The association between necrobiotic xanthogranuloma of the dermis and paraproteinemia and/or B-cell malignancy is best described in the ophthalmologic literature. We report a case which occurred in the eyelid and orbit of a 64 year old man that led to the diagnosis of an IgA multiple myeloma. To our knowledge, this is the first report of an IgA type paraproteinemia and IgA type multiple myeloma associated with necrobiotic xanthogranuloma.

Until now surgery was the only means of removing xanthelasmas. However, their proximity to blood vessels and consequent tendency to bleed makes visual supervision of the excision difficult. The CO2 laser's infrared wavelength permits photovaporization of tissue with excellent hemostasis. It is thus an ideal tool for removing xanthelasmas. Visual supervision is excellent, and the laser's shallow penetration depth permits layerwise removal of the yellow foam cell plaques. The resulting lesions heal without significant scarring, and deeper layers of the skin remain unharmed. The procedure is fast

The fasting plasma lipid and lipoprotein levels were measured prospectively in 41 consecutive patients with xanthelasma seen over a four-year period. The study group included 25 women and 16 men with mean ages of 60 and 56 years, respectively. Necrobiotic xanthogranuloma with paraproteinemia is a distinct clinicopathologic entity defined by skin lesions that are characteristic both clinically and histologically, as well as a by variety of hematologic and chemical abnormalities. It is frequently associated with multiple myeloma or chronic lymphocytic leukemia. A patient with the characteristic findings but with an unusual course is described.

We describe an unusual case of an aggressively invasive, recurrent xanthelasma palpebrarum which was unilateral in its occurrence. Despite characteristics that appeared malignantly invasive clinically, the lesion was benign. The surgical treatment consisted of wide excision and resurfacing of the eyelids with full-thickness skin grafts.

Six patients with xanthelasma palpebrarum of the eyelids have been treated with the CO2 laser and followed for periods varying up to 4 years. Satisfactory cosmetic results were achieved in all patients without significant scarring, and no recurrence has been discovered thus far.

We used an orbicularis oculi muscular pedicle musculocutaneous flap to close the defect resulting from removal of a large xanthoma from an Oriental upper eyelid. This is an excellent method not only to save eyelid skin but also to make a superior palpebral fold at the same time.

A case of multicentric reticulohistiocytosis associated with bilateral Dupuytren's disease and xanthelasmas of the eyelids is described. Ultrastructural examination showed electron-dense intracytoplasmic granules, collagen entrapment and interdigitation of adjacent cytoplasmic membranes.

The prevalence rates of xanthelasma and corneal arcus were computed for normolipidemic and dyslipoproteinemic participants in the Lipid Research Clinics population surveys. A patient with an undetectable meningocele communicating with the lower eyelid is reported. The condition was first suspected during surgical removal of xanthelasmata when liquorrhoea was observed. The diagnosis was confirmed by computer tomography.
