A case of acute orbital cellulitis in an adolescent girl is reported and compared with ten cases treated in the same hospital in recent years. The clinical features and stages of orbital cellulitis and its differentiation from cavernous sinus thrombophlebitis is discussed. Although these complications are nowadays rare with modern chemotherapy and antibiotics, they may still occasionally result in blindness and death. (Duke-Elder et al., 1974).

The authors report on an orbital complication in a child of 14 years of age with an abscess on the upper eyelid and pansinusitis on the left side. The clinical suspicion of involvement of the orbita could be definitely clarified with computed tomography only. The article describes the regression of this finding.

Diagnosis to differentiate successfully an uncommon epidermoid carcinoma of the lacrimal sac from the commoner acute inflammation of the lacrimal sac requires a good history and thorough physical examination. If epidermoid carcinoma of the lacrimal sac is suspected, additional procedures including a dacryocystogram, orbital tomograms, plain films and CAT scans, and otolaryngologic consultation are necessary. Such a successful diagnosis of an epidermoid carcinoma of the lacrimal sac is reported here. An aggressive surgical approach is described and strongly recommended because review of the lit

Seven cases of subperiosteal abscess (SPA) of the orbit are reported. Anatomic relationships shared by the orbits, paranasal sinuses, and facial venous system explain the rapid development of SPA after periorbital infection. Computed tomography and echography allow distinction between SPA and other stages of orbital inflammation that are often indiscriminately grouped under the rubric of orbital cellulitis. These techniques suggest that elevated orbital pressure is the primary, and potentially reversible, mechanism of visual impairment produced by SPA. Difficulty in accurately determining the

We studied 30 patients with infectious sphenoid sinusitis (15 acute cases and 15 chronic cases) in an effort to characterize the clinical presentation, bacteriology, and associated complications of this frequently misdiagnosed infection. Severe frontal, temporal, or retro-orbital headache that radiated to the occipital regions or pain in the trigeminal (V1 to V3) distribution or both were the most prominent presenting symptoms. In acute cases, purulent exudate was frequently seen in the middle and superior nasal turbinates. Computerized axial tomography or sinus tomography and cannulation of t

A case of subperiosteal abscess associated with acute sinusitis is reported. The case is remarkable in that anaerobic organisms played a major role in the pathologic process. In addition, the unusual superolateral position of the abscess in the absence of any defect in the bony sinus walls suggests the hematogenous spread of the sinus infection. The use of Ct scanning proved to be essential in determining the true nature of the orbital pathology.

A patient is described with an orbital fistula complicating frontal sinusitis and osteomyelitis of the frontal bone. The fistula was excised, but a fortnight later an acute exacerbation occurred. From the discharging pus a Staphylococcus aureus was cultured and from mucosa obtained during surgery a microaerophilic Streptococcus. These findings led to the diagnosis: synergistic bacterial inflammation of the frontal sinus, with osteomyelitis and orbital cellulitis.

A patient was examined who had acute dacryocystitis that caused true orbital cellulitis. The symptoms of the cellulitis receded after treatment with cephalothin sodium and hot compresses. A dacryocystorhinostomy was performed three weeks later. Delay of the surgery until the acute episode had resolved was probably responsible for the good surgical results.

The clinical presentation, investigative findings, classification, and management of 17 acute pseudotumors based on patterns of orbital involvement is presented. Acute pseudotumors developed over days to weeks and were dominated by pain, neuropraxia, and inflammatory clinical features. Five patterns of acute inflammatory pseudotumor were seen. Anterior and diffuse acute pseudotumors were characterized by manifestations of inflammation of the globe and orbit including pain, lid swelling, ptosis, diplopia, uveitis, papillitis, optic neuropathy, and exudative retinal detachment. Anterior or diffu

The high incidence of staphylococcal infection in severe forms of orbital cellulitis is reviewed with 4 case reports of children with complications of acute sinusitis due to staphylococcal infection. Three cases of thrombosis of the sinus cavernosus due to ethmoidal extension were observed, complicated in one case by fatal purulent meningitis. In one case, the sinusitis was a mixed staphylococcal and anerobic infection with orbital cellulitis and subcutaneous gas gangrene. No organism was isolated in the fifth child who had exophtalmos due to retroorbital cellulitis, but a rapid recovery was o

Acute orbital pseudotumors are inflammatory lesions of unknown etiology that may affect part or, less often, all of the tissue within the orbit. A retrospective computed tomographic (CT) study of 16 patients demonstrated that these lesions occur in one of five specific anatomic patterns: anterior, posterior, diffuse, lacrimal, or myositic. The most common location was lacrimal followed by anterior pseudotumors. Posterior, most common location was lacrimal followed by anterior pseudotumors. Posterior, diffuse, and myositic pseudotumors were equally frequent. Localization on the basis of clinica

A 37-year-old man had epistaxis, headache, intermittent swelling of the periorbital region, proptosis, epiphora, and chronic sinusitis. X-ray films showed marked erosion of the midline facial bones, total opacification of both maxillary sinuses, and clouding of the sphenoid and left ethmoid sinuses. He also had a cutaneous-maxillary sinus fistula and a purulent discharge. A histopathologic examination disclosed areas of acute and chronic inflammation with occasional noncaseating granulomas. Biopsy specimens and postoperative sputum cultures grew Mycobacterium tuberculosis, although the patient

A small percentage of patients with acute frontal or ethmoid sinusitis develop orbital, cranial or CNS complications. At selected University of Tennessee affiliated hospitals, from 1974 to 1978, there were 14 such cases which required major surgical intervention in addition to intensive medical therapy. The most common complication in this series was subperiosteal orbital abscess. The most common bacterial isolates were streptococcus and staphylococcus. A discussion of complications associated with frontal and ethmoid sinusitis is included as well as recommendations for medical and surgical ma

Six cases of adnexal cysticercosis, (1 in the lacrimal gland, 3 orbital, and 2 palpebral) have been reported. In the lacrimal gland it presented as a painless, translucent cyst which closely simulated a simple dacryops. In the orbit, two of the cases presented as an acute abscess in the upper and inner quadrant. However, one of them initially presented and existed as a case of simple ptosis for a period of 8 months. The third orbital case appeared as a painless cyst in the upper and outer quadrant. In the lid the larva presented as a subcutaneous nodule in one case and in the other it was lodg

We review the literature on orbital involvement in acute sinusitis in children. Because of the potential severity of the disease which may be life threatening a vigorous treatment is required. Hemophilus Influenzae is a major cause of orbital cellulitis followed by Staphylococcus Aureus and group A streptococcus. Because of the emergence of Ampicillin-Resistant H. Influenzae strains in Belgium, chloramphenicol should be included in the initial therapy in combination with a penicillinase resistant semisynthetic penicillin. Therapy is adjusted as soon as the results of bacterial culture are know
