Bayesian outbreak detection algorithm for monitoring reported cases of campylobacteriosis in Germany.

In infectious disease epidemiology, statistical methods are an indispensable component for the automated detection of outbreaks in routinely collected surveillance data. So far, methodology in this area has been largely of frequentist nature and has increasingly been taking inspiration from statistical process control. The present work is concerned with strengthening Bayesian thinking in this field. We extend the widely used approach of Farrington et al. and Heisterkamp et al. to a modern Bayesian framework within a time series decomposition context. This approach facilitates a direct calculat

Bell's palsy presenting after a case of Campylobacter enteritis.

BACKGROUND: A case of unilateral facial paralysis following infection with Campylobacter jejuni is presented. CASE REPORT: A 37-yr-old U.S. Army Reserve aviator presented with complaints consistent with an infectious gastroenteritis. This presentation was quickly followed by acute right-sided facial hemiparesis with sparing of sensation. He had no other neurological symptoms. Complete neurological evaluation showed no evidence of underlying neurological pathology and the patient was diagnosed with Bell's palsy. He was later notified by the state Department of Health that he and his family had

Virulence characteristics of five new Campylobacter jejuni chicken isolates.

Campylobacter enteritis has emerged as one of the most common forms of human diarrheal illness. In this study we have investigated the virulence potential of five new C. jejuni chicken isolates (RO14, RO19, RO24, RO29 and RO37) originated from private households in the rural regions of Banat and Transylvania in Romania. Following isolation and in vitro virulence assay, on HCT-8 cells, our results show that all the C. jejuni chicken isolates overcome the virulence abilities of the highly virulent strain C. jejuni 81-176. Motility, an important virulence factor was significantly improved in all

Campylobacteriosis in urban versus rural areas: a case-case study integrated with molecular typing to validate risk factors and to attribute sources of infection.

Campylobacter infection is a leading cause of bacterial gastroenteritis worldwide, and most clinical cases appear as isolated, sporadic infections for which the source is rarely apparent. From July 2005 to December 2007 we conducted a prospective case-case study of sporadic, domestically-acquired Campylobacter enteritis in rural versus urban areas and a prevalence study of Campylobacter in animal and environmental sources in the Eastern Townships, Quebec. Isolates were typed using Multilocus Sequence Typing (MLST) to reinforce the case-case findings and to assign a source probability estimate

Spectrum of imported infectious diseases among children and adolescents returning from the tropics and subtropics.

BACKGROUND: About 50 million people travel each year from industrialized countries to destinations in the tropics and subtropics. Among them, there are more than 2 million minors traveling. Although their number is increasing constantly, data on health risks during travel are limited. METHODS: This study analyzed demographic, travel, and clinical data of 890 travelers of age <20 years presenting at the outpatient travel clinic of the University of Munich between 1999 and 2009 after returning from the tropics and subtropics. RESULTS: Most (87%) of these young travelers were born in Germany. A

Evaluation of the positive predictive value of a rapid Immunochromatographic test to detect Campylobacter in stools.

The recently developed rapid immunochromatographic tests (ICT) have the potential to provide a quick and easy diagnosis of Campylobacter enteritis in comparison to culture. In a previous study we found them sensitive but lacking in specificity. The aim of the present study was to focus on the problem of specificity and determine the positive predictive value (PPV) of a positive result of the ImmunoCard Stat! Campy (Meridian Bioscience, Cincinnati, OH, USA). For this purpose, the stools positive by ICT were cultured according to 3 different protocols: Karmali agar, Preston enrichment broth subc

Effect of reminders on mitigating participation bias in a case-control study.

BACKGROUND: Researchers commonly employ strategies to increase participation in health studies. These include use of incentives and intensive reminders. There is, however, little evidence regarding the quantitative effect that such strategies have on study results. We present an analysis of data from a case-control study of Campylobacter enteritis in England to assess the usefulness of a two-reminder strategy for control recruitment. METHODS: We compared sociodemographic characteristics of participants and non-participants, and calculated odds ratio estimates for a wide range of risk factors

A tolerogenic mucosal immune response leads to persistent Campylobacter jejuni colonization in the chicken gut.

Campylobacter enteritis is the most reported zoonotic disease in many developed countries where it imposes a serious health burden. Campylobacter transmission to humans occurs primarily through the chicken vector. Chicks are regarded as a natural host for Campylobacter species and are colonized with C. jejuni in particular. But despite carrying a very high bacterial load in their gastrointestinal tract, these birds, in contrast to humans, do not develop pathological signs. It seems that in chickens C. jejuni principally harbors in the cecal mucosal crypts, where an inefficient inflammatory res

Poultry as a host for the zoonotic pathogen Campylobacter jejuni.

Campylobacteriosis is the most reported foodborne gastroenteritic disease and poses a serious health burden in industrialized countries. Disease in humans is mainly caused by the zoonotic pathogen Campylobacter jejuni. Due to its wide-spread occurrence in the environment, the epidemiology of Campylobacter remains poorly understood. It is generally accepted, however, that chickens are a natural host for Campylobacter jejuni, and for Campylobacter spp. in general, and that colonized broiler chicks are the primary vector for transmitting this pathogen to humans. Several potential sources and vect

Climate changes, environment and infection: facts, scenarios and growing awareness from the public health community within Europe.

Climate change is a current global concern and, despite continuing controversy about the extent and importance of causes and of its effects, it seems likely that it will affect the incidence and prevalence of both residual and imported infections in Europe. Climate affects mainly the range of infectious diseases, whereas weather affects the timing and intensity of outbreaks. Climate change scenarios include a change distribution of infectious diseases with warming and changes in outbreaks associated with weather extremes. The largest health impact from climate change for Europe doesn't come fr

15th International Workshop on Campylobacter, Helicobacter and Related Organisms.

The purpose of this communication is to update the veterinary public health community as to what poultry-related interventions were presented at the recent biennial International Workshop on Campylobacter, Helicobacter and Related Organisms (CHRO), which was held in Niigata, Japan, September 2-5, 2009. More than 30 years have passed since the publication of Martin Skirrow's seminal paper in the British Medical Journal in which he described Campylobacter enteritis as a new disease (1). This publication precipitated a global interest in thermophilic campylobacters. Three decades later, these org

Comparison of premier CAMPY enzyme immunoassay (EIA), ProSpecT Campylobacter EIA, and ImmunoCard STAT! CAMPY tests with culture for laboratory diagnosis of Campylobacter enteric infections.

Campylobacter enteritis is a food-borne or waterborne illness caused almost exclusively by Campylobacter jejuni and, to a lesser extent, by Campylobacter coli. These organisms produce indistinguishable clinical diseases and together represent the second most common cause of bacterial diarrhea in the United States and the leading cause of enteric infection throughout the world. The conventional approach to the laboratory diagnosis of Campylobacter enteritis is based on the recovery of the organism from a stool specimen, which requires the use of a specialized medium incubated at 42°C for severa

Campylobacter jejuni induces transcytosis of commensal bacteria across the intestinal epithelium through M-like cells.

BACKGROUND: Recent epidemiological analyses have implicated acute Campylobacter enteritis as a factor that may incite or exacerbate inflammatory bowel disease (IBD) in susceptible individuals. We have demonstrated previously that C. jejuni disrupts the intestinal barrier function by rapidly inducing epithelial translocation of non-invasive commensal bacteria via a transcellular lipid raft-mediated mechanism ('transcytosis'). To further characterize this mechanism, the aim of this current study was to elucidate whether C. jejuni utilizes M cells to facilitate transcytosis of commensal intestina

Pulsed field gel electrophoresis typing of human and retail foodstuff Campylobacters: an Irish perspective.

Campylobacter enteritis is a zoonosis, an infectious disease transmissible under normal conditions from vertebrate animals to man, presenting a major global public health burden. In this study, Pulsed Field Gel Electrophoresis (PFGE) was employed to identify common genotypes in a collection of 600 Campylobacter isolates in order to investigate if profiles obtained from retail samples of foodstuffs matched genotypes causing illness in the community in Ireland. The Campylobacters were isolated from retail foodstuffs, and cases of gastroenteritis, over the same 20-month period in three population

Vibrio cholerae O1 bacteremia in Pakistan: analysis of eight cases.

Bacteremia caused by Vibrio cholerae O1 has been a rare phenomenon. We report on eight cases of V. cholerae O1 bacteremia from Pakistan which occurred during 1992-2008. Six of the cases were seen in children (two neonates and four infants) and seven of the eight patients were female. Urogenital malignancy, hepatitis B virus-associated end-stage liver disease, concurrent Campylobacter enteritis and prematurity were the underlying conditions in four patients. Two of the eight patients died and one was lost to follow up and this outcome may be due to prior immunity leading to less severe illness.

Acute inflammatory encephalomyelitis following Campylobacter enteritis associated with high titre antiganglioside GM1 IgG antibodies.

Campylobacter enteritis is commonly associated with various forms of the Guillain-Barré syndrome but not central nervous system (CNS) inflammation. We present a case of Campylobacter enteritis associated with acute inflammatory encephalomyelitis and high titre antiganglioside GM1 IgG antibodies. The finding of antiganglioside antibodies in inflammatory demyelination of the CNS may identify avenues for research into pathogenesis. The relationship between antiganglioside antibodies and CNS inflammation is discussed.
