BACKGROUND: Dysthyroid optic neuropathy (DON) is occasionally resistant to corticosteroid treatment. In these cases, emergent transnasal endoscopic orbital decompression may be the optimal therapy method. We evaluated 13 patients with steroid-resistant DON to determine the potential benefit of this surgical intervention.
METHODS: Thirteen patients with steroid-resistant DON seen at Shenzhen Eye Hospital (China) between January 2023 and April 2025 were included in our study. All patients underwent emergent transnasal endoscopic orbital decompression and received postoperative corticosteroids a

Thyroid eye disease (TED), the most common extrathyroidal manifestation of Graves' disease, requires individualized management that integrates medical, radiotherapeutic, and surgical interventions. In this article, we present a society-endorsed, survey-informed Korean clinical practice framework, including consensus elements, for TED management. This framework integrates the 2022 American Thyroid Association/European Thyroid Association consensus, the 2021 European Group on Graves' Orbitopathy (EUGOGO) guideline, and contemporary Korean specialist practice patterns. A 2025 nationwide survey of

BACKGROUND: Post-thyroxine treatment of thyroid dysfunction remains a clinical concern, especially in Middle Eastern populations.
METHODS: This descriptive cross-sectional study was conducted in 2023 at King Fahad Hospital, Hufof, Kingdom of Saudi Arabia. Of the 237 patients treated with L-thyroxine (L-T4) for hypothyroidism, 163 patients, almost exclusively females (152 females, 11 males), met the inclusion criteria and were enrolled. Thyroid hormones, lipid profiles, and 25-hydroxyvitamin D (25OH-D) were measured using standard laboratory assays.
RESULTS: Only 57% of patients achieved euth

We report a case of severe thyroid eye disease (TED) complicated by dysthyroid optic neuropathy (DON) in a 55-year-old woman with Graves' disease. Despite insufficient response to high-dose intravenous steroid therapy, teprotumumab led to marked improvement in proptosis, orbital inflammation, and visual function. However, treatment was associated with progressive and persistent sensorineural hearing loss after completing eight infusions. Auditory symptoms began after the fifth infusion with intermittent aural fullness and sound reverberation, progressing to mild down-sloping sensorineural hear

Background/Objective: Dysthyroid Optic Neuropathy (DON) is a severe complication of Thyroid Eye Disease (TED) leading to irreversible visual impairment. Its pathogenesis remains unclear, and early predictive tools are lacking. The study aims to investigate peripheral immune characteristics associated with DON, focusing on the IL-2+Th1/CD4+Tregs axis. Methods: A retrospective study was conducted in 37 TED patients, including DON (n = 22) and non-DON (n = 15) groups. Peripheral blood immune cell subsets were quantified using flow cytometry. Clinical data and peripheral blood immune indicators in

IMPORTANCE: Dysthyroid optic neuropathy (DON) is a sight-threatening complication of thyroid eye disease (TED). The pivotal teprotumumab trials excluded patients with DON, and the only prior treatment meta-analysis addressed intravenous glucocorticoids and orbital decompression, leaving the role of teprotumumab in DON unsynthesised.
OBJECTIVE: To systematically review and, where possible, meta-analyse visual and orbital outcomes following teprotumumab treatment for DON.
METHODS: PubMed, Ovid (MEDLINE/Embase) and Web of Science were searched from inception to 7 June 2026, following PRISMA 202

Thyroid eye disease (TED), also referred to Graves' ophthalmopathy, is a disfiguring and vision-threatening autoimmune disease of the retroocular tissues occurring in patients with Graves' disease (GD). Among the extrathyroidal manifestations of GD, TED is the most frequent, being observed in roughly 30-50% of patients. In the immunopathogenesis of TED, the TSH-R/IGF-1R complex has a determinant role. TED is commonly bilateral, and during its active phase it presents with ocular redness, eyelid edema, photophobia, proptosis, and diplopia. In the most severe cases, the involvement of the optic

INTRODUCTION: Close to 50% of patients with Graves' disease have associated ocular involvement, of which dysthyroid optic neuropathy (DON) is prevalent in 3-5% of patients. The response to intravenous glucocorticoids in DON is dependent on early diagnosis. The purpose of this study was to assess the efficacy of various radiological indices (Barrett's index, Nugent index) in predicting DON and treatment response or relapse in this population.
METHODS: This was a retrospective study that included patients presenting with clinically active Graves' orbitopathy (GO) between January 2005 and June 2

Thyroid eye disease is an immune-mediated orbital disorder most commonly associated with Graves' disease. Inflammatory expansion of orbital tissues causes proptosis, exposure symptoms, diplopia, and, in severe cases, dysthyroid optic neuropathy, with significant physical and psychological impact. Maintaining euthyroidism and promoting smoking cessation are central in its management; requiring close coordination between Ophthalmology and Endocrinology teams. Management of thyroid eye disease is guided by disease activity, severity, and phenotype, with the greatest response to immunomodulatory t

Dysthyroid optic neuropathy (DON) is a severe, vision-threatening manifestation of thyroid eye disease. We conducted a retrospective descriptive case series of consecutive patients with DON treated with teprotumumab at a single center to evaluate functional, structural, magnetic resonance imaging (MRI)-based inflammatory, and safety outcomes. Six patients (10 eyes) were included. Eye-level outcomes were assessed at baseline and during follow-up using generalized estimating equations to account for within-patient correlation. The primary outcomes were best-corrected visual acuity (BCVA), expres

BACKGROUND: Dysthyroid optic neuropathy (DON) is a vision-threatening complication of Graves' ophthalmopathy, and insufficient orbital decompression may result in persistent or irreversible visual impairment. While endoscopic transnasal orbital decompression (ETOD) has become a mainstream surgical approach for orbital decompression, the optimal extent of ETOD remains incompletely defined, particularly regarding reproducible anatomical landmarks and safe surgical boundaries.
METHODS: This retrospective study evaluated patients with DON who underwent an extended ETOD procedure at a tertiary med

Thyroid eye disease (TED) is a complex autoimmune disorder and the most common extrathyroidal manifestation of Graves' disease, characterized by orbital inflammation, tissue remodelling, and varying degrees of functional and cosmetic impairment. Although often mild, moderate-to-severe and sight-threatening forms can significantly compromise vision and quality of life. Advances in the understanding of TED pathophysiology - particularly the roles of orbital fibroblasts, thyrotropin receptor and insulin-like growth factor 1 receptor signalling- have transformed its diagnostic and therapeutic land

BACKGROUND: Spondylolisthesis is a common cause of lower back pain, often accompanied by spinal stenosis, disc degeneration, and altered biomechanics. Reduction is achieved anteriorly through anterior lumbar interbody fusion (ALIF) or posteriorly through pedicle screws/rods. ALIF allows for favorable biomechanics for fusion but has limited reduction capability in high-grade slips. Anterior reduction allows for increased slip reduction without relying on spacer impaction but has only been explored in grade I spondylolistheses. The objective of this study was to evaluate the reduction capacity o

BACKGROUND: Alemtuzumab (Lemtrada) is highly effective for selected relapsing multiple sclerosis (MS) but can cause autoimmune thyroid dysfunction (AITD). Our aim was to retrospectively review patients and the monitoring of their thyroid function tests (TFTs) during and after treatment with alemtuzumab. We wanted to summarise the incidence and characterise the nature of TD in our Northern Ireland (NI) cohort.
METHODS: Data was obtained from the NI Disease Modifying Treatment (NIDMT) database for all patients with relapsing remitting MS receiving alemtuzumab in the period between 2015 and 2024
