Forced Duction Testing for Management of Small Orbital Floor Blowout Fractures.

Forced duction testing (FDT) is a diagnostic clinical test of extraocular muscle function employed to confirm the mechanical and restrictive nature of a patient's defective upgaze. Orbital floor blowout fractures (OFBFs) may lead to bony entrapment of the inferior orbital muscles and connective tissue, resulting in restricted vertical gaze and early or late enophthalmos. Management of OFBFs aims to restore normal ocular anatomy and physiology and may require definitive surgical intervention if there is significant entrapment and large fractures. The authors report a 19-year-old man with a smal

The accuracy of reconstruction of orbital wall fractures using prebent mesh versus patient specific implants: a randomized clinical trial.

BACKGROUND: Repairing fractures of the orbital wall and floor remains a complex challenge due to the intricate anatomy of the orbit and the limited visibility during surgery. With the advancement of digital technologies, tools such as DICOM (Digital Imaging and Communications in Medicine) now allow the creation of virtual mirrored models of the unaffected orbit, which can greatly assist in surgical planning and reconstruction. This study aimed to evaluate the accuracy of orbital wall reconstruction using patient-specific implants (PSIs) versus prebent titanium mesh. MATERIALS AND METHODS: A si

Functional and Aesthetic Outcomes of the Subtarsal Approach in Zygomaticomaxillary Complex Fractures With Orbital Floor Involvement: A Prospective Cohort Study.

Introduction Zygomaticomaxillary complex (ZMC) fractures, often resulting from high-impact trauma, frequently involve the orbital floor, leading to functional and aesthetic complications such as diplopia, enophthalmos, and facial asymmetry. The subtarsal approach, which involves an incision through the skin, provides access to the orbital floor while potentially reducing complications. This study aimed to evaluate the efficacy and safety of the subtarsal approach in managing ZMC fractures with orbital floor discontinuity, focusing on functional outcomes (nerve paresthesia, ocular movement, and

Beyond the Maxillary Sinus: A Proposed Classification of Chronic Sinus Atelectasis.

BACKGROUND: Chronic maxillary atelectasis (CMA) describes the loss of maxillary sinus volume and has been well-described in the literature under many different names, creating inconsistency in reporting. A recent proposal has standardized the nomenclature but does not include disease of other paranasal sinuses. OBJECTIVE: We describe a unique case of chronic ethmoid atelectasis, briefly review literature associated with CMA, and propose an expanded classification of chronic sinus atelectasis. RESULTS: A 70-year-old man presented to the emergency department with transient vision loss and was

Ocular Translation Due to Gravitational Acceleration.

PURPOSE: We asked if human globe position in the orbit is influenced by gravitational head orientation. METHODS: In 11 adult volunteers, surface coil magnetic resonance imaging (MRI) in 2 mm thick axial and quasi-coronal planes with a centered visual target was performed in supine position that aligned the cranial midline along the gravity vector, and was repeated in right and left decubitus positions that oriented gravity orthogonally. From coronal MRI, we computed globe centroid positions relative to the whole head. RESULTS: In decubitus posture relative to supine, the upper eye shifted 0.

An Exceptional Case of Blow-Out Fracture with Complete Globe Dislocation into the Maxillary Sinus: Diagnostic Imaging and Surgical Reconstruction.

Orbital floor fractures are primarily caused by blunt trauma to the area around the eyes. These injuries most commonly affect the orbital floor and medial wall due to the fragility of these structures. The mechanism typically involves transmission of force through the orbital rim or an acute increase in intraorbital pressure caused by globe displacement. Blowout fractures often occur alongside additional maxillofacial fractures and periorbital soft tissue injuries. The reported causes mirror those of general maxillofacial trauma and include motor vehicle collisions, interpersonal violence, fal

Retrospective and Comprehensive Evaluation of Posttraumatic Enophthalmos after Intraorbital Fat Injection.

OBJECTIVES: Post-traumatic enophthalmos (PE) is characterized by a posteriorly displaced eyeball (enophthalmos), deep superior sulcus (sunken upper eyelid), and bilateral asymmetry. Intraorbital injection of autologous fat has shown potential for correcting enophthalmos, but several limitations remain, such as patient heterogeneity, small sample sizes, non-quantitative assessment methods and a focus solely on enophthalmos. This study aims to assess both enophthalmos and sunken upper eyelids in a homogeneous patient group using quantitative methods. METHODS: Forty-three sighted PE eyes were re
