eyelid, which compromises eyelid aesthetics. Non-surgical treatments are often limited by multiple sessions and high cost. Surgical treatment, although effective, is constrained by the limited availability of eyelid skin and the risk of ectropion. A simple, safe, and effective therapeutic technique is essential for managing XP. AIMS: This study aimed to present a new dermatologic treatment for XP and assess its effectiveness. METHODS: A novel technique, punch-assisted xanthelasma nucleus excision (PAXNE), was app

Lymphangiogenesis in hypercholesterolemic skin: A comparative study of xanthomatous and non-xanthomatous lesions.

While hypercholesterolemia is the primary driver of atherosclerosis, emerging evidence indicates that lymphatic vessels are critical for cholesterol transport and become impaired under hyperlipidemic conditions. Xanthelasma palpebrarum, characterized by cholesterol-laden yellowish plaques on the eyelids, is a cutaneous manifestation of dyslipidemia, yet the relationship between local cholesterol accumulation and dermal lymphatic function remains elusive. Here, we compare the lymphatic vessel morphology of xanthomatous and non-xanthomatous skin lesions in patients with various levels of serum l
