Levofloxacin/ketorolac combination versus levofloxacin alone for topically treating moderate-severe acute bacterial conjunctivitis: a randomized controlled trial.

BACKGROUND: This study aimed to obtain the first evidence-based microbiological confirmation of the efficacy and an assessment of the local tolerability of a combination of an ocular antibiotic and a non-steroidal anti-inflammatory drug in treating moderate-to-severe acute bacterial conjunctivitis (ABC). METHODS: This was a phase II, randomized, parallel-group, masked-assessor, multicentre study to assess the non-inferiority of levofloxacin plus ketorolac eye drops versus levofloxacin eye drops alone in microbiological eradication (protocol number: LEVOKETO_02-2020). Treatment lasted 5 days.

Lacrimal Gland Dacryolithiasis With Secondary Bacterial Dacryoadenitis: Clinicoradiological Characterization and Review of Literature.

We report a unique case of a 32-year-old woman with a 4-day history of painful right lacrimal gland swelling. MRI demonstrated enhancement and enlargement of the right lacrimal gland. There were nonenhancing, intraglandular foci, with low signal on T2-weighted sequences and marked restricted diffusion. Surgical excision revealed multiple dacryoliths centered on an impacted eyelash. Microbiological evaluation yielded Streptococcus pneumoniae and histopathology confirmed noncalcified ductal dacryoliths with a background of acute inflammation. At 1-month follow-up, symptoms completely resolved wi

Recurrent acute bilateral cicatricial conjunctivitis as an adverse event of systemic adalimumab for rheumatoid arthritis.

Anti-TNF-α therapies, though known for their anti-inflammatory effects, have been associated with paradoxical inflammatory reactions, of which anterior uveitis is the most common. We report a case of recurrent bilateral cicatricial conjunctivitis linked to systemic adalimumab (ADA) treatment in a 66-year-old man with rheumatoid arthritis (RA). The patient experienced worsening bilateral red eyes, blurry vision, and discomfort following each ADA injection. Examination revealed signs of cicatricial inflammation, which responded to corticosteroids but recurred after subsequent ADA doses. Despite

Chronic neuroinflammation after acute SARS-Cov-2 infection induces retinal damage in the hACE2 transgenic mouse model.

BACKGROUND: A number of patients infected with severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) show a wide range of systemic complications. Previous studies have shown that acute SARS-CoV-2 infection can be accompanied by conjunctivitis, various forms of ocular inflammation and retinal vasculitis. However, long-term changes of the retina after SARS-CoV-2 infection have not been examined. In this study, we investigated neuroinflammation in the retina and optic nerve. METHODS: hACE2 Tg mice, serologically negative for SARS-CoV-2, were infected via intranasal installation with SARS-

Polyprenyl immunostimulant treatment during experimental feline herpesvirus-1 infection is associated with reduced ocular disease after conjunctival rechallenge.

Polyprenyl immunostimulant (PI) was, at the time of this study, a USDA-licensed veterinary biologic for the treatment of feline rhinotracheitis. In two previous controlled challenge trials, PI administration reduced the severity of acute feline rhinotracheitis in specific pathogen-free (SPF) cats (1). We replicated the oral PI challenge-treatment protocol to evaluate humoral and virologic outcomes, measured by serum virus-neutralizing antibody titers and virus isolation, and to assess clinical ocular disease after conjunctival rechallenge. During the study, the frequency of virus isolation and

Contact Lens-Associated Ocular Surface and Corneal Disorders.

Contact lens wear is widely used for vision correction by millions of individuals worldwide; however, it remains associated with a spectrum of ocular complications ranging from mild inflammatory conditions to vision-threatening infections. Common contact lens-related complications are predominantly noninfectious, including contact lens discomfort, dry eye syndromes, and papillary conjunctivitis. These conditions are typically mild and manageable with conservative measures. In contrast, corneal inflammatory conditions, such as contact lens-induced acute red eye and peripheral ulcers, represent

Model-based Differentiation Between Kawasaki Disease and MIS-C: The Importance of the Mitral Valve.

BACKGROUND: Due to increasing proportion of asymptomatic severe acute respiratory syndrome coronavirus 2 infections and previously reported overlapping features, the distinction between Kawasaki disease (KD) and multisystem inflammatory syndrome in children (MIS-C) remains challenging. Statistical differentiation models for clinical guidance are scarce and should focus on cardiac involvement, as differences in cardiac involvement might serve as a potential clinical finding for early recognition of MIS-C. METHODS: We retrospectively analyzed 298 MIS-C and KD cases treated during the 22-year pe

Rural Presentation of Kawasaki Disease with Leukopenia and Transaminitis.

Kawasaki disease (KD) is an acute, self-limited vasculitis that primarily affects young children, and early treatment is essential to prevent coronary artery complications. Diagnosis is clinical and can be challenging in children who present atypically or receive initial evaluation in resource-limited settings. We report a case of KD in a 6-year-old girl who presented to a rural clinic with six days of fever, a progressive polymorphous rash, bilateral conjunctivitis, and mucocutaneous changes. Her evaluation showed leukopenia, marked transaminitis, elevated inflammatory markers, and an extensi

Case Report: Bullous erythema multiforme induced by omalizumab.

Bullous erythema multiforme (BEM) is an acute immune-mediated mucocutaneous disease, with drug induction being one of its common causes. Omalizumab, a recombinant humanized IgG1 monoclonal antibody targeting free IgE, is widely used in chronic spontaneous urticaria and asthma due to its favorable safety profile, while severe cutaneous bullous adverse reactions induced by it are extremely rare. We report a 36-year-old female patient with a history of bronchial asthma, allergic rhinitis, allergic conjunctivitis, and chronic spontaneous urticaria who developed generalized wheals. After the first

Models Estimating the Effects of Changes in the Concentration of Specific Climatic Pollutants and Temperature on the Prevalence of Acute Ophthalmic Inflammatory Conditions: A Retrospective Cross-Sectional Observational Study.

BACKGROUND: Air pollution is a major environmental health risk linked to systemic and ocular diseases, including conjunctivitis, keratitis, and dry eye disease. Pollutants such as particulate matter, nitrogen oxides, and diesel exhaust particles have been associated with an increased prevalence of acute ophthalmic inflammatory conditions (AOICs). Identifying key pollutants may help reduce the prevalence of AOICs. AIM: The aim of this study was to investigate the relationship between atmospheric pollution and AOICs by identifying pollutants with the most consistent impact on AOIC prevalence, d

Molecular characterization of chikungunya viruses associated with outbreaks in Kenya from 2017 to 2020.

Emerging outbreaks of the arbovirus chikungunya virus affect millions of individuals globally, causing debilitating arthralgia and fever. A study on the burden and etiologies of acute febrile illnesses (AFI) in Kenya reported an increase in CHIKV cases in Mombasa between December 2017 and December 2019. In January and February 2020, another outbreak of CHIKV occurred in Dadaab-Hagadera. Using next-generation sequencing on the Illumina MiSeq platform, we established molecular characteristics and phylogenetic differences of the CHIKVs collected from the two epidemiologically distinct settings. Forecasting Acute Hemorrhagic Conjunctivitis Incidence in Henan, China: A Comparative Study of Seasonal Autoregressive Fractionally Integrated Moving Average and Seasonal Autoregressive Integrated Moving Average Models.

BACKGROUND: Acute hemorrhagic conjunctivitis (AHC) is a highly contagious viral disease causing significant public health burden. Accurate forecasting is essential for timely intervention. The seasonal autoregressive integrated moving average (SARIMA) model is widely used, but the seasonal autoregressive fractionally integrated moving average (SARFIMA) model, which captures long-range dependence, may perform better for complex incidence series. This study compared the forecasting performance of SARFIMA and SARIMA models for AHC in Henan. METHODS: Monthly AHC case data from January 2011 to Jun

Genomic epidemiology of coxsackievirus A24 variant during the 2024 acute hemorrhagic conjunctivitis outbreak in Coastal Kenya.

Several African countries experienced a surge in acute hemorrhagic conjunctivitis (AHC) cases in 2024. Investigations in Kenya, Mayotte (an Indian Ocean island) and Tanzania identified coxsackievirus A24 variant (CVA24v) as the causative agent. To date, however, limited genomic data exist to elucidate the sources, epidemiology, and evolution of CVA24v in Africa. We generated 245 CVA24v genomes from samples collected between January and September 2024 in coastal Kenya, representing the largest outbreak CVA24v genomic data set available globally. Phylogenetic analysis showed that these viruses b

Potential alternatives in treatment of kawasaki disease to reduce the extremely rare risk of reye syndrome.

Background/objective: Kawasaki disease is an acute, systemic vasculitis most commonly observed in young Asian children. While the exact pathogenesis remains unknown, it is thought to be triggered by a pathogenic exposure that elicits an inflammatory response several days to 2 weeks before the onset of symptoms. Symptoms include edematous hands and feet, conjunctivitis, mucositis, a diffuse erythematous rash, lymphadenopathy, and a fever lasting for at least 5 days, which is required for diagnosis. In addition, coronary vessels may exhibit aneurysms, thrombosis, and stenosis. Without treatment,
