A patient with simultaneous involvement of the right medial rectus by dysthyroid and myasthenic myopathy is presented. These abnormalities were confirmed by a positive Tensilon test, computerized tomography demonstrating an enlarged right medial rectus, and a right medial rectus muscle biopsy demonstrating histopathology typical of both diseases.

Thirty-four patients with childhood Graves' disease were examined to determine the incidence and severity of Graves' ophthalmopathy in children and adolescents. In many patients with unilateral proptosis dysthyroid disease can be excluded on clinical grounds. The primary purpose of the CT scan is to exclude a mass lesion but the appearances must be correlated with biochemical tests of thyroid function. Sixteen cases of vertical rectus muscle myopathy in dysthyroid patients are described. In mild cases without diplopia prisms may be adequate treatment. When uncomfortable diplopia is present surgery is indicated when the muscle dysfunction has become stable. Even after surgery further contracture is often experienced, particularly after Ogura antral decompression. The amount of correction obtained by recession of a contracted muscle is greater than that following ordinary strabismus surgery so that usually only one muscle should be operated upon at a time. Echography was used to examine the retro-bulbar space of 150 patients with protosis. An NE Diasonograph was used with an 8 MHz probe using a water bath in the supine position. Lesions were classified ultrasonically as (1) tumour, (2) inflammatory oedema (including pseudo-tumour or other granulomas), (3) traumatic, (4) dysthyroid ophthalmopathy, (5) normal. Two cases of dysthyroid orbital diseases are presented which on CT scan had the characteristic appearance of intraorbital tumors. An operation was performed on one of these patients based on the CT scan appearance. This entity should not be confused with true intraorbital neoplasms. One must not rely exclusively on the CT scan in questionable cases as with any other ancillary diagnostic acid and one must bear in mind that dysthyroid disease in evolution may exactly mimic the CT scan appearance of an intraorbital tumor.

Dysthyroid optic neuropathy (DON) was diagnosed in 36 eyes of 21 patients with progressive visual loss and congestive ophthalmopathy. The clinical recognition of dural arteriovenous malformations (AVM) is a challenge to the ophthalmologist. Unilateral dural AVMs produce monocular signs which may simulate orbit disease. A patient with bilateral dural AVMs presenting with bilateral congestive exophthalmos resembling dysthyroid ophthalmopathy is reported. Spontaneous resolution of the ocular manifestations occurred.
