Chronic Unilateral Proptosis in Primary Care: A Case of IgG4-Related Orbital Disease.

Immunoglobulin G4-related orbital disease (IgG4-ROD) is an uncommon fibroinflammatory condition that may closely resemble malignant orbital lesions, posing a significant diagnostic challenge. We report the case of a 50-year-old man who presented with a four-year history of progressive right-sided proptosis. Computed tomography and magnetic resonance imaging revealed enhancing extraconal orbital masses involving the lacrimal gland and extraocular muscles, with imaging characteristics including restricted diffusion that strongly suggested orbital lymphoma. Given the indeterminate radiological fi

Case Report: Pembrolizumab-associated seronegative sicca syndrome during continued immunotherapy and after drug withdrawal in metastatic urothelial carcinoma.

Pembrolizumab, an anti-programmed cell death protein 1 immune checkpoint inhibitor, has been associated with immune-related sicca manifestations, although its course during ongoing treatment and after cessation remains incompletely defined. We report a 78-year-old man with metastatic upper tract urothelial carcinoma who developed abrupt, severe xerostomia and xerophthalmia 45 days after pembrolizumab initiation, with no preceding sicca symptoms. Schirmer's testing and salivary gland scintigraphy confirmed bilateral lacrimal and parotid-predominant salivary hypofunction. Autoimmune serology and

Unmasking Orbital Myopathy: When Enlarged Extraocular Muscles Indicate More Than Thyroid Eye Disease: Report of Two Cases.

INTRODUCTION: Orbital myopathy is a condition involving the extraocular muscles, and establishment of a differential diagnosis is crucial to determine its etiology. However, the broad spectrum of the etiologies and the overlap in their clinical and radiological findings often make the diagnosis challenging. We report two cases of orbital myopathy that clinically mimicked thyroid eye disease (TED). CASE PRESENTATION: Both patients underwent unsuccessful medical ± surgical treatments, based on the presumed diagnosis of TED at other institutions. One patient presented with enlargement of multipl

Structural and mechanical insights into the extracellular matrix of the aging human lacrimal gland for tissue engineering applications.

Aqueous-deficient dry eye disease (ADDE) frequently results from structural and functional impairment of the lacrimal gland. Regenerative approaches require biomaterials that replicate the extracellular matrix (ECM) composition and biomechanical properties of native lacrimal gland tissue. However, quantitative data on the ECM composition and large-strain biomechanics of the human lacrimal gland remain limited. In this study, histological and immunofluorescence-based ECM analyses were combined with large-strain mechanical testing and quantitative enzyme-linked immunosorbent assay (ELISA)-based

Receptor tyrosine kinase signaling in mammalian eye development.

Receptor tyrosine kinase (RTK) signaling is a central mechanism by which local tissue interactions are translated into coordinated morphogenesis during eye development. The vertebrate eye provides a uniquely tractable system for studying this logic because it is assembled from neural epithelium, surface ectoderm, neural crest-derived mesenchyme, vascular endothelium, and specialized epithelial and endothelial derivatives across a prolonged developmental timeline. In this review, we synthesize how major RTK families, including FGF-FGFR, EGF-ErbB, PDGF-PDGFR, VEGF-VEGFR, Eph-ephrin, ANGPT-TEK/Ti

Lacrimal gland ultrasonography as an adjunct tool for Sjögren disease (SjD) diagnosis from undifferentiated connective tissue diseases.

OBJECTIVE: Sjögren disease (SjD) is characterized by autoimmune lymphocytic infiltration of lacrimal and salivary glands, leading to dry eye and dry mouth. This study aimed to investigate the features and diagnostic value of lacrimal gland ultrasonography (LGUS) for distinguishing SjD from undifferentiated connective tissue diseases (UCTDs). METHODS: This prospective cohort study enrolled 80 patients, including 46 with SjD and 34 with non-SjD UCTDs. All participants underwent LGUS, scored according to the OMERACT guidelines for greyscale and color Doppler systems, alongside salivary gland ult

Dynamic progression of ectopic lymphoid structure formation in lacrimal glands of a Sjögren's disease murine model.

BACKGROUND: Sjögren's Disease (SjD) is a chronic autoimmune condition characterized by lymphocytic infiltration of lacrimal glands (LG) and salivary glands (SG). In SG, these immune structures have properties of ectopic lymphoid structures (ELS) and appear to play a critical role in disease pathology. While the presence of ELS in patients' SG biopsies is linked to disease severity, their presence and composition in LG has not been well characterized. METHODS: The properties and time course of apparent ELS development in LG from the male non-obese diabetes free (NOR) sub-strain of the Non-Obes

Chrysanthemum indicum L.-derived extracellular vesicles enhance the therapeutic efficacy of cyclosporine a against dry eye disease.

The pathological progression of dry eye disease (DED) involves a vicious cycle of oxidative stress and inflammation, posing a critical therapeutic challenge. Conventional therapies, such as cyclosporine A (CsA), are limited by poor corneal permeability and low ocular bioavailability. Here, we developed a novel, biocompatible nano-eye drop formulation using Chrysanthemum indicum L.-derived extracellular vesicles (CELNs) as a natural nanocarrier to engineer CsA-loaded CELNs (CsA@CELNs) for synergistic DED therapy. By combining CsA-mediated immunoregulation with enhanced corneal permeability and

Gut-ocular surface axis in dry eye disease: phenotype-specific mechanisms, evidence, and microbiome-targeted interventions.

Dry eye disease is a multifactorial, heterogeneous ocular surface disorder characterized primarily by an imbalance in tear film homeostasis. The traditional classification into "aqueous deficiency" and "evaporative" types fails to fully account for the differences in its inflammatory biology, clinical manifestations, and treatment responses. In recent years, the gut microbiota has been implicated in influencing ocular surface homeostasis through immune-inflammatory, metabolic-barrier, and neuroimmune pathways; however, the magnitude of its effects and their biological significance may vary dep

Pancytopenia and splenomegaly as uncommon manifestations in a patient with immunoglobulin G4-related disease.

Haematological manifestations of immunoglobulin (Ig)G4-related disease (IgG4-RD) are atypical, and few studies have addressed cytopenia in this condition. We present here a case of IgG4-RD with marked pancytopenia and splenomegaly in the absence of bone marrow abnormalities. A 67-year-old woman developed diplopia, dyspnoea, pancytopenia, and hypocomplementemia. Contrast-enhanced computed tomography showed bilateral lacrimal and submandibular gland swelling, enlargement of the cervical, mediastinal, inguinal, and para-aortic lymph nodes, splenomegaly, and granular shadows in both lungs. She had

Intermittent fasting inhibits ferroptosis by modulating CD36 and its palmitoylation to alleviate diabetic dry eye.

BACKGROUND: Excessive lipid accumulation in the lacrimal glands under diabetic conditions can lead to lacrimal gland dysfunction and reduced tear secretion, subsequently resulting in ocular surface inflammation, dry eye syndrome, and corneal vulnerability, ultimately compromising the patient’s quality of life. There are limited and easily implemented intervention strategies to reduce excessive lipid accumulation. While intermittent fasting (IF) has emerged as a promising metabolic intervention, its mechanistic underpinnings and therapeutic potential in diabetes-associated lacrimal gland disord

Evaluation and analysis of tear fluid volume and surgical outcomes for lacrimal punctum obstructive diseases using anterior segment OCT.

BACKGROUND: Lacrimal punctum obstructive diseases are a frequent cause of persistent epiphora, leading to discomfort, impaired quality of life, and repeated clinic visits. Accurate and objective assessment of tear dynamics is essential for diagnosis and evaluation of surgical outcomes. OBJECTIVE: To evaluate tear fluid volume using anterior segment optical coherence tomography (AS-OCT) and correlate these objective parameters with surgical outcomes in patients undergoing procedures for punctal obstruction. METHODS: Fifty-two eyes from 34 patients with punctal stenosis or occlusion were evalu

The Ontogeny of Mouse Salivary Gland Macrophages Is Distinct between Sexes.

Sexual dimorphism is found in gene expression and polarization of macrophages in mammals but remains unclear in the ontogeny of tissue-resident macrophages. Remarkable sex differences are present in salivary glands and risks for a related autoimmune disease, Sjögren'sdisease. Macrophages are the most abundant immune cells in healthy mouse salivary glands and essential for the maintenance of immune quiescence and tissue repair after radiation or inflammatory damages. Therefore, we compared the origins of macrophages in salivary glands between male and female mice using conditional Cx3cr1 and Cc

Zanubrutinib monotherapy for IgG4-related head and neck disease.

OBJECTIVES: Immunoglobulin (Ig) G4-related disease (IgG4-RD) is commonly treated with glucocorticoids and B-cell depletion, but cumulative toxicity and relapse underscore the need for alternative approaches. We evaluated the efficacy, safety, and immunological effects of Bruton's tyrosine kinase (BTK) inhibition with zanubrutinib in active IgG4-RD. METHODS: In this phase 2, open-label, proof-of-concept trial, 10 participants with lacrimal and submandibular gland IgG4-RD received zanubrutinib 80 mg twice daily for up to 24 weeks without glucocorticoid induction or background immunosuppression.

Orbital and periorbital amyloidosis: A systematic review of clinical characteristics, radiological features, management, and systemic correlations.

We performed comprehensive systematic review of orbital and periorbital non-familial amyloidosis, including clinical features, radiology, histopathology, management and outcomes. We included English-language articles describing biopsy-proven orbital and periorbital amyloidosis published from inception to February, 2025. Cases without ocular histopathology and familial amyloidosis were excluded. We included 120 studies detailing 168 cases. Most were primary localized amyloidosis. The clinical presentation was variable, resulting in diagnostic uncertainty. Amyloidosis most commonly affects super

Transcriptomic changes in the lacrimal glands of a Sjogren's disease animal model highlight key molecular mediators and altered biological functions underpinning glandular inflammation and hypofunction.

INTRODUCTION: Sjogren's disease (SjD) is a chronic autoimmune condition that targets the lacrimal glands (LGs), resulting in aqueous-deficient dry eye disease (DED). Insufficient or low-grade tear secretion onto the ocular surface affects the normal physiology of the cornea and conjunctiva and can result in ocular surface damage. Herein, we evaluated the transcriptomic profiles of diseased versus healthy LGs to investigate the underlying molecular mechanisms underscoring LG autoimmune inflammation and secretory hypofunction. METHODS: LGs were removed from male NOR/LtJ mice (SjD mouse model) a

Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

Susceptibility Weighted and Perfusion Imaging for Differentiating Recurrence and Radiation Necrosis in Post-Operative Malignant Brain Tumors: A Retrospective Cohort Study.

BACKGROUND: Brain tumors present a challenge in oncology, necessitating comprehensive treatment strategies. Despite treatment, recurrence rates, especially in malignant tumors, remain high. Novel therapies have emerged, but treatment-related effects such as pseudo-progression (PsP) and radiation necrosis (RN) complicate diagnosis. Imaging techniques such as perfusion imaging and susceptibility-weighted imaging (SWI) show promise in distinguishing tumor progression from treatment effects. This retrospective study evaluated the effectiveness of these modalities in detecting brain tumours, their
