Bilateral blepharochalasis.

Blepharochalasis is a rare disorder characterized by recurrent painless periorbital oedema, which leads to atrophy of the periorbital skin. Pathomechanism of the disease is probably immunological with a nearly complete loss of elastic fibres. The authors describe a 17-year-old woman, who was followed for 7 years. IgA deposits were found in the periorbital tissues, which confirm the immunological background of the condition. Electron microscopy results show that not only elastic but also collagen fibres were affected.

Repetitive Er:YAG laser irradiation of human skin: a histological evaluation.

BACKGROUND AND OBJECTIVE: Deep coagulation of skin collagen by Er:YAG laser repetitive pulses has been predicted by previous theoretical models and later demonstrated on animal skin. The goal of this study was to determine the effect of repetitive Er:YAG laser pulses on human skin and its response to this treatment. STUDY DESIGN/MATERIALS AND METHODS: Lid skin of six female volunteers with blepharochalasis has been treated with laser at day 0, 7, and 21 before elective surgery-blepharoplasty. The treated skin was excised as part of the procedure and prepared for further histological examinati

Lax eyelid syndrome or 'progeria' of eyelid tissues.

PURPOSE: The lax eyelid syndrome was described by Van den Bosch and Lemij as an uncommon disorder seen in non-obese elderly people and characterised by chronic ocular surface irritation symptoms and a "floppy upper eyelid". The authors present some new features of the lax eyelid syndrome. METHODS: The authors report five patients, belonging to a younger age group, who presented with premature laxity of all the eyelid tissues. This caused medial and lateral canthal dystopia and eyelid malposition including ptosis, entropion and ectropion. Initial surgical correction was often followed by recur

[Etiology of eyelid edema].

Lidedema can be a common symptom of diseases of the orbital part of the face. One can differentiate lidedema in two groups, those that are related to an inflammation and those that are not related to an inflammation. Lidedema of the eyelid can also be secondary to a systemic disease. There is also another group that is common in the ageing population that consists of blepharochalasis and dermatochalasis where the skin becomes loose with the years and forms redundant folds.

Long-term in vivo pineal microdialysis.

Respiratory infections in infancy may protect against developing Th2-mediated allergic disease (hygiene hypothesis). To estimate the relative contribution of particular viruses to the development of the immune system and allergic disease, we investigated longitudinally the prevalence of respiratory viral infections in infants. One hundred and twenty-six healthy infants were included in this prospective birth cohort study in their first year of life. Physical examination was performed and nasal brush samples were taken during routine visits every 6 months and during an upper respiratory tract i

Unilateral blepharochalasis.

Unilateral blepharochalasis is an extremely rare disorder with an unknown etiology and pathogenesis. The authors present a 22-year old patient in whom a right-sided skin overhang of the upper eyelid caused visual field impairment. The condition was corrected by a standard blepharoplasty. The histological examination suggested a localised anomaly of the lymphatic system and an almost complete absence of elastic fibres as the causative agent.

Congenital cutis laxa syndrome: type II autosomal recessive inheritance.

Cutis laxa is a term that refers to markedly loose skin that is not hyperelastic. It is regarded as a genetically heterogeneous group of diseases and is presently divided into five types. We report a male patient with type II autosomal recessive disease. The patient was the third child of first-cousin consanguineous, healthy parents. His two siblings died a few hours after birth. One of the siblings also had similar features and wrinkled skin. Our case had markedly loose and wrinkled skin especially over the dorsum of the hands and feet, and on the face and abdomen, dolichocephaly, hypertelori

Progressive autosomal dominant optic atrophy and sensorineural hearing loss in a Turkish family.

PURPOSE: To describe the clinical features, mode of inheritance, and linkage analysis of ten affected members of a three-generation family with progressive optic atrophy and progressive hearing loss. MATERIALS AND METHODS: The proband, a 10-year-old boy, presented with progressive visual failure. Ten other members in his family, including his mother, half-sister, aunt, two uncles, grandfather, and some of the cousins, also had progressive visual loss and hearing loss. Six affected and four unaffected cases were examined in detail. Blood samples were drawn from 16 members for DNA extraction. T

[Unilateral blepharochalasis with IgA-deposits].

A 36-year-old male patient presented with unilateral periocular skin atrophy. The blepharochalasis developed without any obvious inflammation of the eyelids over the past 10 years. Interestingly, elongated blood vessels and microaneurysmatic vessel changes were found in the tarsal conjunctiva. A punch biopsy revealed a nearly complete loss of elastic fibres in the papillary and superficial reticular dermis. The contralateral side was histopathologically normal. On immunohistology IgA-deposits could be observed especially on perifollicular elastic fibres. Immunoelectronmicroscopy confirmed the

Ophthalmic manifestations of nasal obstruction can involve palpebral, orbital and ophthalmic disturbances. They can be the consequence of: directly with the nasal obstruction because the orbital cavity drains partly in the nasal fossae: rings, lipoptosis, blepharochalasis, fat protrusions, aggravation of exophthalmy in Grave's disease, enophthalmia. Oral breathing: there are modifications in the shape of the palpebral fissure ("round eye") by stretching of the facial mask and modifications of the orbital rims ("sad eye") due to lack of naso-sinusal expansion, often associated to malocclusion,

[Eye diseases in sleep apnea syndrome].

Sleep apnea syndrome is characterized by recurrent complete or partial upper airway obstructions during sleep and recognized as an important risk factor for cardiovascular and cerebrovascular diseases. Several eye diseases have been associated with sleep apnea syndrome. Due to floppy eyelids often a chronic conjunctivitis occurs. A leaking mask used for apnea treatment may also induce a conjunctivitis. The lids may show an upper lid ptosis, lower lid ectropium, blepharochalasis, or trichiasis. Corneal findings include infectious keratitis, dry eye, recurrent erosion, keratoconus, and progressi

Maxillary double lip: report of two cases.

Two cases of double lip malformation, an uncommon oral anomaly, are presented, and the factors involved in the development, pathogenesis, diagnosis, and treatment of these uncommon lesions are reviewed. Double lip is usually associated with Ascher's syndrome, which is a rare disease with three more or less consistently associated abnormalities: double upper lip, blepharochalasis and enlargement of the thyroid. Two cases of double lip are reported; one of which was related with Ascher's syndrome, while the other had a traumatic origin.

[Prevention of eyelid retraction following lower blepharoplasty].

This review covers the techniques used to prevent lid retraction after lower blepharoplasties. After a brief overview of the lower lid anatomy (anterior, middle, posterior lamella), the causes of blepharochalasis and of lid retraction are addressed. Clinical examinations are described that can detect a candidate at risk. Many preventive measures are discussed, examining patient positioning, approaches (transcutaneous, transconjunctival, combination), incision types, flap dissection (skin, skin-muscle, dermal flaps), suspension techniques (muscle-muscle, muscle-periosteum, horizontal wedge exci

Correction of the epicanthal fold using the VM-plasty.

The VM-plasty combines the principles of YV-plasty and multiple Z-plasty. It consists of three V flaps. For correction of the epicanthal fold, both A and B V-flaps were designed on the outer surface and the C V-flap was designed on the inner surface of the fold. The vertical tissue deficiency of the epicanthal fold was reconstructed with the C V-flap which brought additional tissue from the inner surface of the fold. The VM-plasty for epicanthoplasty was used alone in 8 patients, in combination with double eyelid surgery in 51 young patients and in combination with blepharoplasty and double ey

Bilateral eyelid edema: cutis laxa or blepharochalasis?

A 59-year-old woman with massive bilateral edema of the upper and lower eyelids is presented. The edema occurred suddenly and without provocation. No cause could be identified despite a multitude of examinations. Initially the patient was diagnosed as having blepharochalasis, but later skin biopsy suggested an acquired form of cutis laxa. Most likely the cutis laxa was a result of angio-edema. Pharmacological treatment was not successful, but surgical intervention provided an acceptable result. The differential diagnosis of periorbital edema is discussed with special attention to cutis laxa, a

Pathological ptosis of the eyelids can be manifest already at birth but may develop also in advanced age. In children the congenital form predominates with or without affection of the elevators of the eyes (Congenital fibrous syndrome or Misdirection syndrome). A special form is blepharophimosis, juvenile blepharochalasis and Marcus-Gunn's syndrome. In adult age the etiology of acquired ptosis of the upper eyelid is more varied. There are mechanical causes incl. dermatochalasis or it develops on a myogenic and neurogenic background or as the aponeurotic form. The group comprises 46 patients 3-
