Two hundred and seventy-five children were admitted to the Nutrition Rehabilitation Centre (NRC), Madurai in 1975; 213 had xeropthalmia and 62 showed no eye signs. All were malnourished, the largest single group being less than half their expected weight for age. A case of Sjögren's disease is described in which isolated cerebellar symptoms were prominent. Serological investigations gave no evidence of disseminated immune vasculitis, systemic lupus erythematosus or any other autoimmune disease. The cerebrospinal fluid protein changes, which were diagnostic of an autoimmune process in the CNS and suggestive of Sjögren's syndrome are discussed.

Sequential salivary scintigraphy, labial salivary gland biopsy, and measurement of stimulated parotid flow rate were performed in 50 patients suspected of having Sjögren's syndrome. The value of these tests in the diagnosis of this disease was compared. Salivary scintigraphy and labial salivary gland biopsy can be used together to diagnose the oral component of Sjögren's syndrome. Abnormal salivary scintigraphy correlated with both of the other tests and may be considered optional in the diagnostic evaluation.

An irreversible, unilateral keratoconjunctivitis sicca developed in a healthy 16-year-old female patient due to traumatic denervation of the lacrimal gland after oto-basal skull fracture with reversible complete peripheral facial paralysis, the corneal sensitivity remaining normal. This observation and other reports on keratoconjunctivitis sicca caused by isolated lacrimal gland insufficiency indicate the physiological importance of the main lacrimal gland as indispensable part of the secretory tear system.

Two cases of bilateral atresia of the nasolacrimal orifice are described. The presenting sign was a persistent ocular discharge, clear in the one case and purulent in the other. By means of the indirect immunofluorescence method smooth-muscle antibodies (SMA) were determined in 182 patients with various diseases and the findings were compared with those of 7 patients with chronic active hepatitis (CAH) and 580 normal controls. Histopathological study of filamentary keratitis was done by using replica technique in five patients. The filaments are produced by sliding of the epithelial cells around small areas of focal degeneration of the superfical epithelium. In comparison to the filaments in other conditions, the filaments in KCS are smaller and thicker and do not show the torsional segment. The filaments contain degenerated epithelial cells and alcian blue-PAS positive mucoid material. Making a corneal replica may result in healing of filamentary keratitis.

A new case of Reynold's syndrome is reported. This patient was a 70 years old woman with scleroderma (grade II) and PBC (grade III - IV), associated with Sjögren's syndrome and cryoglobulinemia. She experienced 10 years earlier Raynaud's syndrome and arthralgia. A review of the Literature reveals 22 other detailed cases whose data are collected in this study. Considerations upon a common pathophysiology for scleroderma, PBC and Sjögren's syndrome are developed. Impairment of the function of suppressor T cells in patients undergoing these diseases is possible.

Sera from 70 patients with Sjogren's syndrome (SS), of whom 40 had primary and 30 secondary SS, were tested for various auto-antibodies of the IgG, IgA and IgM classes. 20% had liver-cell-membrane antibody (LMA), 90% had anti-nuclear antibodies (ANA), 40% salivary-gland antibodies (SGA), 13% anti-mitochondrial antibodies (AMA), 33% smooth-muscle cell antibodies (SMA), 9% skeletal-muscle antibodies (SKA) and only 1% had parietal-cell antibodies (PCA). In addition, 53% had IgM rheumatoid factor and 6 patients with anti-DNA antibodies all had SLE. 64% had increased serum IgG, 24% IgA and 14% IgM.
