Lymphoid proliferations in the orbit: malignant or benign?

Clinical, pathological, and immunological analysis of 20 patients with ocular adnexal lymphoid disease has demonstrated several parameters which are useful for distinguishing malignant from benign lesions. Patients in the fourth or fifth decade of life presenting with an acute history of pain, oedema, epiphora, double vision, and ptosis, with a mass localised in the lacrimal gland area, are more likely to have a pseudolymphoma or a chronic inflammatory lesion than a true non-Hodgkin lymphoma (NHL). It is not possible to obtain a definite diagnosis without surgical intervention, because only th

B- and T-lymphocytes in ocular disease.

Ocular inflammatory diseases and ocular adnexal lymphoid tumors have become less obscure and intimidating by virtue of our ability to study the infiltrates in these various diseases for their B-lymphocyte and T-lymphocyte composition. Comparisons are also possible between lymphocytic profiles in the peripheral blood and the precise composition of the in situ infiltrates within the ocular tissue themselves. The availability of monoclonal antibodies, which can determine T-lymphocytic subsets such as T-helper cells and T-suppressor/cytotoxic cells, natural killer cells, and monocytes-histiocytes,

Orbital aspergillosis. Report of a case diagnosed by fine needle aspiration biopsy.

Presentation is made of a case of orbital aspergillosis, a rare condition that may clinically mimic nonspecific orbital inflammatory disease. It can be a subacute or chronic disease that develops in otherwise healthy individuals. Fine needle aspiration biopsy, which has been useful in the diagnosis of a variety of orbital diseases, provided the initial diagnosis in this case.

Mucin impaction tumor of the paranasal sinuses: a new clinical entity?

A variety of uncommon benign lesions occur in the paranasal sinuses and have been reported to masquerade as carcinomas. Nearly all of such cases have been limited to the frontal, sphenoid, and ethmoid sinuses with an isolated report of maxillary sinus involvement. The classic roentgenologic picture is that of bony destruction. Heretofore described destructive lesions of the maxillary sinus include the mucocele, mucous retention cyst, pseudocyst, pyocele and cholesteatoma. This report deals with a previously undescribed entity, the mucin impaction tumor located in a septate maxillary sinus. Thi

[2 cases simulating Tolosa-Hunt syndrome].

The Tolosa-Hunt syndrome is characterized by recurrent unilateral painful ophthalmoplegia which responds to systemic steroid therapy dramatically. The etiology appears to be a non-specific inflammation in the cavernous sinus and the superior orbital fissure. Two interesting cases similar to this syndrome are described. One is a 54-year-old man with moderate left exophthalmos who had no complaint of retro-orbital pain. CT scan demonstrated the left orbital tumor, and the orbital decompression surgery was performed. The white-yellowish tumor was found extending the orbit through the superior orb

Angiolymphoid hyperplasia with eosinophilia (Kimura's disease) of the orbit and ocular adnexa.

We conducted a clinicopathologic study of eight cases of angiolymphoid hyperplasia with eosinophilia (Kimura's disease). The orbit was involved in five patients and the ocular adnexa (eyelid and inner canthus) in three. The median age of the six men and two women was 51 years (range, 38 to 72 years). The lesions were characterized by an abnormal proliferation of small vessels lined by plump endothelial cells, associated with a chronic inflammatory infiltrate with prominent tissue eosinophilia and scattered lymphoid follicles. Serial sectioning of two tumors disclosed that these small vessels a

Orbital arteriovenous malformation with secondary capillary angiomatosis treated by embolization with silastic liquid.

A 19-year-old Caucasian man developed signs of an orbital arteriovenous malformation, which was biopsied and then treated by embolization with a rapidly polymerizing silastic liquid. The biopsy from the initial specimen showed arteries and veins that were malformed with irregular elasticas and muscularis thicknesses, but the most curious feature was a secondary endothelial cell proliferation of such proportions as to simulate in various fields a capillary hemangioma of childhood. Approximately 50% of the bulk of the tumor was the result of the secondary endothelial cell proliferation, which we

Fine-needle aspiration biopsy in the diagnosis of orbital tumours.

Fine-needle aspiration biopsy was used for the diagnosis of 25 consecutive patients with orbital tumours. CT scan was used to locate the lesion accurately. In two cases of orbital sarcoma and three out of four cases of orbital meningeoma, pre-operative fine-needle biopsy provided the correct diagnosis for proper operative treatment. Post-operative histopathological examination of the tumours proved that the cytological diagnoses had been correct. Two cases were diagnosed as metastatic carcinomas, 4 as malignant lymphomas and 11 as chronic inflammatory lesions. Orbitotomies were performed in 3

Pseudo-tumours, a clinical concept.

UNLABELLED: Space-filling lesions in the orbit can be divided into 3 large groups: 1. Acute and subacute inflammatory processes (bacterial, fungal, parasitic, foreign body). 2. Benign and malignant tumours, true new growths. 3. Pseudo-tumours. The conditions in group 3 are also space-filling lesions, but have the characteristics of a chronic, infiltrating, inflammatory process. In 11% of 340 consecutive patients in the Orbital Centre in Amsterdam this diagnosis was made. The group is far from uniform: the dominant cell type varies from one case to another. Usually lymphocytic cells are dominan

Sinus involvement in inflammatory orbital pseudotumor.

Orbital pseudotumor is a difficult diagnosis to establish preoperatively. The relationship between sinus disease and orbital pseudotumor is controversial. We describe two patients with unilateral proptosis, diplopia, palpable orbital masses, ocular discomfort, and sinus problems of short duration. Echographically, both had low reflective masses in the orbit and the adjacent sinuses. Roentgenograms and echograms were interpreted as showing erosion of the bony orbital wall. A presumptive diagnosis of sinus malignant neoplasm with orbital extension was made. Sinus histopathologic examination in o

Mucosal cyst of lacrimal gland fossa simulating lacrimal gland neoplasm.

A 28-year-old man presented with an orbital tumour that was localized by computérized tomography to the fossa of the right lacrimal gland. A benign mixed tumour of the lacrimal gland was the clinical diagnosis, but lateral orbitotomy revealed a subperiosteal cystic mass in the lacrimal gland fossa that extended superiorly and medially into the frontal sinus. Light microscopic examination showed a cyst lined by nonkeratinized stratified squamous epithelium with occasional goblet cells; blood products were present in the lumen, and a chronic inflammatory cell infiltrate was noted in the subepith

Otolaryngologic manifestations of orbital pseudotumors.

Orbital pseudotumor is believed to be a nonspecific, acute or chronic inflammatory process involving the intraorbital soft tissue. Orbital pseudotumor may masquerade as sinusitis with orbital complications or as a severe upper respiratory tract infection. Five cases are presented to illustrate the difficulties in diagnosis when initial symptoms relate to the upper respiratory system rather than to an isolated orbital mass. In these cases, orbital ultrasound and computed tomograms were helpful in making the diagnosis. Treatment consisted of systemic corticosteroids in four of the five patients.

Cranial neuropathies in sinus disease.

To study the problem of cranial neuropathies in sinus disease the inpatient experience at the University of California, San Francisco, and San Francisco General Hospitals was reviewed. The incidence of cranial nerve involvement in acute and chronic sinus inflammations was low (8 percent and 4 percent respectively). The incidence in neoplastic disease of the sinuses was considerably higher (32 percent). Cranial neuropathies occurred in inflammatory disease more frequently when associated with mucopyocele, mucormycosis, and orbital cellulitis. In both inflammatory and neoplastic disease, when cr

Endoscopic findings of maxillary sinus after middle face fractures.

Of 93 patients with fractures of the mid-face who appeared for postoperative check-up, 25 were selected by chance for sinuscopy of the maxillary sinus. Chronic inflammatory mucosal changes occurred in 35 per cent; other pathological sequelae included solitary polyps, mucocele, scarry synechia, and prolapse of orbital fat and osteoma. In spite of these findings, all patients were free of complaints. The primary surgical revision was therefore declined in favour of a waiting attitude. For the diagnostic clarification of late complications, sinuscopy with Hopkin's optics offers a simple and effic

Chronic lymphocytic thyroiditis, thyrotoxicosis, and low radioactive iodine uptake. Report of four cases.

To characterize four patients with thyrotoxicosis and a low radioactive iodine uptake, thyroid biopsies were performed, and iodine metabolism was studied. Histologic examination showed the presence of chronic lymphocytic thyroiditis, with no features of Graves's disease, in all. Detailed studies in one patient revealed insufficient metabolism of iodine to account for the clinical and chemical features of thyrotoxicosis, which implies that release of stored hormone by the inflammatory process causes the thyrotoxic state. The thyrotoxicosis in this entity subsides spontaneously. Thus, this form

Ultrastructure of plasma cells containing Russell bodies in human stomach and thyroid.

The fine structure is illustrated of plasma cells containing Russell bodies. The material is from chronic inflammatory cell infiltrates in human gastric mucosa and thyroid glands in Graves' disease and Hashimoto's thyroiditis. It is noted that the Russell bodies lie in distended cisternae of rough endoplasmic reticulum and are easily distinguished from less electron-dense ;secretory granules' covered by a smooth membrane that are situated in the Golgi complex.
