T cells in Sjögren's disease: update on their role in salivary gland pathogenesis and treatment.

Sjögren's disease (SjD) is a chronic, systemic autoimmune disease that mainly targets saliva- and tear-producing glands. T-cells are a substantial immune cell subset involved in SjD pathogenesis, including break of tolerance toward self, and constitute a major part of tissue-resident and infiltrating immune cells in salivary glands (SGs) of patients with SjD. CD4+ T-cells provide help to B-cells, thereby contributing to B-cell hyperactivity in this disease. Yet, interactions between T-cells and non-immune cells, including epithelial cells and fibroblasts, and how such interactions contribute t

Hormonal transitions across the lifespan shape susceptibility to Sjogren's disease.

OBJECTIVES: Sjogren's disease (SjD) shows a strong female predominance, but the contribution of age-related hormone changes to this sex bias remains uncertain. We investigated whether natural hormonal transitions across the lifespan align with variation in male and female prevalence of SjD. METHODS: Electronic health records from 101 856 SjD patients and 1.33 million controls were analysed. Sex-specific prevalence was compared with serum testosterone, oestradiol and SHBG levels. Population-level hormone distributions from the National Health and Nutrition Examination Survey (NHANES) were inco

Serological Stratification by Anti-Ro52 and Anti-Ro60 Profiles Reveals Distinct Systemic Phenotypes in Primary Sjögren's Disease.

BACKGROUND: Anti-Ro52 and anti-Ro60 antibodies are key serological markers for primary Sjögren's disease (pSjD), yet their association with specific clinical phenotypes remains unclear. This study aimed to define the predictive value of anti-Ro profiles for systemic involvement in pSjD. METHODS: In this retrospective study of 725 pSjD patients, we categorized patients into four subgroups based on their anti-Ro52/Ro60 status. Multivariable logistic regression, adjusted for confounders via LASSO, assessed independent associations. Relative excess risk due to interaction (RERI), attributable pro

Functional human salivary gland organoids for tissue regeneration in chemically defined culture systems.

Regenerative therapies for salivary gland dysfunction remain unavailable. We establish a fully chemically defined, xeno-free three-dimensional culture system to generate functional human salivary gland organoids. These organoids recapitulate the typical glandular architecture and display multilineage cellular composition, supporting long-term expansion with high transcriptomic fidelity to the primary tissue. The organoids exhibit key salivary functions, including glycoprotein secretion, amylase expression, and calcium flux in response to cholinergic stimulation. Single-cell transcriptomic anal

Identification of molecular diagnostic markers and potential therapeutic drugs for Sjögren's syndrome.

Sjögren's Syndrome (SS) is a long-term autoimmune disorder marked by damage to exocrine glands and irregularities in the immune system. Currently, there is a significant lack of effective diagnostic biomarkers and targeted therapeutic options for this disorder. This research aimed to identify molecular biomarkers that could be used for SS diagnosis and to explore therapeutic candidates that could address current clinical needs. Multiple public datasets (GSE84844, GSE40611, GSE7451, GSE208260) were retrieved from the Gene Expression Omnibus (GEO) database, including samples of salivary glands,

Altered Carnitine Homeostasis Modulates Hyperosmolarity-Induced Mitochondrial Dysfunction and Lipotoxicity in Human Corneal Epithelial Cells.

PURPOSE: Tear hyperosmolarity, a hallmark of dry eye disease (DED), contributes to corneal epithelial injury, inflammation, and mitochondrial dysfunction. This study examined the effects of hyperosmolar stress on mitochondrial carnitine metabolism in human telomerase-immortalized corneal epithelial (hTCEpi) cells and evaluated the osmoprotective potential of L-carnitine supplementation. METHODS: The hTCEpi cells were exposed to 90 mM NaCl to induce 490 mOsM hyperosmolar conditions, with or without 200 mM L-carnitine supplementation. Free fatty acid (FFA) uptake, triglyceride (TG) accumulation

Diagnostic utility of salivary gland ultrasonography in suspected primary Sjögren's disease: a comparison of OMERACT-based ordinal and summative scoring.

BACKGROUND: Salivary gland ultrasonography (SGUS) is a promising, non-invasive tool in the diagnostic workup of suspected primary Sjögren’s disease (SjD). This study aimed to compare the diagnostic accuracy of two SGUS scoring methods - an ordinal score (0–6) and a sum score (0–12) - derived from the OMERACT 0–3 grading system, using labial salivary gland biopsy as the reference standard. METHODS: Sixty consecutive patients with suspected primary SjD underwent SGUS of the four major salivary glands. Each gland was graded using the OMERACT 0–3 system. Two composite scores per patient were calcu

H3K18 Lactylation in Salivary Gland Epithelial Cells Regulates Autophagy and Is Associated With the Progression of Sjögren Syndrome.

Abnormal signal transduction within glandular epithelial cells is a pathological feature of advanced Sjögren syndrome (SS). Preliminary investigations have demonstrated that the progression of SS is marked by a substantial accumulation of metabolic byproducts, notably lactate, in the salivary glands. This study corroborated the accumulation of lactate within the salivary glands by analyzing peripheral blood and lip gland tissue samples from patients with SS in addition to constructing an experimental Sjögren syndrome mouse model. Lactate exacerbates the severity of xerostomia and the extent of

Lacrimal gland spheroids in tissue specific hydrogel for dry eye modelling.

A profound understanding of the physiology of the lacrimal gland is essential for developing regenerative therapies for aqueous deficient dry eye syndrome (ADDE). This study presents a reproducible, high-throughput method to generate functional spheroids of the lacrimal gland as building blocks for a three-dimensional (3D) in vitro model. Co-culturing primary lacrimal gland epithelial cells (EpC), mesenchymal stromal cells (MSC), and human umbilical vein endothelial cells (HUVEC) in non-adhesive agarose microwells resulted in rapid spheroid formation within 24 h. These spheroids developed a de

Prevalence and predictors of dry eye syndrome among the young population in Jordan: A cross-sectional survey study.

Dry eye syndrome (DES) is one of the leading causes of eye care visits. Research has shown that the prevalence of DES is higher in Asian populations than in Europeans and Americans. It poses a significant global burden, and with the increasing use of digital screens, particularly among the younger, the global burden of DES is anticipated to increase. This study aimed to investigate the prevalence and predictors of DES among the young population in Jordan. This was an online cross-sectional survey study that utilized Qualtrics survey software and was conducted in Jordan between November 2021 an

The burden and profile of neurological symptoms in patients with Sjögren disease: results from an international survey.

OBJECTIVES: Neurological involvement in Sjögren's disease (neuro-SjD), reported by up to 20% of patients, represents a highly relevant extra-glandular manifestation. Owing to the clinical complexity, frequent diagnostic delay and the poor response to treatment, neuro-SjD remains a major unmet need. This study aimed to better understand the burden of neuro-SjD from the patient perspective, focusing on the overall care journey. METHODS: An anonymous multilingual survey was co-developed by patient research partners and rheumatologists and distributed via patient associations. RESULTS: A total o

A transient receptor potential vanilloid 1-dependent corneal-trigeminal neuroinflammatory circuit promotes corneal neuropathy.

Corneal neurosensory abnormalities cause pain and discomfort in ocular surface disease, yet their pathophysiology is poorly understood. Here we show that in a mouse dry eye model, the ocular (over)activation of transient receptor potential vanilloid 1 (TRPV1) channels in response to tear deficiency and tissue damage promotes neuroinflammatory gene expression and macrophage reactivity in the trigeminal ganglion, where the cornea-innervating sensory neurons are located. This is accompanied by ocular surface macrophage activation, impaired corneal sensitivity to mechanical and non-TRPV1-mediated
