Idiopathic sclerosing inflammation of the orbit. A distinct clinicopathologic entity.

BACKGROUND: Idiopathic sclerosing inflammation of the orbit is a poorly delineated, fibrosing, immune-mediated entity resulting in significant ocular disability. To characterize this process and propose more specific and effective therapy, clinical and pathologic findings in 16 cases are reviewed. METHODS: The clinical records of 16 patients with biopsy-proven disease were retrospectively reviewed to determine demographic and clinical features, radiologic features, course, management, and outcome. These findings were correlated with pathologic features to describe this unique entity. Immunohi

Inflammatory pseudotumor of the maxillary sinus: CT and MR findings in six cases.

OBJECTIVE: Inflammatory pseudotumor is a chronic inflammatory lesion of unknown origin. The purpose of this study was to analyze the CT and MR findings in six cases of inflammatory pseudotumor of the maxillary sinus in order to determine if there were any imaging findings that distinguish this lesion from the more common type of pseudotumor that occurs in the orbit. MATERIALS AND METHODS: Six patients with pseudotumor of the maxillary sinus were identified retrospectively. The diagnosis was based on biopsy specimens. Their CT scans and MR images were studied by two radiologists to identify sp

[The computed tomographic semeiotics of rhino-sinusal inflammatory pathology].

Recent advances in "functional" endoscopic surgical techniques require the radiologist to understand the anatomy and pathophysiology of the nose and paranasal sinuses as well as their normal and abnormal CT patterns. Endoscopy and CT are complementary procedures, but in the evaluation of the extent of chronic inflammation and in the demonstration of possible associated local and regional complications, the latter method has become the imaging technique of choice, providing a detailed map for the functional sinus surgeon and replacing plain radiography. The authors reviewed a personal series of

Orbital disease in herpes zoster ophthalmicus.

Three cases of orbital inflammatory disease caused by herpes zoster are described. This extremely rare complication occurred between 5 days and 14 days following the skin eruption and slowly resolved with or without treatment. Biopsy of a chronic inflammatory lesion on the cheek of one patient demonstrated a sterile vasculitis and granulomatous liponecrosis, a process which may underlie the orbital disease in these patients.

Non specific orbital inflammatory diseases.

The understanding of 'pseudotumors' has significantly changed with the advent of modern imaging techniques by which the tissues affected in the orbit can be identified, and nonspecific orbital inflammatory disease is classified according to the tissue involved. Twenty cases of nonspecific orbital inflammatory diseases were treated between July 1987 and September 1991. Eight patients had myositis, 6 had diffuse disease, 2 each had dacryoadenitis, periscleritis and perineuritis. Involvement of all four recti in one patient and isolated superior rectus and superior oblique each in 2 others are th

Cellular fibronectin and tenascin in an orbital nylon prosthesis removed because of infection caused by Staphylococcus aureus.

An orbital nylon prosthesis was removed because of an infection caused by Staphylococcus aureus that was resistant to antimicrobials. It was processed for histopathology and immunohistochemistry. Within 3 weeks the implant had an extensive ingrowth of fibrovascular tissue containing chronic inflammatory cells, foreign body giant cells, and myofibroblasts. By using the indirect immunofluorescent method, this tissue was found to react with monoclonal antibodies (Mabs) against extradomain A of cellular fibronectin (EDA-cFN) and tenascin (TN). The presence of EDA-cFN and TN within the implant are

Age and gender influence the severity of thyroid-associated ophthalmopathy: a study of 101 patients attending a combined thyroid-eye clinic.

BACKGROUND: The pathogenesis of thyroid-associated ophthalmopathy is thought to be autoimmune, although environmental and genetic factors are also considered to be important. As the morbidity of thyroid-associated ophthalmopathy is considerable and treatment often unsatisfactory, there a need to identify possible predisposing factors. OBJECTIVE: The study was undertaken in order to ascertain the relationship between age, gender and severity of thyroid-associated ophthalmopathy. PATIENTS: One hundred and one consecutive patients with thyroid-associated ophthalmopathy who presented over a peri

Idiopathic sclerosing inflammation of the orbit: immunohistologic analysis and comparison with retroperitoneal fibrosis.

Idiopathic sclerosing inflammation of the orbit is clinically characterized by an insidious, chronic and progressive fibrosing process damaging orbital structures through entrapment and mass effect. Histologically, desmoplasia and a sparse infiltrate of lymphocytes, histiocytes, plasma cells, and occasional neutrophils and eosinophils are seen. An immune pathogenesis is suspected but presently poorly understood. To characterize the inflammatory infiltrate and to compare orbital and other inflammatory fibrosing lesions, immunoperoxidase studies using the streptavidin method were performed on 16

Hematic cyst of the orbit.

We describe a case of a 44-year-old man, presenting with progressive proptosis of the right eye. Computed Tomographic (CT)-examination showed a cystic mass in the lacrimal gland fossa of the right orbit. The lesion was removed in toto. Histological examination showed evidence of old and recent bleedings and a chronic inflammatory reaction with numerous foreign-body giant cells without epithelial lining. The diagnosis of an orbital hematic cyst was established. The pathologic and pathogenetic features of this lesion are discussed. The hematic cyst should be borne in mind as a possible cause of

Hydraulic orbital injection injuries.

BACKGROUND: A high-pressure stream from mechanical equipment may inject gas or liquids deep into the orbit with few initial clinical signs. Aggressive surgical debridement as used in the extremities for the treatment of injection injuries is not possible in the orbit. METHODS: Four patients with orbital injection injuries from farm or industrial equipment are presented. Previously reported cases of high-pressure injection injury are reviewed. RESULTS: Two patients suffered localized anterior orbital inflammation partially responsive to steroidal and nonsteroidal anti-inflammatory agents. Lat

Pulseless (Takayasu) disease with ophthalmic manifestations.

Pulseless disease (PD) is a rare disorder in which inflammation of the aorta and its major branches leads to stenosis or occlusion of these arteries. It mainly affects young Oriental women, who suffer chronic ischemic injury to tissues of the brain, orbits, upper limbs, myocardium, and kidneys. The ophthalmologic features of pulseless disease tend to be late manifestations, and can include ischemia of the retina, choroid, and anterior segment. The inflammatory process may be reversed in early stages with systemic corticosteroids, but, more frequently, significant arterial stenosis necessitates

[Neck tumors: struma].

Goiters are most commonly caused by increased, usually nodular growth of thyroid tissue ('simple' goiter), stimulation of thyroid growth by autoantibodies (Grave's disease) or thyroiditis. Rarer causes of goiter are malignant thyroid tumors and hereditary disturbances of thyroid hormone synthesis goiter: due to iodine deficiency should be found no longer in Switzerland because of the adequate iodine supplementation in this country. Differentiating the various types of goiters is usually possible on the basis of clinical signs and a few laboratory tests (e.g. measurement of thyroid antibodies).

Role of connective tissue autoimmunity in Graves' ophthalmopathy.

Histologic similarities exist between the tissues involved in the extrathyroidal manifestations of Graves' disease, namely ophthalmopathy and pretibial dermopathy. Both conditions are characterized by an accumulation of glycosaminoglycans (GAGs) and an infiltration of lymphocytes. We have shown that interleukin-1 and transforming growth factor-beta, cytokines released by the local inflammatory cell infiltrate, are capable of stimulating GAG synthesis by retroocular and pretibial fibroblasts. Additionally, affected retroocular and pretibial fibroblasts demonstrate an enhanced induction of HLA-D

Retroocular fibroblasts: important effector cells in Graves' ophthalmopathy.

Retroocular and pretibial fibroblasts are likely important effector cells in Graves' ophthalmopathy and pretibial dermopathy. Histologic similarities exist between the tissues involved in these clinically diverse extrathyroidal manifestations of Graves' disease. Both conditions are characterized by an accumulation of glycosaminoglycans (GAGs) and an infiltration of lymphocytes. We have shown that particular cytokines, probably released by the local inflammatory cell infiltrate, are capable of stimulating GAG synthesis by retroocular and pretibial fibroblasts. Varied clinical spectrum of necrobiotic xanthogranuloma.

Four cases are presented that illustrate a wide spectrum of ophthalmologic and systemic features of necrobiotic xanthogranuloma (NXG). Case 1 initially had signs of Cogan syndrome, and then developed chronic lymphocytic leukemia. Case 2, the first case of NXG to undergo autopsy, had progressive cicatricial lid retraction and corneal perforation. Case 3 had a more typical presentation of diplopia and blepharoptosis caused by orbital and periorbital infiltrative masses. Case 4 had nondeforming periocular skin lesions over a 6-year period. In all four cases, the diagnosis was made on the basis of

[Functional endoscopic ethmoid bone revision in inflammatory paranasal sinus diseases in childhood].

A total of 57 children aged between 5 and 15 years underwent endoscopic ethmoid surgery for recurrent acute or chronic rhinogenic sinusitis, chronic sinusitis associated with bronchial asthma or chronic bronchitis, early rhinogenous orbital complications and paranasal sinus disease associated with mucoviscidosis. The indications for surgical intervention are based on symptoms, the findings on nasal endoscopy and a CT scan of the paranasal sinuses in the coronal plane. The preoperative examination shows that in childhood there are endoscopically detectable anatomical variations of the ethmoid b

Low-dose methotrexate therapy for ocular inflammatory disease.

BACKGROUND: Methotrexate is a second-line anti-inflammatory agent used in the treatment of rheumatic diseases. At low doses (12.5 mg/week), it is associated with few serious side effects. METHODS: Twenty-two patients (5 men, 17 women) with chronic noninfectious ocular inflammatory disease, who had not responded to or who had become intolerant of corticosteroid or alternate cytotoxic agents, were treated weekly with oral low-dose, pulse methotrexate. Treated diseases included chronic uveitis-vitreitis (9), scleritis (4), inflammatory pseudotumor (3), orbital myositis (3), and retinal vasculiti

Orbital lesions with granulomatous inflammation.

Orbital lesions characterized by granulomatous inflammation are a heterogeneous group of diseases of various causes with a common histopathological substrate involving aggregates of epithelioid cells. Forty-one patients (27 females and 14 males) with biopsy-proven granulomatous inflammation were seen at an orbital clinic between 1978 and 1989. The mean age at presentation was 40.2 (extremes 6 and 77) years. Two main clinical presentations were noted: painless, subacute or chronic mass effect, and tender, subacute inflammatory process. Six patients had secondary features that were infiltrative

[Two cases of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) associated with Graves' disease].

We have reported two cases of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) associated with Graves' disease. Case 1: a 45-year-old woman noticed a diffuse goiter, palpitation and emaciation in 1977. Laboratory studies confirmed that she had Graves' disease, and she was treated with antithyroid drug. In 1986, when the hyperthyroidism was subsided, she showed progressive symmetrical weakness and numbness in her limbs, and she was almost in tetraplegia at 1987. Markedly slowed motor and sensory nerve conductions and elevated CSF proteins as well as clinical manifestations confi
