Complete Supernumerary Nostril with Accessory Alar Fistula Reconstructed via Alar Wedge Excision Type 2: A Case Report and Literature Review.

BACKGROUND: Supernumerary nostril with an accessory nasal fistula is an exceptionally rare congenital anomaly with no standardised reconstructive protocol, especially with nasolacrimal involvement. CASE REPORT: A nine-year-old boy presented with a complete supernumerary nostril and synchronous accessory nasal fistula at the right alar base, persistent epiphora and prior endoscopic dacryocystorhinostomy. Imaging demonstrated a cystic tract communicating with the nasolacrimal system. Definitive management utilised a modified alar wedge excision type 2, incorporating both orifices into a single

RBM39 upregulation promotes ostium re-obstruction after endoscopic endonasal dacryocystorhinostomy (En-DCR).

PURPOSE: Chronic dacryocystitis, characterized by epiphora, is primarily treated with dacryocystorhinostomy (DCR), yet postoperative anastomotic re-obstruction remains a significant cause of failure. This study aimed to identify the key molecular mechanisms driving this recurrence. METHODS: Nasal mucosal tissues were collected from 30 post-DCR patients: 15 with re-obstruction (proliferation group) and 15 with successful outcomes (non-proliferation group). Proteomic analysis based on LC-MS/MS was conducted, with differentially expressed proteins (DEPs) defined by a fold change ≥ 1.5 and p < 0.

Clinical Predictors of Surgical Management in Patients with Chronic Epiphora: A CT-DCG-Supported Analysis.

Background: Chronic epiphora is a common ophthalmic complaint with multifactorial etiology, including both structural obstruction of the nasolacrimal drainage system (NLDS) and functional or ocular surface-related causes. Accurate differentiation between these conditions is essential for appropriate management, yet conventional diagnostic methods provide limited anatomical detail. The complementary role of advanced imaging in routine clinical decision-making remains incompletely defined. Methods: In this retrospective observational study, 127 patients with chronic epiphora who underwent CT-DCG

Diffuse Large B-Cell Lymphoma of the Ethmoid Sinus With Orbital Invasion: A Case Report.

Diffuse large B-cell lymphoma (DLBCL) rarely involves the paranasal sinuses, and ethmoidal localization with orbital extension is exceptional. We report the case of a 76-year-old patient presenting with progressive nasal obstruction and epiphora. Imaging revealed an ethmoidal mass with orbital invasion. Histopathological examination confirmed the diagnosis of DLBCL. The patient was referred to the medical oncology department for systemic immunochemotherapy followed by consolidative radiotherapy. No surgical intervention was performed. This case emphasizes the need to consider lymphoma in the d

Translacrimal Antrostomy: An Augmented Endoscopic Approach to the Maxillary Sinus.

Conventional middle meatal antrostomy has limitations in surgical accessibility and may be associated with postoperative stenosis. Alternative approaches that improve access to the maxillary sinus while maintaining long-term patency are therefore needed. This study aimed to evaluate the safety and efficacy of a novel technique, translacrimal antrostomy (TLA). Unlike standard middle meatal antrostomy (MMA), TLA involves the removal of the inferior lacrimal bone to facilitate access to the anterior maxillary recesses. A retrospective review was conducted of 64 patients (81 sinuses) who underwent

Excessive Tearing and Lacrimal and Canalicular Blockage Secondary to Docetaxel in Breast Cancer Patients in the Metastatic and Adjuvant Settings.

While most ophthalmologists and medical oncologists are aware of the excessive tearing and canalicular and lacrimal duct blockage associated with docetaxel, there is lack of deeper understanding of the appropriate management of this side effect and what clinical settings justify surgical intervention as opposed to conservative management. In this review, we summarize the findings in the seminal original research studies that document the association between the frequency of administration of docetaxel and treatment duration and the frequency of excessive tearing as a subjective symptom versus

Tumors of the Lacrimal Drainage System: Diagnosis and Management.

Tumors of the lacrimal drainage system are rare and include benign and malignant variants. Their presentation frequently mimics inflammatory or obstructive lacrimal disease, which often results in delayed diagnosis and diagnostic uncertainty. A high index of suspicion, appropriate imaging, and timely biopsy are required to verify the histology and guide management. Surgical excision remains the cornerstone of treatment for most lesions, with the aim of achieving histologically clear margins. Advances in endoscopic and combined surgical approaches have enabled radical tumor resection whilst red
