The most common "physiologic" change that occurs in the eyes after age 40 is loss of elasticity of the lens, resulting in presbyopia. Xanthelasmas commonly occur at this age and should be removed only for cosmetic reasons. Entropion and ectropion can be quite irritating; both results from laxity of the eyelid tissues. Relaxation of eyelid tissues also contributes to blepharochalasis. Glaucoma and formation of cataracts are not normal changes but do appear with increasing frequency in persons over 40.

Blepharochalasis is a distinct entity of redundant eyelids. It is characterized by a history of recurrent edema that leads to permanent changes in the eyelids. Even in the young, blepharoplasty may be indicated for psychological reasons or to relieve obstruction of vision, eyelid asthenia, and fatigue. Following surgery, recurrent edema may compromise the results or cause further eyelid changes. Effort should be made to define the etiology of the edema and correct the defect if possible.

A rare case of bilateral blepharochalasis of the upper eyelids is presented in a 10-year-old boy with several systemic abnormalities--unilateral agenesis of the left kidney, multiple skeletal anomalies of the vertebral column, and congenital heart disease with a left to right shunt. Such widespread congenital defects in association with blepharochalasis have not been described before. Blepharochalasis may represent a part of a more generalised disorder.

A 79-year-old white man of Irish descent presented with lattice corneal dystrophy, blepharochalasis, and peripheral seventh cranial nerve palsies. Family studies revealed that his 23-year-old daughter had early lattice cornea dystrophy. The corneal button removed by penetrating keratoplasty exhibited characteristic amyloid accumulation by light and electron microscopy. During the past 10 years, the authors have observed several cases of malfunction of the eyelids, such as congenital lid retraction, lagophthalmos accompanying facial nerve palsy, ocular myopathy, blepharochalasis, traumatic lid ptosis, and Horner's syndrome. Several operational techniques were utilized in these cases. Some operative cases are presented together with pre- and postoperative photographs. To obtain aesthetically pleasing results, the authors felt the most exacting techniques were necessary for even minor deformities.

True blepharochalasis occurring in young adults and associated with recurrent bouts of eyelid swelling and eventual lid laxity is an uncommon entity. We report the case of an 18-year-old woman who had a nine-year history of unilateral blepharochalasis. A skin biopsy specimen that showed the absence of stainable elastic tissue confirmed the clinical impression, and appropriate surgical correction was carried out.

The subject of the esthetic qualities of the upper eyelid-eyebrow complex is discussed and how the cosmetic surgeon should diagnostically evaluate the problems occurring in this area. Emphasis is put on the other contributing factors that may contribute to blepharochalasis of the upper lid other than redundant skin limited to the upper lid space. A blepharoplasty technique with an incision at the supratarsal fold and evaluation of a skin muscle flap is discussed briefly.

We designed an instrument to facilitate the operation for blepharochalasis. The skin to be excised is lifted into a fold and the latter than stabilized by tightly clamping the two layers of skin at its base with the new instrument. In this way the situation after excision can be simulated and the position of the future scar and the apposition of the lids against the globe can be estimated in advance and the fold can be adjusted if required. In addition, the compression of the skin decreases the tendency for bleeding.

Herniation of the lacrimal gland is a condition that occurs unilaterally or bilaterally. Either or both lobes of the lacrimal gland may prolapse. Orbital lobe prolapse may be associated with blepharochalasis, a disease of puberty. We developed a surgical technique for the repair of prolapsed lacrimal glands. It is important to have adequate hemostasis and closure of the orbital septum.

Dermatochalasis and blepharochalasis of the upper lids, in addition to producing a cosmetic defect, causes asthenopic symptoms. A blepharoplasty for dermatochalasis removes skin only. Blepharochalasis removes protruding fat, tightens the orbital septum, and, in addition, when indicated removes excess skin. The surgical techniques for these procedures are presented.

The case report of a 20 year old woman illustrates the course of blepharochalasis. This disease was defined as an entity by E. Fuchs in 1896. It is characterized by relapsong edematous tumefaction and increasing relaxation of the eye lids with atrophy of the skin, blepharophimosis and emergence of a pseudoepicanthus. Folding of the oral mucosa in the upper lip, the so-called double lip, euthyroid struma, and, in later stages, orbital fat hernia and prolapse of the lacrimal gland, are further facultative symptoms. Blepharochalasis is a redundancy of the eyelid skin. It may be associated with herniation of orbital fat, and the eyelids may have a baggy appearance. They may become heavy and reduce the visual field. Blepharoplasty, in which the excess skin and fat are removed, can not only improve appearance but can also improve ocular function.
