Charting the course of Sjögren's disease: A historical analysis of scientific progress.

Sjögren's disease (SjD) bears the name of the Swedish ophthalmologist Henrik Sjögren. His 1933 doctoral thesis on keratoconjunctivitis sicca ignited interest, yet much remains under debate. SjD is an autoimmune disorder that primarily affects the exocrine glands, particularly the salivary and lacrimal glands, leading to a dryness of the eyes and mouth. Diagnostic criteria have evolved significantly, with over 10 classification criteria proposed since 1965. From its inception, SjD was often overshadowed by other rheumatic diseases, struggling for recognition as a distinct autoimmune entity. Ear

Autoimmune CD4+T cells cause Meibomian gland dysfunction in an adoptive transfer model.

Sjögren disease (SjD) is an autoimmune disease driven by autoimmune CD4+T cells that leads to an immune-mediated loss of lacrimal glands (LG). Meibomian glands are lipid-producing glands in the eyelids that help prevent tear evaporation. While the role of T cells in LG-mediated destruction is well established, it is unknown whether pathogenic T cells can cause MG dysfunction (MGD). Herein, we investigated whether autoimmune CD4+T cells induce MGD and characterized the pathophysiologic mechanisms using an adoptive transfer model. T cells were isolated from CD25KO (KOCD4) or wild-type (WTCD4) mi

Regeneration and Cancer-The Two Sides of a "Stem Cell" Coin: The 2025 Mildred Weisenfeld Award Lecture.

Stem cells endowed with properties of self-renewal and differentiation have opened new avenues for restoration and regeneration of damaged tissues. The same adult stem cells, when they undergo irreversible mutations, transform into cancer cells and generate a tumor. In this lecture, I present our work on these two aspects of stem cells: one focuses on regeneration of the ocular surface, whereas the other targets cancer stem cells (CSCs) in retinoblastoma (Rb). In regeneration, I present our experience of transplantation of the cultured limbal epithelial cells to restore the ocular surface in s

A Stratification Approach Based on Salivary Gland Ultrasonography for Assessing Secretory Function in Sjögren Disease.

OBJECTIVE: Our aim was to develop an ultrasonographic scoring model for staging hypofunction of salivary glands (SGs) in patients with Sjögren disease (SjD). METHODS: The assessment of SG secretory hypofunction was conducted by measuring whole salivary flows. B-mode ultrasonography was performed bilaterally on the parotid and submandibular glands to quantitatively evaluate the gland score and Outcome Measures in Rheumatology (OMERACT) score. The correlation between these scores and SG secretory function in patients with SjD was analyzed, leading to the development of an ultrasonographic scori

Extracellular superoxide dismutase 3 (SOD3) attenuates non-Sjögren and Sjögren syndrome dry eyes in animal models.

This study evaluated the effects of superoxide dismutase 3 (SOD3), a known anti-oxidant and anti-inflammatory agent, in both non-Sjögren and Sjögren syndrome (SS) dry eyes in murine models. Dry eyes (DE) were induced in 6-week-old C57BL/6 female mice by extraorbital lacrimal gland excision, and SOD3 or PBS was topically administered once daily for seven days. For SS DE model, NOD/LtJ female mice with developed dry eyes were treated with topical SOD3 or PBS once daily for 14 days. Clinical DE parameters such as tear volume and corneal stain scores, conjunctival goblet cell density, and pro-infl

PPARα-Dependent and Independent Mechanisms Are Complexly Involved in the Ameliorative Effect of a Dietary Supplement Containing Lactic Acid Bacteria in a Dry Eye Mouse Model.

PURPOSE: A supplement, OPTIADE® DE, containing specific lactic acid bacteria, Enterococcus faecium WB2000, has been shown to provide significant ameliorative effects in patients. However, the mechanism of the supplement has not been elucidated. In this study, we investigated the mechanism of the supplement on dry eye symptoms using an air stress-induced dry eye mice. METHODS: Stress-induced dry eye in mice was produced by exposing the mice to an air stream. OPTIADE® DE was administered for 5 days during the daily exposure to the air stress, and then the tear volume and the expression of speci

Metastasis of Castration-Resistant Prostate Adenocarcinoma to the Lacrimal Gland: A Case Report.

Background and Clinical Significance: Metastases of prostate adenocarcinoma most commonly involve the skeletal system, while orbital metastases, including those in the lacrimal gland, are extremely rare. Despite significant advances in the sensitivity of available imaging methods, their diagnosis is often delayed due to nonspecific clinical presentation and rarity of occurrence. Although exceedingly uncommon, orbital metastases have also been reported in other solid tumors, including certain gynecologic malignancies. Case presentation: A 49-year-old patient treated at our center for prostate a

The prostatic acid phosphatase in prostate cancer: A novel theranostic target.

Prostate-specific membrane antigen (PSMA)-targeted theranostics have transformed the diagnostic and therapeutic landscape of prostate cancer; however, clinically relevant limitations persist, including heterogeneous or absent PSMA expression in a substantial subset of tumors and dose-limiting off-target uptake in salivary glands, lacrimal glands and kidneys. Prostatic acid phosphatase (ACP3), expressed in more than 95% of prostate cancers, has re-emerged as a promising complementary and potentially alternative theranostic target. ACP3 demonstrates favorable biological characteristics, includin

Unilateral orbital mass as the initial presentation of pediatric antineutrophil cytoplasmic antibody associated vasculitis: eosinophilic granulomatosis with polyangiitis versus granulomatosis with polyangiitis.

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA), formerly known as Churg-Strauss syndrome, is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis characterized by asthma, eosinophilia, and systemic vasculitis. Ocular involvement is uncommon (6-20%) and usually follows systemic disease. In contrast, granulomatosis with polyangiitis (GPA) more frequently (30-60%) involves the orbit and may present as isolated orbital disease. Distinguishing EGPA from GPA can therefore be challenging when orbital inflammation is the initial manifestation, particularly in child

A metal-organic framework-based trilayer biomimetic coating on lacrimal stents for dual-drug therapy to prevent restenosis.

Postoperative reocclusion remains a major challenge in lacrimal duct stent implantation, primarily driven by persistent inflammation and fibrosis. To address this issue, we developed a coating system that integrates drug release with interfacial bioactivity regulation. A polydopamine-hybridized zeolitic imidazolate framework (pZIF-8) was synthesized via a one-pot method and co-loaded with mometasone furoate (Elocon Cream) and verteporfin (anti-fibrotic) to construct a reactive oxygen species (ROS)/pH responsive nanocarrier. A trilayered coating consisting of a polydopamine (PDA) adhesive layer

Protocol for Single-Cell Dissociation, RNA Sequencing, and Isolation of Myoepithelial Cells and Fibroblasts from the Aged Lacrimal and Salivary Glands.

Aging of the lacrimal gland (LG) is associated with reduced tear secretion and chronic inflammation, contributing to dry eye disease. Age-related LG changes include immune cell infiltration, cellular damage, lipid accumulation in acinar cells, extracellular matrix deposition, and ductal dilation. Single-cell RNA sequencing (scRNA-seq) provides a powerful approach to profile cellular heterogeneity and transcriptional changes during aging; however, generating viable single-cell suspensions from aged LG tissue is technically challenging due to increased fibrosis, cellular fragility, and lipid acc

Application of imaging studies for orbital tumors: a practical review and representative cases.

Orbital imaging has undergone a significant transformation over the past few decades, evolving from basic radiographic techniques to sophisticated cross-sectional imaging modalities. Historically, conventional radiography was limited to visualizing only the bony structures of the orbit or radiodense foreign bodies, making it inadequate for detecting soft tissue pathology. Advances in imaging over the last several decades-first with computed tomography (CT) in the 1970s and later with magnetic resonance imaging (MRI) in the 1980s-have revolutionized orbital diagnostics by enabling detailed visu

Adenoid Cystic Carcinoma of the Lacrimal Gland With Late Mandibular Metastasis Presenting as an Intraoral Mass: A Case Report and Literature Review.

Adenoid cystic carcinoma (ACC) of the lacrimal gland is an uncommon but clinically aggressive malignancy, characterized by perineural invasion, local recurrence, and delayed distant metastasis. Mandibular metastasis is exceptionally rare. We report a 61-year-old man who presented with progressive right orbital swelling, proptosis, and visual loss. Imaging demonstrated a destructive lacrimal gland mass with temporal bone involvement. The patient underwent surgical debulking followed by radiotherapy, with subsequent local recurrence. Two years later, he developed a painless right posterior mandi

Emerging Function of Prolactin-Inducible Protein-Is This Important Tear Protein Found in Alzheimer's Disease?

Alzheimer's disease is characterized by a chronic, long-term neurodegenerative process and an increasing need for easily accessible biomarkers that enable early diagnosis and disease monitoring. For this reason, tears have attracted growing interest as a potential source of such biomarkers, and prolactin-inducible protein is a candidate tear protein of mechanistic interest whose clinical value remains to be established as a biomarker of Alzheimer's disease. The literature indicates that prolactin-inducible protein is physiologically present in the lacrimal apparatus. Proteomic studies in patie

Pediatric Castleman Disease Manifesting as a Lacrimal Gland Tumor.

Castleman disease constitutes a rare spectrum of conditions with rheumatologic, hematologic, and oncologic features. Orbital involvement is extremely rare with only several cases involving the lacrimal gland. This report describes Castleman disease in a 15-year-old patient who presented with progressive left eye bulging and intermittent periorbital pain. Neuroimaging demonstrated a well-defined, enhancing, homogeneous left lacrimal gland mass with decreased diffusivity, smooth bone remodeling, and anteromedial globe displacement, and no other systemic findings. Introducing the blood-tear barrier: Structure, function, and disruption in ocular surface homeostasis.

Tear fluid exhibits a unique molecular composition distinct from blood, suggesting the presence of a selective blood-tear barrier (BTB) that regulates molecular exchange between ocular vasculature and the tear film. Although sporadically mentioned in the literature, the structure, functional roles, and mechanisms of disruption of this barrier have not yet been characterized. Based on current evidence, this review proposes a conceptual model in which the BTB comprises three successive compartments: the tear film, the epithelial layer, and the vascular endothelium, with tight junctions playing a
