A Case of Eosinophilic Granulomatosis With Polyangiitis Presenting With Conjunctival Lesions During Treatment With IL-5 Inhibitors.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic rheumatic disease characterized by necrotizing vasculitis in the small vessels with eosinophilic inflammation. EGPA involves multiple organs, such as the heart, lungs, kidneys, gastrointestinal tract, nerves, and ophthalmic organs. Ophthalmic involvement includes conjunctival nodules, orbital myositis, orbital inflammatory syndrome, dacryoadenitis, and cranial nerve palsy. Notably, conjunctival nodules in patients with EGPA are very rare lesions but may present with atypical eyelid swelling. Herein, we present a case of EGPA wi

An Overview of IgG4-Related Disease.

IgG4-related disease is a chronic inflammatory condition that has been formally described and extensively researched only in the recent few decades, making it a relatively new clinical entity. Furthermore, IgG4- related disease has been highly underreported and misdiagnosed as other similarly presenting conditions. In this article, we discuss the variable presentation, diagnosis process and criteria of IgG4-related disease and Sjogren's syndrome in consideration of the literature currently available. In IgG4- related disease patients, an accurate diagnosis is crucial to timely treatment and ci

Orbital inflammation in VEXAS syndrome.

INTRODUCTION: Vacuoles, E1-ligase, X-linked Auto-inflammatory, Somatic (VEXAS) syndrome is a recently described multisystem inflammatory disorder. Ocular features are described, though not well known; we aimed to describe them. METHODS: This is a case series of VEXAS patients submitted by ophthalmologists from relevant specialty organisations, including the Australian and New Zealand Society of Ophthalmic Plastic Surgeons, British Oculoplastic Surgical Society and the Orbital Society. Patient consent was received. RESULTS: 14 males with VEXAS syndrome and eye features were included, median a

When Tears Signal Vasculitis: Bilateral Dacryoadenitis as the Initial Manifestation of Granulomatosis with Polyangiitis-Case Report.

Background and Clinical Significance: Granulomatosis with polyangiitis (GPA) is an ANCA-associated vasculitis that often affects the respiratory tract and kidneys, while ocular involvement is less common and may delay diagnosis. Bilateral dacryoadenitis as an initial manifestation is particularly uncommon and can obscure early recognition. Case Presentation: A 24-year-old woman presented with recurrent epistaxis, headaches, and progressive bilateral eyelid swelling. MRI showed enlarged lacrimal glands consistent with granulomatous dacryoadenitis. Over the following weeks, she developed systemi

Imaging features of lacrimal gland disease.

Lacrimal gland masses represent a diverse group of pathological processes, including inflammatory, lymphoproliferative, and neoplastic lesions. They often present as a palpable mass in the superolateral orbit. There is significant overlap in clinical presentation, and imaging with ultrasound, computed tomography, and magnetic resonance imaging is essential for further characterisation. Key radiological features such as laterality, lobe involvement, lesion composition, margin definition, enhancement pattern, and associated bony changes can significantly narrow the differential diagnosis. This r

A rare case of infantile dacryoadenitis: a case report.

A 10-month-old boy, otherwise healthy, presented with significant left upper eyelid edema, erythema, and near-complete ptosis. He was afebrile and nontoxic. Examination revealed an inflamed lacrimal gland, with no other orbital signs. There was mild leukocytosis and elevated ESR and CRP. Intravenous ampicillin/sulbactam was initiated. Forty-eight hours later, the contralateral eyelid developed similar symptoms. Blood cultures, respiratory viral panel, COVID-19 antigen, and EBV serologies were negative. Orbital imaging revealed moderate bilateral lacrimal gland enlargement without abscess or ot

The clinical and multimodal imaging features of IgG4-related dacryoadenitis with coexisting vitiligo: A case report.

RATIONALE: IgG4-related dacryoadenitis is an ocular manifestation of IgG4-related disease. Reports on its multimodal imaging features, particularly the manifestations on color Doppler ultrasound, are rare. PATIENT CONCERNS: A 76-year-old woman experienced persistent edema of the bilateral upper eyelids for more than 2 years, with a medical history of vitiligo, allergic dermatitis, and hypertension. Physical examination revealed enlarged lacrimal glands with palpable, firm, and movable nodules, accompanied by swollen eyelids and swollen lymph nodes in front of the ears. Color Doppler ultrasoun

IgG4-Related Prostatitis Detected by Whole-Body Diffusion-Weighted Imaging With Background Body Signal Suppression (DWIBS).

IgG4-related disease (IgG4-RD) is a systemic fibro-inflammatory disorder that can affect multiple organs, but involvement of the prostate is rare. Most reported cases of IgG4-related prostatitis are associated with multiorgan disease and present with lower urinary tract symptoms (LUTS). Herein, we present a rare case of IgG4-related prostatitis, which was detected by whole-body diffusion-weighted imaging with background body signal suppression (DWIBS) and was pathologically diagnosed by MRI-fusion target biopsy, accompanied by IgG4-related dacryoadenitis/sialadenitis and multiple lymphadenopat

Transcriptomic changes in the lacrimal glands of a Sjogren's disease animal model highlight key molecular mediators and altered biological functions underpinning glandular inflammation and hypofunction.

INTRODUCTION: Sjogren's disease (SjD) is a chronic autoimmune condition that targets the lacrimal glands (LGs), resulting in aqueous-deficient dry eye disease (DED). Insufficient or low-grade tear secretion onto the ocular surface affects the normal physiology of the cornea and conjunctiva and can result in ocular surface damage. Herein, we evaluated the transcriptomic profiles of diseased versus healthy LGs to investigate the underlying molecular mechanisms underscoring LG autoimmune inflammation and secretory hypofunction. METHODS: LGs were removed from male NOR/LtJ mice (SjD mouse model) a

[Orbital pathology, not always caused by endocrine factors].

Not all cases of Exophthalmos are related to Graves' disease. Normal thyroid function test results should prompt extended differential diagnostics. IgG4-related disease (IgG4-RD) mimics various inflammatory, infectious, or neoplastic conditions. Diagnosis requires integration of clinical, laboratory, imaging, and histopathological findings. Early glucocorticoid therapy can reduce organ involvement and improve patient outcomes.

An Uncommon Mould in an Uncommon Place: A Case of Scopulariopsis brevicaulis Keratitis From the Northern Himalayas, India.

Scopulariopsis brevicaulis is an uncommon cause of fungal keratitis, usually occurring after ocular trauma, and is associated with therapeutic difficulties because of its limited susceptibility to antifungal agents. We report the case of a 48-year-old man who presented with pain, redness, and watering of the left eye for one month following injury by iron foreign body particles during welding. Ocular examination revealed a corneal ulcer with hypopyon and markedly reduced visual acuity (2/60). Initial microscopic evaluation was negative for fungal elements; however, fungal culture after extende

Preseptal Cellulitis as the Presenting Sign of Systemic Inflammatory Disease Secondary to Chikungunya Virus: A Case Report.

INTRODUCTION: Preseptal cellulitis in children, a leading cause of pediatric orbital inflammation, often masquerades as more severe systemic inflammatory conditions. Chikungunya virus is an arbovirus with a wide spectrum of systemic manifestations that can lead to potentially harmful complications within the nervous, cardiovascular, and renal systems if left undetected. A diagnostic challenge resides with its rare ophthalmic presenting feature of preseptal cellulitis. Herein, we described a case of Chikungunya virus (CHIKV) masquerading as a preseptal cellulitis that led to a systemic inflamma

Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

Visual Impairment Following Periocular Triamcinolone Acetonide Injection into the Lacrimal Gland: A Case Report and Literature Review.

BACKGROUND: Idiopathic dacryoadenitis is a diagnosis of exclusion and may present with unilateral or bilateral eyelid swelling, erythema, pain, and characteristic S-shaped ptosis of the upper eyelid. Glucocorticoids remain the mainstay of treatment when symptoms are functionally limiting or clinically significant. Although local triamcinolone acetonide (TA) injection provides targeted anti-inflammatory effects with fewer systemic adverse effects, it carries a rare but potentially catastrophic risk of iatrogenic vascular embolization and acute vision loss. METHODS: We searched the PubMed datab

Single-cell profiling reveals clonal CD8+T cell-derived IFNγ drives Sjögren's disease-related dacryoadenitis.

BACKGROUND: Sjögren's disease (SjD) is a chronic progressive autoimmune disorder primarily affecting the exocrine glands. Dacryoadenitis represents one of the typical clinical manifestations of SjD, yet its underlying pathogenic mechanisms remain poorly understood. This study investigated the immune microenvironment of the lacrimal gland to elucidate the pathogenic cell subset and key molecular factors driving SjD-related dacryoadenitis. METHODS: Aire-/- mice, a well-established SjD-related dacryoadenitis model, were employed in this study. Lacrimal gland infiltrating immune cells were profil
