Marcus Gunn jaw-winking ptosis and contralateral lid retraction: management with Müller muscle-conjunctival resection with tarsectomy.

PURPOSE: Marcus Gunn jaw-winking syndrome (MGJWS) is a rare congenital cranial dysinnervation disorder characterized by synkinetic eyelid elevation during jaw movement. Standard surgical management often involves levator excision with bilateral frontalis suspension, which abolishes levator function but carries morbidity and variable cosmetic outcomes. Posterior ptosis approaches such as Müller muscle-conjunctival resection (MMCR) are well established for phenylephrine-responsive ptosis, but their role in MGJWS remains unclear. METHODS: A 12-year-old male with congenital left upper eyelid ptos

Efficacy of intradermal acupuncture for ocular surface diseases secondary to refractory peripheral facial palsy: a randomized controlled trial study protocol.

INTRODUCTION: Refractory Peripheral Facial Palsy (RPFP) is defined as peripheral facial paralysis (PFP) that fails to achieve significant clinical recovery despite conventional therapeutic interventions. Due to severe facial nerve injury and a prolonged recovery course, RPFP often results in lagophthalmos, which in turn predisposes to ocular surface diseases (OSDs), most commonly exposure keratitis and dry eye disease. Currently, symptomatic and supportive therapies such as sodium hyaluronate eye drops are standard for managing OSDs secondary to RPFP, yet no other effective treatments have bee

Oral Sirolimus Treatment for HRAS-Related Epidermal Nevus Syndrome.

This is the case of a 10-year-old female with HRAS-related epidermal nevus syndrome with extensive and severe cutaneous involvement treated with oral sirolimus. The patient had progressively enlarging verrucous periorbital plaques involving the eyelids, resulting in lagophthalmos and threatening vision. Oral sirolimus rapidly improved the cutaneous lesions and resolved the lagophthalmos. This case of a patient treated with oral sirolimus for HRAS-related epidermal nevus syndrome suggests that therapies targeting the mTOR pathway may indirectly affect the RAS/MAPK pathway.

Frontal pneumosinus dilatans as a cause of delayed post-traumatic proptosis in a patient with chronic facial nerve palsy.

Pneumosinus dilatans (PSD) is a rare expansile disorder characterized by enlargement of an aerated paranasal sinus beyond its normal anatomic boundaries while preserving bony integrity. We report a 26-year-old man with progressive unilateral proptosis presenting 18 years after severe craniofacial trauma complicated by permanent sixth and seventh cranial nerve palsies. Examination demonstrated 5 mm axial left proptosis with paralytic lagophthalmos, lower eyelid retraction, and exposure keratopathy. Computed tomography revealed marked left frontal sinus enlargement (62.7% volumetric increase) wi

Sequential oculoplastic reconstruction in an irradiated field after resection of large NF1-associated orbital MPNST: a case report.

We report a 25-year-old male who underwent en bloc resection of a large NF1-associated high-grade malignant peripheral nerve sheath tumor (MPNST) and oculoplastic reconstruction. Following adjuvant chemo-radiotherapy and 3 years of oncologic surveillance, sequential oculoplastic refinements were performed to address radiation-induced periorbital and facial fibrosis, ptosis, and volume loss. These included brow lift, ptosis repair, blepharoplasty, lateral canthal lengthening, and autologous fat grafting. At one-year follow-up after reconstruction: symmetrical palpebral fissures (30 mm OD, 31 mm

Incomplete Ascher's Syndrome Misdiagnosed as Eczema: A Case Report.

Ascher syndrome is a rare disorder classically characterized by blepharochalasis, double lip, and non-toxic thyroid goiter. We report a case initially diagnosed as eczema in the dermatology department, who presented with acquired double lips and blepharochalasis of the upper eyelids, without thyroid enlargement. Magnetic resonance imaging revealed uniformly increased T2-weighted signals in both the upper and lower lips, leading to a diagnosis of incomplete Ascher's syndrome. The patient underwent oculoplastic surgery, which successfully restored the natural aesthetic contour, demonstrating the

Double-bridge novel modification of Cutler-Beard flap for the reconstruction of full-thickness eyelid deformity.

INTRODUCTION: Eyelid deformities can come from birth defects or other causes, and they often need full thickness reconstruction. The Cutler-Beard and Hughes techniques are still the best methods for repairing upper eyelid colobomas. However, there are not many choices available when the eyelid margin is still intact. Here, we report the clinical outcomes of a novel modification of the Cutler-Beard flap surgery in eyelid deformity for these difficult cases. METHODS: Design: Retrospective study. Setting: Tertiary care hospital. Study Population: All patients who presented with significantly def

Congenital encephalomeningocele presenting as a giant orbital mass in a neonate: a case report.

BACKGROUND: Congenital orbital masses encompass a broad spectrum of etiologies, among which encephalomeningocele is exceedingly rare and particularly diagnostically challenging when atypical. CASE PRESENTATION: We report a 2-hour-old female neonate who presented with a giant left orbital mass causing marked proptosis. Imaging revealed a mixed cystic-solid lesion with calcifications and septations, but no bony defect was identified preoperatively. A transcranial approach was performed, revealing a small orbital roof defect with herniation into the anterior cranial fossa. Histopathology confirm

[Botulinum therapy within a three-stage medical rehabilitation framework for patients with disorders of craniofacial muscles].

Disorders of craniofacial muscles (CFM) frequently arise from various conditions and injuries affecting the central and peripheral nervous systems, as well as from neurosurgical, reconstructive, and cosmetic interventions. The principal clinical manifestations include facial asymmetry, lagophthalmos, paralytic strabismus, synkinesis, contractures, trismus, bruxism, temporomandibular joint dysfunction, chronic facial pain, and sialorrhea. Such disorders not only restrict daily functioning but also lead to significant psychosocial maladjustment, a decline in quality of life, and increased disabi

Molecular Basis and Clinical Spectrum of WNT10A-Related Oligodontia.

WNT10A mutations, a major genetic determinant of dental agenesis and ectodermal dysplasia, exert profound effects on craniofacial development. Although classified as rare disorders, these mutations account for more than half of oligodontia cases, reflecting their critical role. The associated phenotypes span a broad clinical spectrum, ranging from isolated tooth agenesis to complex multisystem disorders, including hypodontia, enamel and root defects, palmoplantar keratoderma, alopecia, nail dystrophy, skin xerosis, hypohidrosis, eyelid cysts, and skin cancer, skeletal abnormalities and impaire

Development of Human Conjunctival Goblet Cell Segmentation Datasets to Improve Quantitation.

Dry eye disease is an inflammatory disease of the ocular surface and one of the most common pathologies in medicine. This multifactorial disease can cause significant morbidity and visual disturbance. The ocular tear film consists of an outer lipid layer, an underlying aqueous layer and an innermost mucus layer produced by goblet cells interspersed in the conjunctiva. This inner mucus layer is vital for tear film stability, immunoregulation and ocular surface health. With dry eye disease and several other ocular surface pathologies the goblet cells can be affected, with decreased density and f

Molecular Characterisation of Treacher Collins Syndrome in a South African Cohort: Novel Disease-Causing Variants in TCOF1 and POLR1D.

BACKGROUND: Treacher Collins syndrome (TCS) is a rare craniofacial disorder characterised by variable expressivity. It is caused by pathogenic variants in the TCOF1, POLR1D, POLR1C, or POLR1B genes. Common clinical features include hypoplasia of the zygomatic complex and mandible, downward-slanting palpebral fissures, lower eyelid anomalies, microtia, and hearing loss. Owing to its phenotypic overlap with other craniofacial syndromes, molecular testing is essential for establishing an accurate diagnosis and guiding effective clinical management. METHODS: Ten South African patients with a susp

How to manage thyroid eye disease: recent advances - a narrative review.

Thyroid eye disease (TED) is a complex autoimmune disorder and the most common extrathyroidal manifestation of Graves' disease, characterized by orbital inflammation, tissue remodelling, and varying degrees of functional and cosmetic impairment. Although often mild, moderate-to-severe and sight-threatening forms can significantly compromise vision and quality of life. Advances in the understanding of TED pathophysiology - particularly the roles of orbital fibroblasts, thyrotropin receptor and insulin-like growth factor 1 receptor signalling- have transformed its diagnostic and therapeutic land
