Pseudotumour of the maxillary antrum.

Orbital pseudotumour is a non-specific inflammation of the orbit which forms a tumour-like mass composed of increased connective tissue and ground substance with vascular proliferation and an infiltrate of chronic inflammatory cells. No demonstrable aetiological agents are found locally, and there is no evidence of specific disease entities such as Hodgkins, SLE or temporal arteritis. The condition may present in many different ways to the otolaryngologist, and is therefore a rare but important differential diagnosis to be born in mind. We report a rare case of orbital pseudotumour presenting

Orbital myositis--a case report.

Orbital myositis is an idiopathic inflammatory condition involving the extraocular muscles. A case of presumed orbital myositis is presented, displaying typical features on orbital computed tomography, and a dramatic response to corticosteroid therapy. Atypical features of acute-on-chronic course, and concomitant sinus disease are discussed. A differential diagnosis of enlarged extraocular muscles is also discussed, and the benefits of corticosteroid trial before formal biopsy in the appropriate clinical setting is emphasised.

[Histopathological observations on orbital fatty tissue in dysthyroid ophthalmopathy].

Seventeen specimens of orbital fatty tissue taken from cases of dysthyroid ophthalmopathy at the time of operation were observed by light and electron microscopy, and also by immunohistochemical staining. The pathological changes observed were infiltration of the chronic inflammatory cells, deposit of fibrin, sclerosis of vascular wall, occlusion of the vascular lumen, and accumulation of hydrophilic mucopolysaccharide. Immunoglobulin G was detected in fat cells and lymphocytes, and some of the infiltrating lymphocytes were determined to be T cells, by immunohistochemistry. These findings sugg

[Orbital myositis: an ultrastructural and immunohistochemical study].

We present a morphological and ultrastructural study from one case of orbitary myositis in a 35 years old male patient with asymmetrical localytation and clinical behaviour as a tumor. The morphological study shows a chronic inflammatory lesion which causes atrophy and necrosis of the muscular tissue. On the ultrastructural image it is remarkable the atrophy of the muscle fibers with loos of the sarcomers, joined with signs of regeneration. The inflammatory infiltrate is polimorfous, with T and B cells immunihistochemically detected. There is also a macrophagic population identified by the mor

[MR imaging of orbital tumors].

Nine cases of pathologically confirmed orbital tumors were evaluated with magnetic resonance imaging (MRI) at 0.5T. Seven cases were examined with orbit-coil using 7-mm-thick sections and two cases were examined with head-coil using 10-mm-thick sections. All lesions were clearly demonstrated on T1-weighted images as low intensity. However, the lesions were indistinct on T2-weighted images because of low contrast between the lesions and surrounding orbital fat and artefact produced by eye movement. Two of three cases with meningioma examined with orbit-coil were clearly discriminated from the o

Orbital inflammation and optic neuropathies associated with chronic sinusitis of intranasal cocaine abuse. Possible role of contiguous inflammation.

Three cases of long-standing intranasal cocaine abuse were associated with orbitopathy or optic neuropathy. All three cases were characterized by chronic sinusitis. Histopathologic examination of involved sinus and orbital tissues revealed chronic and acute nonspecific inflammation, with no evidence of unusual infections or of idiopathic midline destructive disease. In two of the cases, there was radiographic evidence of contiguous orbital inflammation associated with sinusitis. The inflammatory orbital process in these two cases was steroid responsive, but in one case recurrent inflammation o

Sarcoidosis associated uveitis. Parasitization of vitreous leucocytes by mollicute-like organisms.

Mollicute-Like Organisms (MLO) have been reported to be a cause of uveal tract and orbital chronic inflammatory disease. MLO are intracellular cytopathogenic cell wall deficient bacteria. No culture system exists for MLO, MLO disease diagnosis is based chiefly on direct detection of the organisms within diseased cells using a transmission electron microscope. Uveitis producing MLO are detectable within vitreous leucocytes as 0.005-0.01 micron filaments and undulating pleomorphic 0.01-1.0 micron tubulo-spherical bodies. Human uveitis producing MLO can be passed to laboratory animals. Inoculatio

[Molecular genetic detection of polyclonal immune response in autoimmune thyroiditis and inflammatory bowel diseases].

Southern blot hybridization techniques were used to analyze the arrangements of the immunoglobulin and the T cell antigen receptor genes in lymphocytes of patients with Graves disease and Hashimoto's thyroiditis as well as in patients with Crohn's disease, chronic ulcerative colitis, and with other gastrointestinal disease. The results indicate that the immune response of autoimmune thyroid disease and inflammatory bowel disease is of polyclonal origin.

Chronic orbital inflammatory disease: parasitisation of orbital leucocytes by mollicute-like organisms.

Chronic orbital inflammatory disease (COID) is usually considered non-infectious and idiopathic. Treatment is empirical, palliative, and may not prevent disease progression. COID occurs in isolation or in association with various systemic diseases. Exophthalmos may be an important presenting sign. Vasculitis, lymphoid infiltrates, and granulomas are common. Mollicute-like organisms (MLO) parasitising and destroying vitreous leucocytes are often found to cause human chronic uveitis when an appropriate search is made. Inoculation of these MLO into mouse eyelids produced chronic uveitis and exoph

Mouse exophthalmic chronic orbital inflammatory disease. Induction by human leucocyte intracellular Mollicutes.

Mollicutes are cell wall deficient bacteria which may be overlooked or confused with viruses because of indistinct light microscopic morphology, poor staining, difficulty in cultivation, and the ability to pass 0.450 micron filters. As they have a distinctive ultrastructural appearance they can be identified using transmission electron microscopy [TEM]. Using TEM vitreous leucocytes from chronic endogenous uveitis patients may demonstrate non-cultivable intracellular 0.005-1.0 micron mollicute-like organisms [MLO], some of which develop into distinctive cell walled cocci, 0.5-0.7 micron in dia

Acute severe irreversible visual loss with sphenoethmoiditis-'posterior' orbital cellulitis.

Orbital cellulitis secondary to adjacent paranasal sinusitis presents with marked proptosis, ophthalmoplegia, eyelid edema, chemosis, and/or conjunctival hyperemia. These conditions often precede visual dysfunction. "Posterior" orbital cellulitis secondary to sphenoethmoidal sinusitis may be defined as the clinical syndrome in which early severe visual loss overshadows or precedes accompanying inflammatory orbital signs. The visual loss may be attributed to involvement of the intracanalicular or orbital apical segment of the optic nerve. Total irreversible unilateral visual loss developed in t

Painful orbital inflammatory lesions and mastocytosis.

A 15-year-old female patient developed painful nodules in an anophthalmic socket that were exacerbated by treatment with steroids and narcotics. Although orbit biopsy showed chronic inflammation, the diagnosis of mastocytosis was confirmed with skin biopsy and urinary levels of histamine. The patient responded to excision of nodules and treatment for mastocytosis. Painful orbital lesions associated with mastocytosis have not been reported previously.

Paranasal sinusitis, orbital abscess, and inflammatory tumors of the orbit.

We present the clinical, radiological, and histopathological findings from a case of unilateral inflammatory mass of the right orbit, originating in the ipsilateral paranasal sinuses. A connection between inflammatory tumors of the orbit and paranasal sinusitis has been suspected for many years. This case shows a definite association between chronic paranasal sinusitis, orbital cellulitis, subperiosteal abscess, and formation of an inflammatory orbital tumor. Early diagnosis combined with appropriate therapeutic measures is essential in order to prevent adverse consequences, which can include

Painful ophthalmoplegia (Tolosa-Hunt variant): autopsy findings in a patient with necrotizing intracavernous carotid vasculitis and inflammatory disease of the orbit.

A 79-year-old man with unremitting painful ophthalmoplegia had a necrotizing inflammatory process that involved the intracranial and intracavernous portions of the right internal carotid artery. The condition ultimately resulted in rupture of the carotid vessel and death. At autopsy, thrombophlebitis of the sphenopalatine sinus was also noted. To our knowledge, this is the second reported autopsy study of painful ophthalmoplegia and chronic nongranulomatous inflammation that involved the carotid siphon and the cavernous sinus.

Orbital lymphoproliferative and inflammatory lesions.

We reviewed the records and biopsy specimens of 38 patients with clinically similar orbital lymphoproliferative and low-grade inflammatory lesions referred to one ophthalmologist at the University of British Columbia between 1977 and 1984. Twenty-six patients had lymphoproliferative lesions, the main feature being a densely cellular population of small lymphocytes. This group was further divided into reactive lymphoid hyperplasia, malignant lymphoma, Hodgkin's disease and atypical lymphoid hyperplasia. Symptoms and signs were similar within the subgroups. The mean length of follow-up was 3.7 y

Orbital myositis: a study of six cases.

Orbital myositis implies orbital inflammation confined to one or more of the extraocular muscles. Orbital computerised tomography (CT) demonstrates irregular extraocular muscle enlargement which extends anteriorly to involve the tendon (muscle insertion). Six cases of presumed orbital myositis are reported, in each of whom the diagnosis was suspected clinically and confirmed by the orbital CT scan appearances. The mean age of the patients was 33 years (range 8-45 years). All presented with painful ophthalmoplegia and the majority manifested proptosis (five cases), conjunctival congestion (five

Orbital pseudotumor of the levator muscle.

A 58-year-old white man with chronic progressive external ophthalmoplegia developed proptosis and an improvement in his ptosis from a mass in the superior orbit. A biopsy showed the mass to consist of an inflammatory mass of the levator palpebrae superioris muscle. As the inflammatory mass responded to periorbital and systemic steroids, the ptosis recurred. Inflammatory pseudotumor of the levator muscle is extremely unusual and has not been reported previously in patients with chronic progressive external ophthalmoplegia.

Bilateral recurrent orbital inflammation associated with retroperitoneal fibrosclerosis.

Orbital inflammation initially presented in a patient who five years later developed renal obstruction from retroperitoneal fibrosclerosis. Biopsy revealed infiltration of chronic lymphocytic cells in the orbit and dense fibrous connective tissue in the pelvis. Episodes of proptosis were controlled with systemic steroids and non-steroidal anti-inflammatory agents. The hydronephrosis was relieved by intraperitonisation of the ureters. Similarities to previously reported cases are discussed. This case shows the importance of searching for systemic disease in patients with orbital inflammatory di

Orbital pseudotumor in Crohn's colitis.

A 17-year-old woman presented with orbital pain, diplopia, ptosis, and upper lid swelling due to an orbital pseudotumor involving the superior rectus-levator complex. Although she had no prior symptoms of chronic inflammatory bowel disease, diagnostic evaluation revealed Crohn's colitis. Diagnostic evaluation of patients with "idiopathic" orbital pseudotumor should include testing to rule out chronic inflammatory bowel disease.

The nonspecific orbital inflammatory syndromes.

The nonspecific orbital inflammatory syndromes are a peculiar group of inflammations that may occur in acute or subacute forms and can become chronic. They may be diffuse or primarily localized to a specific tissue of the orbit. The nonspecific inflammations that are targeted toward specific tissues are myositis, dacryoadenitis, perineuritis, and periscleritis. Each of these syndromes has definite signs, symptoms, ultrasonic and radiologic findings. Therefore, this group of nonspecific inflammations should be more clearly classified for purposes of better understanding and better management. A
