Pneumococcal meningitis secondary to chronic orbital inflammation with skull base erosion: a case report.

This case report describes a 32-year-old female patient who developed acute pneumococcal meningitis following six-year chronic orbital inflammatory changes after strabismus corrective surgery. The patient experienced gradual deterioration of right-eye vision and recurrent retro-orbital headache; serial magnetic resonance imaging (MRI) from 2021 to 2023 demonstrated diffuse intraconal retrobulbar abnormalities and extraocular-muscle thickening. She subsequently presented with high fever, generalized seizures and impaired consciousness. Cerebrospinal fluid (CSF) metagenomic next-generation seque

Infected Frontal Sinus Mucocele Mimicking a Cutaneous Abscess.

Frontal sinus mucoceles are benign expansile lesions that may progressively erode the surrounding bone and extend toward the orbit or intracranial compartment. When expansion occurs anteriorly, however, they may present as an apparently superficial forehead swelling and be mistaken for a cutaneous abscess. A 77-year-old woman with chronic sinusitis developed recurrent left paramedian frontal swelling over several months, with only transient improvement following antibiotic treatment. The lesion was initially interpreted as a cutaneous abscess and incised in an outpatient setting. Clear, nonpur

Orbital cellulitis with posterior scleritis in a 13-year-old girl: A case report.

RATIONALE: Clinical signs and symptoms of posterior scleritis vary and are easily confused with other diseases. Inflammation spreads anteriorly, involving the upper lid structures, causing lid swelling and simulating cellulitis. However, we report a case of orbital cellulitis with posterior scleritis and complications, with the aim of helping clinicians identify and treat early to prevent irreversible loss of vision. PATIENT CONCERNS: A 13-year-old girl presented with pain in her right eye and an acute deterioration in vision. DIAGNOSES: The case was diagnosed as orbital cellulitis with post

Hypersensitivity Reaction to Hyaluronidase After Peribulbar Anesthesia for Cataract Surgery: Two Case Reports.

BACKGROUND: Hypersensitivity reaction to hyaluronidase, though rare, can occur following regional anesthesia in ophthalmic surgery and may mimic serious conditions such as orbital cellulitis. Prompt recognition is critical to avoid misdiagnosis and unnecessary interventions. CASE PRESENTATION: We report two cases of hypersensitivity reaction following hyaluronidase-augmented peribulbar anesthesia for cataract surgery. Case 1: A 90-year-old female with no prior allergy history developed progressive periorbital edema extending to the ipsilateral face and neck within 7 hours after surgery. Infec

A series of ruptured dermoid cysts in pediatric patients.

Ruptured dermoid cysts can often have atypical presentations and be challenging to manage. We report 3 cases of pediatric patients with ruptured dermoid cysts. Case 1 was a 16-year-old boy with an enlarging left superotemporal orbital mass who presented with 2 days of acute inflammation. Computed tomography revealed a well-circumscribed lesion that was excised. Histopathology confirmed a ruptured dermoid cyst. Case 2 was a 4-year-old girl with a right superotemporal orbital mass that enlarged following trauma. Magnetic resonance imaging revealed a bilobed dermoid, and excision confirmed prior

Occult invasive group A streptococcal infection with rapid progression to pediatric intraorbital abscess: a case report.

BACKGROUND: Invasive Group A Streptococcal (iGAS) infection is relatively rare in pediatric upper respiratory tract infections, but it can breach anatomical barriers to cause deep abscesses. However, iGAS-induced rapid development of intraorbital abscess in children is extremely uncommon. Due to its high occult nature, it is easily overlooked, which may lead to permanent visual loss or even death. CASE DESCRIPTION: A 6-year-old girl was admitted to the hospital with “rhinorrhea and cough for 1 week, accompanied by fever for 2 days”. Upon admission, the pediatrician diagnosed acute tonsillitis

Reengineering Aligned D-Orbital Energy Levels in FeMn Dual-Atom Nanozyme Inhibits Pyroptosis for Effective Alleviation of Inflammatory Diseases.

Pyroptosis inhibition via Fe single-atom nanozymes is promising for inflammation therapy, but the common Fe-N4 configuration restricts oxygen intermediate desorption and lacks cooperative sites, thus limiting catalytic performance. To overcome this, we develop a FeMn dual-atom nanozyme supported on oxygen-nitrogen-doped bamboo-like carbon nanotubes (FeMnDA/BCNT). Through the precise alignment of Fe and Mn 3dz2 orbital energy levels by the electron-delocalized BCNT support in the FeMn-N/O active center, thereby lowering the dissociation energy barrier for *O2 or *H2O molecules, promoting O─O bo

Chemotherapy-associated Optic Neuropathy in a Patient with Chronic Myeloid Leukemia and Hepatitis B: A Diagnostic Challenge.

Optic neuritis is a sight-threatening condition marked by inflammation or dysfunction of the optic nerve, typically presenting with sudden, often painful vision loss in one eye. While idiopathic and demyelinating forms are most common, optic nerve damage related to systemic chemotherapy remains an underreported but clinically significant entity. Patients undergoing intensive cancer treatment - particularly those with compromised immunity - are vulnerable to optic neuropathy from direct neurotoxic effects, opportunistic infections, or malignancy-related infiltration. We describe the case of a 3

Tissue resident memory T cells populate the human uveal tract.

The current concept is that the eye is an immune privileged site endowed with innate immune regulatory networks to maintain organ function. We now have evidence that resident T cells occupy intraocular tissues. In immune-mediated inflammatory diseases, such as psoriasis and rheumatoid arthritis, tissue resident T cells trigger disease flares in the skin and joints. This suggests resident T cells in the uvea may have similar functions in non-infectious immune-mediated uveitis, a collective term for autoinflammatory and autoimmune diseases of the uveal tract causing intraocular inflammation. Ethmoid Sinus Mucocele Manifesting as Acute Ophthalmoplegia and Ptosis Rapidly Exacerbated by COVID-19 Infection: A Case Report.

A sinus mucocele is a dilated, epithelial-lined cystic lesion containing encapsulated mucus. Although its etiology remains uncertain, secondary infection may transform it into a pyocele. A 68-year-old female presented with sudden-onset ophthalmoplegia, ptosis, and blurred vision during the acute phase of COVID-19 infection. Physical examination and imaging identified an ethmoid sinus mucocele causing significant orbital compression. The patient underwent urgent endoscopic marsupialization. Intraoperative findings revealed increased internal pressure within the mucocele. Postoperatively, her oc

Early surgical intervention for orbital complications of pediatric acute rhinosinusitis: outcomes and algorithmic approach.

BACKGROUND: Orbital complications of acute rhinosinusitis (ARS) in children require prompt intervention, but consensus on optimal surgical timing and approach remains limited. This study defines evidence-based indications and evaluates outcomes of surgical management while proposing a severity-stratified algorithm to guide intervention. METHODS: We conducted a retrospective cohort study of 48 patients aged <15 years who underwent surgery for ARS-related orbital complications (47 subperiosteal abscesses [SPA], 1 orbital abscess) at a tertiary center. Surgical criteria included SPA >4 mm, visual

Amniotic membrane in ophthalmology: Historical perspectives, biological properties, and clinical applications.

The use of amniotic membrane (AM) in ophthalmology has evolved significantly since its first documented application in ocular surface reconstruction in the 1940s. Widespread clinical interest surged in the late 20th century following the development of improved processing and preservation methods. This comprehensive review covers the history, properties, and clinical applications of AM in ophthalmology. Derived from the innermost layer of the placenta, AM is composed of epithelium, a thick basement membrane and avascular stromal matrix, providing a biologic scaffold that supports epithelial ce

Case Report: A rare case of ocular giant cell arteritis with bilateral orbital inflammation and diagnostic challenges.

BACKGROUND: Giant cell arteritis (GCA) is a systemic vasculitis of medium- and large-sized arteries, most commonly presenting with headache, scalp tenderness, and, if untreated, can lead to visual loss from ischemic optic neuropathy. Although the most common ophthalmic manifestations of Giant Cell Arteritis are related to ocular ischemia, orbital involvement can occur and remains underrecognized. Fewer than 10 cases in the literature describe perioptic or retrobulbar infiltration, underscoring the uniqueness and diagnostic challenge of such presentations. CASE DESCRIPTION: 75-year-old male wi

Orbital Myeloid Sarcoma Masquerading as Orbital Cellulitis: A Diagnostic Challenge in an Adult Patient.

Myeloid sarcoma (MS), also known as granulocytic sarcoma or chloroma, is a rare extramedullary manifestation of acute myeloid leukaemia (AML). It represents a tumour-like accumulation of myeloid blasts outside the bone marrow. Orbital involvement is uncommon and may mimic inflammatory or infectious conditions, creating significant diagnostic challenges. We report a 24-year-old female who presented with a two-week history of progressive left orbital pain and swelling, initially managed as presumed orbital cellulitis with oral flucloxacillin and topical chloramphenicol, which subsequently progre

Understanding the spectrum between orbital infarction syndrome and secondary orbital inflammation following endovascular thrombectomy: a case series.

BACKGROUND: Orbital infarction syndrome (OrbIS) is a rare but devastating complication following mechanical thrombectomy for ischemic stroke, characterized by a near-global ischemia of intraocular and orbital tissues. It can mimic a more benign secondary orbital inflammation (SOIN), leading to diagnostic uncertainty. We present the largest case series of seven patients who developed acute periorbital swelling following endovascular thrombectomy (EVT) for anterior circulation large vessel occlusion with clinically diagnosed OrbIS or SOIN. METHODS: A retrospective review of our EVT database was

Rhino-Orbital Mucormycosis Following COVID-19 Viral Vector Vaccination in an Immunocompetent Patient.

Rhino-orbital mucormycosis is a rare, life-threatening opportunistic fungal infection, typically affecting immunocompromised patients. During the COVID-19 pandemic, increased cases were mainly linked to SARS-CoV-2 infection, diabetes, and corticosteroid exposure. We report a severe case in a previously healthy 44-year-old immunocompetent man who developed acute left-sided exophthalmos, ophthalmoplegia, severe visual loss, and systemic deterioration 10 days after AZD1222 COVID-19 vaccination. Clinical and radiologic findings suggested invasive rhino-orbital fungal disease, prompting immediate l
