Reactive Infectious Mucocutaneous Eruption Following a Respiratory Infection in an Adolescent: A Case Report.

Reactive infectious mucocutaneous eruption (RIME) is a recently defined mucocutaneous manifestation associated with respiratory infections, most commonly reported secondary to Mycoplasma pneumoniae. The core diagnostic criteria of RIME involve greater than or equal to two mucosal areas with erosive mucositis, minimal cutaneous lesions, a recent infectious prodrome, and exclusion of drug-induced etiologies. RIME presents with acute mucositis involving the oral, ocular, and genital mucosa, with presentations similar to other mucocutaneous disorders such as Stevens-Johnson syndrome (SJS), erythem

Excess primary healthcare consultations in Norway in 2024 compared to pre-COVID-19-pandemic baseline trends.

BACKGROUND: The risk of post-acute sequelae of COVID-19 (PASC) is estimated at 3-6% per infection in 2024. We hypothesized that widespread SARS-CoV-2 infections could lead to population-level consequences. Our previous study identified substantial increases in Norwegian primary healthcare consultations in 2023-compared to pre-pandemic levels-for conditions associated with acute COVID-19 and PASC. This study extended that analysis to 2024. We then assessed whether observed patterns were compatible with our hypothesis. METHODS: We used data from the Norwegian Syndromic Surveillance System, whic

Refractory Kawasaki Disease With a Coronary Artery Aneurysm in a Three-Month-Old Infant: Diagnostic Challenges and Rescue With Infliximab.

Kawasaki disease (KD) is an acute systemic vasculitis and the leading cause of acquired heart disease in children. Diagnosis is particularly challenging in early infancy due to incomplete presentations. We describe a three-month-old infant with persistent fever, irritability, and lymphadenopathy who was initially treated for bacterial infection. By Day 7, the patient developed conjunctivitis, cracked lips, and pharyngeal congestion, meeting criteria for incomplete Kawasaki disease. Laboratory findings included leukocytosis, anemia, thrombocytosis, elevated C-reactive protein, and hypoalbuminem

Bilateral proptosis as the first manifestation in a patient with acute myeloid leukemia: Case report.

RATIONALE: Myeloid sarcoma (MS), also known as granulocytic sarcoma or chloroma, is a rare manifestation of acute myeloid leukemia (AML) that occurs concurrently with, following, or rarely before the onset of leukemia. It consists of immature myeloid cells that proliferate in extramedullary tissues. Owing to the rare association between bilateral proptosis and AML, it is often misdiagnosed as other conditions that present with proptosis, such as infections (as in our case), vascular lesions, or tumors, particularly rhabdomyosarcoma and neuroblastoma, creating a diagnostic dilemma. PATIENT CON

Incomplete Kawasaki Disease Complicated by Shock: A Diagnostic Challenge in a Child.

Kawasaki disease (KD) is an acute vasculitis of medium-sized vessels that primarily affects children. It can present with incomplete or atypical features, leading to diagnostic delay and increased risk of cardiovascular complications. Kawasaki disease shock syndrome (KDSS) is a rare but severe manifestation characterized by hemodynamic instability and heightened inflammatory response. We report the case of an eight-year-old previously healthy boy who presented with fever and unilateral cervical lymphadenitis following recent travel. Initial evaluation suggested an infectious etiology, and he w

Nummular Keratitis Following Epidemic Keratoconjunctivitis: A Guide to Managing Subepithelial Infiltrates.

INTRODUCTION: Epidemic keratoconjunctivitis (EKC) is a highly contagious viral ocular surface infection characterized by conjunctivitis with corneal involvement. Although EKC is usually self-limiting, corneal sequelae can persist for weeks to months and may significantly impact the quality of life. AIM: To analyze the effectiveness of the treatment protocol and healing of nummular keratitis after EKC. METHODS: This is a retrospective observational study of eight patients with a history of viral conjunctivitis and EKC. Diagnosis and management in the acute stage relied solely on clinical eval

The Evaluation of Zika virus susceptibility in Aedes (Ochlerotatus) japonicus from Hokkaido, Japan.

Zika virus (ZIKV) is a mosquito-borne orthoflavivirus that causes acute symptoms, such as fever and conjunctivitis. Aedes (Stegomyia) aegypti and Ae. (Stg.) albopictus are the primary and secondary vectors, respectively, although other Aedes species may also transmit ZIKV. Aedes (Ochlerotatus) japonicus, native to East Asia, has recently spread to North America and Europe; however, its potential in ZIKV transmission, particularly in Japanese populations, remains unclear. In our previous study, a low viral titer of ZIKV was detected in a single virus-challenged Ae. japonicus individual. In the

EHR Phenotyping Methods for Measuring Treatment Adherence Among People Living With HIV in All of Us: Towards Disparities and Inequalities in HIV Care Continuum.

OBJECTIVES: Kawasaki syndrome (KS) is an acute systemic vasculitis of unknown origin predominantly affecting young children. Early diagnosis is crucial to prevent cardiac complications. However, the differential diagnosis of patients with the incomplete or atypical form of the disease poses a heavy challenge for the paediatrician. Our aim was to evaluate the prevalence of incomplete and atypical cases among children with KS and to identify clinical and laboratory variables that may help differentiate incomplete and atypical KS from other febrile diseases at this age. METHODS: We established a

Orbital apex syndrome secondary to Sweet syndrome.

Sweet syndrome (SS), or acute febrile neutrophilic dermatosis, is a dermatologic, auto-inflammatory disorder of unclear origin, often accompanied by systemic inflammation affecting various tissues, including the eyes. Common ocular manifestations include conjunctivitis but can extend to other ocular tissues. Orbital apex syndrome (OAS) involves damage to several cranial nerves transversing the orbital apex, leading to ophthalmoplegia and vision loss. This case report describes a rare case of OAS secondary to SS. A woman in her 50s with a history of SS presented with right eye loss of vision an

Causal relationships between allergic diseases and significant declines in lung function: a multivariable Mendelian randomization study.

BACKGROUND: Asthma and other allergic diseases are increasing globally each year, with some patients experiencing the co-occurrence of two or more conditions, significantly impacting their quality of life. While these diseases may share certain immune mechanisms, the independent causal relationships between them and lung function remain unclear. OBJECTIVE: This study aims to investigate the independent and interactive effects of allergic asthma, atopic dermatitis, allergic rhinitis, and allergic conjunctivitis on significant declines in lung function. By elucidating the potential causal relat

Incomplete Kawasaki Disease in a Six-Month-Old Child: A Complex Journey to Recovery.

Kawasaki disease (KD) is an acute, self-limited vasculitis that predominantly affects children under five years and can lead to coronary artery aneurysms. Incomplete KD involves an incomplete clinical picture supported by echocardiographic and laboratory findings. In this case, a six-month-old male child presented with a two-day fever, maculopapular erythematous rash, nonpurulent conjunctivitis, throat congestion, mild cough, intermittent loose stools, and was initially diagnosed with a viral illness treated symptomatically. Three days later, he re-presented with unresolved symptoms. Laborator

Identification of atopic dermatitis-associated diseases based on the National health and nutrition examination survey (NHANES) 2013-2018.

Atopic dermatitis (AD) is the most common chronic inflammatory skin disease. Massive cohort studies revealed that AD was associated with allergic diseases, inflammatory diseases, autoimmune diseases, cardiovascular diseases, and mental disorders. We comprehensively and systematically analyzed the correlation between AD and diseases to identify AD-associated diseases (ADADs). We involved 17,924 individuals from the National Health and Nutrition Examination Survey (NHANES) (2013-2018) dataset, and analyzed the correlation between AD and 422 diseases classified by International Classification of

A Closed-Loop Audit: The Assessment of Red Flags and Management of Acute Conjunctivitis in Primary Care.

Background Acute red eye is a common presenting complaint to primary care. Although conjunctivitis is the most common cause, some conditions can be sight-threatening. Diagnosing such conditions can be difficult given the broad differential diagnoses and limited specialist equipment. The National Institute for Health and Care Excellence (NICE) has published five "red flags," which may indicate the need for urgent ophthalmological assessment: reduced visual acuity (VA), copious discharge, marked eye pain/photophobia, contact lens use and recent trauma. For patients not requiring specialist refer
