The Role of Corneal Sensation in Facial Nerve Paralysis.

PURPOSE: Patients with facial nerve paralysis (FNP) can have corneal decompensation resulting in poor visual outcomes. Here, we measure the associations between clinical parameters and corneal epitheliopathy in FNP. METHODS: In this cross-sectional cohort study, FNP patients from a 6-year period were reviewed. Etiology of FNP, visual acuity, grade of corneal epitheliopathy, and corneal sensation were recorded. Multivariate analysis was completed to assess the association between severe epitheliopathy and 4 ocular protective mechanisms: corneal innervation, House-Brackmann orbicularis score, B

A Muscle-Based Approach to Superior Orbital Sulcus Hollowness: Preseptal Orbicularis Oculi Flap in Upper Blepharoplasty.

BACKGROUND: Superior orbital sulcus hollowness is a common aesthetic concern that may result from structural changes, trauma, or excessive fat removal during upper blepharoplasty, and it can be further exacerbated by age-related orbital remodeling. Traditional corrective approaches such as fat redistribution and autologous fat grafting carry limitations, including graft atrophy and the risk of embolic complications. This study evaluated the safety and outcomes of superior orbital sulcus correction using a medial-pedicled preseptal orbicularis oculi muscle flap. METHODS: This retrospective stu

Extended endoscopic transnasal orbital decompression for dysthyroid optic neuropathy: efficacy and prognostic factors.

BACKGROUND: Dysthyroid optic neuropathy (DON) is a vision-threatening complication of Graves' ophthalmopathy, and insufficient orbital decompression may result in persistent or irreversible visual impairment. While endoscopic transnasal orbital decompression (ETOD) has become a mainstream surgical approach for orbital decompression, the optimal extent of ETOD remains incompletely defined, particularly regarding reproducible anatomical landmarks and safe surgical boundaries. METHODS: This retrospective study evaluated patients with DON who underwent an extended ETOD procedure at a tertiary med

Eyelid Elevation and Ocular Surface Rehabilitation Using Quadrant-Asymmetric Scleral Contact Lenses in Facial Nerve Palsy with Keratoconus: A 2-Case Series.

INTRODUCTION: Facial nerve palsy (FNP) with associated ptosis and keratoconus (KC) presents a combined challenge of visual dysfunction, ocular-surface disease, and cosmetic impairment. While surgical procedures are frequently performed, scleral contact lenses can offer both ocular-surface protection and mechanical eyelid support. While scleral lenses have previously been described for exposure keratopathy and irregular corneal astigmatism, the present 2-case series provides objective margin reflex distance-1 (MRD1) documentation of eyelid elevation achieved through quadrant-asymmetric scleral

Multiple gold weight implants to treat lagophthalmos and improve contour.

PURPOSE: To report a novel approach for managing persistent lagophthalmos and eyelid contour abnormality after initial gold weight implantation. OBSERVATIONS: Paralytic lagophthalmos is defective eyelid closure due to facial nerve damage. One common, effective treatment is lid loading with eyelid weights. By implanting a weight into the upper eyelid, gravity is used to help close the eyelid, decrease corneal exposure, and prevent further disease such as exposure keratopathy. This case details a patient with paralytic lagophthalmos post resection of a vestibular schwannoma. She underwent gold

Secondary Lower-Motor-Neuron Facial Palsy Revealing Buccal Squamous Cell Carcinoma with Parotid Duct Obstruction and Perineural Invasion.

Peripheral facial palsy is often attributed to idiopathic Bell's palsy, but secondary structural causes should be considered when local red flags are present. A 61-year-old man with a 40-pack-year smoking history and former betel quid chewing presented with a verrucous-appearing mass involving the left oral commissure and buccal mucosa, intermittent purulent discharge from the lesion, progressive left facial swelling, and ipsilateral lower-motor-neuron facial palsy with lagophthalmos. Magnetic resonance imaging demonstrated a left buccal/oral-cavity lesion with ipsilateral parotid duct obstruc

Marcus Gunn jaw-winking ptosis and contralateral lid retraction: management with Müller muscle-conjunctival resection with tarsectomy.

PURPOSE: Marcus Gunn jaw-winking syndrome (MGJWS) is a rare congenital cranial dysinnervation disorder characterized by synkinetic eyelid elevation during jaw movement. Standard surgical management often involves levator excision with bilateral frontalis suspension, which abolishes levator function but carries morbidity and variable cosmetic outcomes. Posterior ptosis approaches such as Müller muscle-conjunctival resection (MMCR) are well established for phenylephrine-responsive ptosis, but their role in MGJWS remains unclear. METHODS: A 12-year-old male with congenital left upper eyelid ptos

Efficacy of intradermal acupuncture for ocular surface diseases secondary to refractory peripheral facial palsy: a randomized controlled trial study protocol.

INTRODUCTION: Refractory Peripheral Facial Palsy (RPFP) is defined as peripheral facial paralysis (PFP) that fails to achieve significant clinical recovery despite conventional therapeutic interventions. Due to severe facial nerve injury and a prolonged recovery course, RPFP often results in lagophthalmos, which in turn predisposes to ocular surface diseases (OSDs), most commonly exposure keratitis and dry eye disease. Currently, symptomatic and supportive therapies such as sodium hyaluronate eye drops are standard for managing OSDs secondary to RPFP, yet no other effective treatments have bee

Oral Sirolimus Treatment for HRAS-Related Epidermal Nevus Syndrome.

This is the case of a 10-year-old female with HRAS-related epidermal nevus syndrome with extensive and severe cutaneous involvement treated with oral sirolimus. The patient had progressively enlarging verrucous periorbital plaques involving the eyelids, resulting in lagophthalmos and threatening vision. Oral sirolimus rapidly improved the cutaneous lesions and resolved the lagophthalmos. This case of a patient treated with oral sirolimus for HRAS-related epidermal nevus syndrome suggests that therapies targeting the mTOR pathway may indirectly affect the RAS/MAPK pathway.

Frontal pneumosinus dilatans as a cause of delayed post-traumatic proptosis in a patient with chronic facial nerve palsy.

Pneumosinus dilatans (PSD) is a rare expansile disorder characterized by enlargement of an aerated paranasal sinus beyond its normal anatomic boundaries while preserving bony integrity. We report a 26-year-old man with progressive unilateral proptosis presenting 18 years after severe craniofacial trauma complicated by permanent sixth and seventh cranial nerve palsies. Examination demonstrated 5 mm axial left proptosis with paralytic lagophthalmos, lower eyelid retraction, and exposure keratopathy. Computed tomography revealed marked left frontal sinus enlargement (62.7% volumetric increase) wi

Double-bridge novel modification of Cutler-Beard flap for the reconstruction of full-thickness eyelid deformity.

INTRODUCTION: Eyelid deformities can come from birth defects or other causes, and they often need full thickness reconstruction. The Cutler-Beard and Hughes techniques are still the best methods for repairing upper eyelid colobomas. However, there are not many choices available when the eyelid margin is still intact. Here, we report the clinical outcomes of a novel modification of the Cutler-Beard flap surgery in eyelid deformity for these difficult cases. METHODS: Design: Retrospective study. Setting: Tertiary care hospital. Study Population: All patients who presented with significantly def
