Bilateral simultaneous external dacryocystorhinostomy: outcomes and patient satisfaction in a single-stage approach.

OBJECTIVE: To evaluate the viability of simultaneous bilateral external dacryocystorhinostomy (ex-DCR) through a multidimensional analysis of surgical feasibility, patient-reported outcomes, complication incidence, and cost-efficacy. METHODS: The study included 54 eyes of 27 patients who underwent bilateral simultaneous ex-DCR for bilateral symptomatic epiphora due to NLDO. Patient symptoms were systematically assessed using the validated Nasolacrimal Duct Obstruction Symptom Score (NLDO-SS) questionnaire. Additionally, all patients completed a Thurstone scale-based questionnaire to evaluate

A Rare Metastatic Squamous Cell Carcinoma of the Lacrimal Sac Originating from Nasopharyngeal Carcinoma: A Case Report.

Background and Clinical Significance: Metastatic carcinoma of the lacrimal sac originating from primary nasopharyngeal carcinoma (NPC) is a rare entity, usually presenting with chronic, unilateral epiphora. Case Presentation: A 55-year-old male patient presented with symptoms of chronic persistent dacryocystitis of the left eye for a year. His history revealed a non-keratinizing NPC diagnosed 5 years earlier, which was treated with combined radiotherapy (RT) and chemotherapy (CMT). Following CT and MRI scans, a mass was identified at the left lacrimal sac suggestive of a neoplasm in that regio

Clinical Presentation and Surgical Outcomes of Congenital Divided Nevus of the Eyelids in Three Filipino Patients: A Case Series.

Congenital divided nevus of the eyelids is a rare form of melanocytic nevus which involves contiguous portions of the upper and lower eyelid margins unilaterally, hence the term 'kissing nevus'. While usually present at birth, these nevi may also appear later in life. When the mass enlarges, it may cause cosmetic issues to the patient, as well as functional problems such as mechanical ptosis, ectropion, and epiphora. We report three cases of congenital divided nevus of the eyelids, all presenting with unilateral upper and lower hyperpigmented lid masses since birth. The first case had an upper

Congenital nasolacrimal duct obstruction and amblyogenic risk factors: a systematic review of the evidence and controversies.

PURPOSE: Congenital nasolacrimal duct obstruction (CNLDO) represents the predominant etiology of infantile epiphora. It frequently resolves spontaneously; but persists in approximately 5-6% of cases. The possible correlation between CNLDO and risk factors for amblyopia, including anisometropia, refractive errors, and strabismus, remains a subject of debate, which consequently affects the timing and selection of therapeutic interventions. METHODS: This systematic review adhered to the PRISMA2020 guidelines. Comprehensive search of PubMed, Scopus, Web of Science, and Google Scholar (1998-2024)

Primary Malignant Melanoma of the Nasolacrimal Duct Presenting Without Hemolacria: A Case Report and Literature Review.

Primary malignant melanoma of the nasolacrimal duct is a rare and aggressive mucosal neoplasm. Its diagnosis is frequently delayed because early symptoms often mimic benign inflammatory conditions like chronic dacryocystitis. While hemolacria is considered a hallmark sign, it is not universally present. We present a case of a 78-year-old woman with a 1-year history of persistent epiphora and discharge from the left eye, without hemolacria. Initial lacrimal irrigation suggested obstruction. Computed tomography (CT) revealed a soft tissue lesion with widening of the lacrimal bony canal but witho

Endoscopic dacryocystorhinostomy for primary acquired nasolacrimal duct obstruction: A systematic review and pooled analysis of powered vs. non-powered technique.

PURPOSE: Endoscopic dacryocystorhinostomy (eDCR) has become the standard surgical treatment for primary acquired nasolacrimal duct obstruction (PANDO), demonstrating safety and success rates comparable to external DCR. However, surgical failure remains a concern, often related to aberrant mucosal healing, synechiae formation, and ostium restenosis. The optimal surgical approach, particularly regarding powered versus non-powered techniques and the role of adjunctive treatments such as silicone stents and mucosal flaps, remains debated. METHODS: A systematic review and pooled analysis were cond

Radioactive Iodine Symptom Burden, Psychosocial Factors, and Health-related Quality of Life among Differentiated Thyroid Cancer Survivors: A Cross-Sectional Analysis.

BACKGROUND: Few studies have examined whether specific symptoms following radioactive iodine (RAI) treatment are associated with worse health-related quality of life (HRQOL) among patients with differentiated thyroid cancer (DTC), a population that is understudied in cancer survivorship research. More specifically, the relationship between RAI symptom burden and holistic HRQOL measures has not yet been evaluated in this population. This study aimed to 1) evaluate the correlation between a comprehensive patient-reported RAI symptom measure, the SAlivary, LAcrimal, and NaSal measure (SALANS), an

Plug or Punctoplasty (POPPY) Study: Perforated Punctal Plug Versus Three-Snip Punctoplasty in the Treatment of Acquired Punctal Stenosis.

PURPOSE: Acquired punctal stenosis (APS) represents a common cause of epiphora and a reason for attendance at oculoplastic clinics. The three-snip punctoplasty is the mainstay of APS treatment, although perforated punctal plugs (PPPs) represent an alternative, offering reversibility and less burden on minor-operation lists. This study aimed to identify if PPPs are an acceptable alternative to the three-snip punctoplasty through comparison of functional, anatomical, and quality of life outcomes. METHODS: Participants with APS were randomized to receive either a three-snip punctoplasty or PPP i

A case of a paranasal sinus mucocele of the frontal sinus invading the anterior cranial fossa and orbital wall.

This report details a rare case of a frontal-ethmoid sinus mucocele with concurrent invasion of both anterior cranial fossa and orbital wall, manifesting primarily with ophthalmologic symptoms. A 64-year-old woman presented with right periorbital swelling accompanied with mild pain, photophobia, and epiphora. Endoscopic sinus surgery revealed extensive destruction of the skull base bone. Intraoperative findings included an intact dura mater without cerebrospinal fluid leakage. Histopathology identified a benign mucocele lined by respiratory epithelium. This case represents an exceptional prese

B-cell lymphoblastic lymphoma masquerading as primary dacryocystitis: A case report.

Primary B-cell lymphoblastic lymphoma (B-LBL) of the lacrimal drainage system is exceptionally rare and diagnostically challenging. A 45-year-old man presented with left eyelid swelling, epiphora, medial canthal erythema, and a palpable mass in the lacrimal sac area. Imaging revealed an invasive mass with bone erosion and focal hypermetabolism (SUVmax 5.0). Although the initial clinical impression favored a chronic inflammatory process, the aggressive radiographic features raised suspicion of malignancy, prompting complete surgical resection of the involved lacrimal drainage system. The diagno

Spaceflight-Associated Epiphora Mechanisms and Countermeasures.

INTRODUCTION: Epiphora, or excessive tearing, is a commonly reported symptom among those with dry eyes in the terrestrial environment. It is therefore not surprising that a higher frequency of epiphora was demonstrated among astronauts onboard both Space Transportation Shuttle and International Space Station missions. In this review paper, we discuss the pathophysiology and mechanisms of epiphora and consider the additional risk factors due to the extreme environments of spaceflight, including microgravity and, to a lesser extent, radiation. We also discuss potential strategies to mitigate spa

Proteome Analysis of Nasolacrimal Duct Lavage Fluid in Patients with Primary Acquired Nasolacrimal Duct Obstruction.

Primary acquired nasolacrimal duct obstruction (PANDO) is a common cause of epiphora in adults, yet the biochemical environment within the nasolacrimal duct (NLD) remains poorly understood. This study aimed to characterize the proteomic composition of NLD lavage fluid and identify subtype-specific molecular features distinguishing membranous and mucinous obstruction. Paired tear and NLD lavage fluid (NLD-LF) samples were collected from patients undergoing dacryoendoscopic recanalization, and proteomic profiling was performed using LC-MS/MS. A total of 1345 proteins were identified in NLD-LF an

Intraoperative biopsy during dacryocystorhinostomy in a patient with lacrimal sac MALT lymphoma mimicking dacryocystitis: A case report.

Lacrimal sac primary tumors are uncommon and often mimic inflammatory conditions, rendering preoperative diagnosis difficult. The present study describes the case of an 83-year-old female patient presenting with left-sided epiphora and mucopurulent discharge. Based on the clinical findings, dacryocystitis was strongly suspected. Computed tomography dacryocystography revealed only a mild filling defect, with no radiological evidence suggestive of a neoplastic lesion. However, during endoscopic dacryocystorhinostomy, a well-defined polypoid lesion was observed within the lacrimal sac. A lesion b
