A case of acute pansinusitis with the complication of orbital cellulitis has been described. The patient initially had pain around the maxillary right first molar. Accurate diagnosis allowed for prompt, vigorous treatment, culminating in surgical intervention. The need for dental and oral surgical practitioners to be thoroughly familiar with all of the manifestations of paranasal sinus disease cannot be overemphasized.

Acute orbital cellulitis is most commonly caused by sinusitis. This complication is based on the anatomy of the sinuses, orbit, and their venous connections. A series of 134 patients is reviewed to demonstrate the etiology and clinical course of orbital cellulitis. Antibiotic therapy alone does not prevent permanent complications. Surgical intervention is found necessary in all age groups and is required in 20% of the study population.

A three and one-half year-old female who presented with orbital cellulitis and leukocoria is described. Enucleation was performed as the eye was microophthalmic, blind, and painful. Histopathologic study revealed uveitis with total retinal detachment. The etiology of the retinal detachment was indeterminable. The differential diagnosis of orbital cellulitis in children is reviewed.

47 patients with orbital complications resulting from acute sinusitis have been treated at the Freiburg ENT Department during the last seven years. A case of retinoblastoma with secondary glaucoma, in which the initial clinical findings were indistinguishable from those of orbital cellulitis, is presented. Acute glaucoma, especially in childhood, may mimick suppurative disease of the orbit, and primary ocular pathology must be considered in evaluating apparent disease processes in other facial structures.

A review of 104 patients with acute orbital cellulitis during the past decade showed that the frequency of hospital admissions for this disease has increased recently. The bone separating the frontal sinus from the anterior cranial fossa and the orbit is often quite thin. This, plus the interrelated venous drainage system of these areas, forms the anatomic basis for serious orbital and intracranial complications that must be recognized early. These include orbital cellulitis and abscess, cavernous sinus thrombosis, subdural and brain abscess and meningitis. Early hospitalization for intravenous antibiotic therapy and, occasionally, emergency surgical drainage are required.
