A gatekeeper sympathetic control of lacrimal tear secretion and dry eye onset through the NA-Adra1a-Ucp2 pathway.

Tear secretion from the lacrimal gland is essential for maintaining ocular surface homeostasis, and its insufficiency causes aqueous-deficient dry eye. Unlike the well-established parasympathetic neuronal regulation, the role of sympathetic nervous system (SNS) in tear secretion remains controversial. Here, we demonstrate the intact sympathetic innervation in lacrimal gland and its activation under multiple dry eye stresses. Pharmacological, surgical, and genetic blockade of SNS increases tear secretion and alleviates dry eye signs. Mechanistically, SNS-driven noradrenaline (NA) release activa

Chronic Prostate Cancer-Related Symptoms and Toxicities Following Stereotactic Body Radiation Therapy: Implications for Future Radionuclide Therapy.

Background Men treated for prostate cancer can experience a high symptom burden. Stereotactic body radiation therapy (SBRT) is increasingly utilized in the management of localized prostate cancer, while radionuclide therapy (RNT) is rapidly becoming a standard treatment option for metastatic prostate cancer. The Functional Assessment of Cancer Therapy-Radionuclide Therapy (FACT-RNT) is a newly validated questionnaire assessing symptoms and toxicities among patients receiving RNT. This study aimed to characterize long-term, patient-reported symptoms in prostate cancer survivors treated with SBR

Unilateral Exophthalmos as the Initial Presentation of Acute Myeloid Leukemia in a Pediatric Patient: A Report of a Rare Case.

Acute myeloid leukemia (AML) is a malignant hematological condition characterized by the uncontrolled proliferation of immature myeloid cells in the bone marrow, which disrupts the normal production of blood cells. Although it is a relatively rare subtype of pediatric acute leukemia, it is a significant cause of leukemia-related mortality in children. Ocular involvement in acute leukemias, though uncommon, can be a revealing sign of the disease, sometimes presenting as either an initial or secondary manifestation, especially during relapse. We report the case of a five-year-old child presentin

A Case in Which the Endoscopic Denker's Approach Was Useful in the Diagnosis of IgG4-Related Ophthalmic Disease.

IgG4-related disease (IgG4-RD) is a chronic inflammatory condition characterized by elevated serum IgG4 levels, infiltration of IgG4-positive plasma cells, and fibrosis in various organs. We report the case of a 76-year-old man who presented with left-sided proptosis. Computed tomography revealed a mass lesion in the left orbit. An initial biopsy via a transnasal approach under local anesthesia was inconclusive. Although endoscopic sinus surgery was performed under general anesthesia, a definitive diagnosis could not be obtained. The lesion continued to enlarge, and subsequent ophthalmologic e

Dacryoendoscopy as a frontier technology for lacrimal drainage disorders.

Dacryocystorhinostomy (DCR) for adult patients with acquired nasolacrimal duct obstruction can be performed via 2 approaches: an external nasal approach from the skin and an internal nasal approach from the nasal cavity. Both techniques have a history of over 100 years and are the gold standard approaches for the treatment of nasolacrimal duct obstructions. Alternatively, researchers have also attempted lacrimal passage reconstruction using various stents to restore the patency of the lacrimal passage, with nunchaku-type silicone tubes showing good surgical results according to several publish

Differential Regurgitation From the Puncta on Compressing Lacrimal Sacs.

Regurgitation on pressure over the lacrimal sac (ROPLAS) is a common clinical sign that is elicited in nasolacrimal duct obstructions. It is more evident in the presence of lacrimal sac dilatation or non encysted mucoceles. Gabriele Falloppio is credited with the first scientific documentation of a regurgitation on pressure over the lacrimal sac in the 16th century. Regurgitation of the lacrimal sac contents was believed, in the 18th century (Stahl, Platner, and Schobinger), to show differential patterns of exit from the upper or the lower punctum. To the best of the authors' knowledge, no sub

The Effects of Mesenchymal Stem Cell-Derived Exosomes on the Attenuation of Dry Eye Disease in Sjögren Syndrome Animal Model.

BACKGROUND: Sjögren's syndrome (SS) is a chronic autoimmune disease delineated by excessive lymphocyte infiltration to the lacrimal or salivary glands, leading to dry eye and dry mouth. Exosomes secreted from mesenchymal stem cells (MSC) are known to have anti-inflammatory and tissue regeneration abilities. This study endeavored to demonstrate the effect of MSC-derived exosomes on the clinical parameter of dry eyes and associated pathology in SS mouse model. METHODS: Exosomes obtained from bone marrow-derived human MSC (Catholic MASTER Cells) were injected into the subconjunctival sac of 17 w

Peroral endoscopic myotomy followed by antireflux mucosectomy for gastroesophageal reflux in a rare case of Allgrove syndrome.

This report relates to two female children (identical twins) aged 13 years, one of whom presented with dysphagia, vomiting, nasal regurgitation, and weight loss. The other twin, in history, also had similar symptoms. Esophagogastroduodenoscopy, barium swallow, and esophageal manometry suggested type I achalasia in the former; the other had type II achalasia cardia. Both gave a history of hyperpigmentation of knuckles, under eyes and elbows. Serum cortisol levels were low, with elevated ACTH levels in one, while the other had preserved adrenal function. Their mother gave a history that neither

Bilateral Parotid Gland Cysts as an Atypical Indicator of Sjögren's Syndrome: A Case Study and Literature Overview.

INTRODUCTION: Sjögren's syndrome is a systemic autoimmune disorder that gradually impairs exocrine function, primarily affecting lacrimal and salivary glands. CASE REPORT: We describe an unusual presentation involving an elderly female patient diagnosed with late-stage Sjögren's syndrome. Laboratory testing eliminated viral infections including HIV and HCV. Fine-needle aspiration biopsy of parotid swellings revealed inflammatory cystic lesions, excluding malignancy and other common cystic conditions. MRI revealed several fluid-filled nodules in both glands. CONCLUSION: This report supports i

Oligocentric Castleman Disease (OligoCD): A Novel Diagnostic Entity in the Spectrum of Castleman Disease.

Castleman disease (CD) is a rare lymphoproliferative disorder traditionally classified as unicentric (UCD) or multicentric (MCD). However, some cases exhibit features that do not fit either category. These atypical cases warrant recognition as a distinct subtype, proposed here as oligocentric Castleman disease (OligoCD). A 38-year-old woman presented with bilateral lacrimal gland swelling and cervical lymphadenopathy, fatigue, and low-grade fever. Laboratory investigations, including hemogram, liver and kidney function tests, inflammatory markers, and interleukin-6 (IL-6), were within normal l

Radiographic Changes of the Nasolacrimal Duct in Silent Sinus Syndrome.

PURPOSE: Silent sinus syndrome (SSS), known as chronic maxillary atelectasis, is characterized by inward collapse of the maxillary sinus walls with resultant bowing of the maxillary sinus bones due to negative antral pressures. Functional endoscopic sinus surgery (ESS) is an important first step to normalize pressures. The impact of SSS on nasolacrimal duct (NLD) anatomy remains unclear. The study hypothesized that SSS may distort NLD anatomy, since the duct sits within the maxillary bone, and aimed to compare the radiographic changes on the affected and unaffected sides, as well as pre- and p

Comparative Outcomes of Primary Probing Versus Lacrimal Sac Massage in Older Infants With Congenital Nasolacrimal Duct Obstruction.

Introduction Congenital nasolacrimal duct obstruction (CNLDO) is one of the most common causes of persistent epiphora and discharge in infants. While lacrimal sac massage is frequently advised as an initial therapy, the role of early probing in infants older than six months remains a subject of discussion. This study aimed to compare the outcomes of primary probing and lacrimal sac massage in infants aged 6-12 months with CNLDO.  Methods A prospective interventional study (non-randomized controlled trial) was conducted over a one-year period involving infants aged 6 to 12 months diagnosed with
