Establishing a filarial clinical research platform in a resource-limited setting: Lessons and experiences from Tanzania.

This paper examines the establishment and lessons learned from the LeDoxy Trial, a multinational, double-blind, randomized, placebo-controlled study that evaluated doxycycline (200 mg/d and 100 mg/d) over six weeks for the treatment of filarial lymphedema in Tanzania's Lindi and Pwani regions. Conducted from 2018 to 2021 across four sites, the trial enrolled 420 participants aged 14-65 years, addressing a critical gap in lymphatic filariasis (LF) research in geographically remote and historically under-researched areas. The trial faced significant challenges, including limited pre-existing res

Host, vector, and parasite dynamics: exploring intrinsic and extrinsic factors shaping tick-borne filarial nematode transmission.

Tick-borne filarial nematodes are a complex and understudied group of parasites that rely on ticks for transmission in vertebrates. This review examines how intrinsic and extrinsic factors may influence the successful transmission of filarial nematodes in tick vectors, drawing insights from extensively studied haematophagous dipteran vector systems. We explore how different tick life stages contribute to potential nematode transmission to the host and the influence of abiotic factors on nematode survival and development within ticks. Understanding nematode life stage periodicity and enhancemen

Mosquito (Diptera: Culicidae) surveillance for Dirofilaria immitis (Rhabditida: Onchocercidae) using a zoo as a focus for operational detection in central Utah.

Across the United States, Dirofilaria immitis (Leidy), commonly known as dog heartworm, poses a significant threat to common pets, including dogs, ferrets, and cats, but also to exotic and feral animals. This study focused on surveillance of D. immitis in mosquitoes collected in the Salt Lake Valley of Utah, with an emphasis on animals housed at a local zoo. Mosquitoes (Diptera: Culicidae) from the genera Culex, Culiseta, and Aedes were collected, identified, sorted, and subjected to DNA extraction. In the summer of 2023, extracted DNA from 320 pooled samples of 3,459 mosquitoes was tested by

The economic burden of loiasis: A comprehensive cost-of-illness analysis of regionally representative, individual-level data from rural Gabon.

Loiasis is a vector-borne filarial infection endemic to parts of sub-Saharan Africa. It disproportionally affects economically disadvantaged communities in rural, forested regions. To better understand the economic burden of loiasis, we conducted a comprehensive cost-of-illness study in an endemic region of Gabon, with the aim of quantifying the financial costs incurred by individuals infected with the disease from a societal perspective. We conducted a cross-sectional survey in 2023 in rural Gabon. Study participants took part in diagnostic testing for loiasis and were interviewed based on a

The impact of the termination of Lymphatic Filariasis mass drug administration on Soil-transmitted Helminth prevalence in school children in Malawi.

BACKGROUND: Soil-transmitted helminths (STH) have been passively targeted through the implementation of mass drug administration (MDA), with the drugs ivermectin and albendazole, against the parasitic disease, lymphatic filariasis (LF). In Malawi, LF MDA was administered to communities between 2008 and 2014. The aim of this analysis was to estimate the impact of LF MDA and its termination on STH prevalence in school aged children (SAC). METHODOLOGY: School survey data of STH prevalence in Malawi were obtained through the ESPEN website. The surveys spanned the periods before (1998-2004), durin

Exploring potential pathogenesis mechanisms of onchocerciasis-associated epilepsy through cerebrospinal fluid analysis of children with complex febrile seizures: a pilot study in Aketi, Democratic Republic of the Congo.

INTRODUCTION: Onchocerciasis, commonly known as river blindness, is a parasitic disease caused by Onchocerca volvulus affecting millions predominantly in sub-Saharan Africa. Robust epidemiological evidence points to a clinical relationship between onchocerciasis and epilepsy, a condition termed onchocerciasis-associated epilepsy (OAE). Despite extensive research and various successful elimination programmes over the past decades, the pathogenesis of OAE is still unknown. Current hypotheses propose that O. volvulus microfilaria, their excretory-secretory products or the newly discovered filaria

High-Dose Rifampicin Plus Albendazole Rapidly Clears Lymphatic Filariasis Circulating Filarial Antigen in a Randomised Clinical Trial: A Promising Step Toward Short-Course Macrofilaricidal Therapy.

Background: The lack of a short-course of safe and effective macrofilaricidal therapy for lymphatic filariasis (LF) hinders elimination efforts, especially in the endgame scenario. Preclinical studies in mice demonstrated that high-dose rifampicin (RIF) plus albendazole (ALB) produced macrofilaricidal effects within seven days, prompting this randomised, open-label, parallel-group, interventional phase II pilot trial to determine the efficacy of high-dose RIF plus ALB against LF in humans. Methods: In three LF-endemic districts of Ghana's Upper East Region, circulating filarial antigen (CFA)-p

Transient Intraperitoneal Residence of Dirofilaria immitis Larvae in the Mongolian Gerbil (Meriones unguiculatus).

Understanding the determinants of host specificity in Dirofilaria immitis can be advanced through the use of the nonpermissive Mongolian gerbil (jird) model. We hypothesized that host immunity dictates D. immitis establishment following the third larval molt. Jirds were infected intraperitoneally with 100 Brugia malayi (permissive control) or D. immitis third-stage larvae (L3). Necropsies occurred at 1, 3, 10, and 36 days post infection (dpi) to quantify larvae via peritoneal lavage. Initial recovery at 1 dpi showed 37.4% for B. malayi but only 0.4% for D. immitis (p < 0.0001). Dirofilaria imm

Cost-Effectiveness of Triple Versus Dual-Drug Therapy for Elimination of Lymphatic Filariasis in India.

OBJECTIVES: Mass Drug Administration programs are crucial for eliminating lymphatic filariasis (LF) in India, where it remains a significant public health challenge. Shifting from the existing dual-drug regimen (diethylcarbamazine, albendazole [DA]) to a triple-drug regimen (ivermectin, diethylcarbamazine, and albendazole [IDA]) aims to accelerate LF elimination. This study aims to evaluate the cost-effectiveness of the IDA compared with the DA, providing insights for policy and resource allocation in LF elimination. METHODS: We conducted a cost-effectiveness analysis to compare the health sy

Unveiling filariid infections in dogs living in the Western Amazon, Brazil.

Dirofilaria immitis and Dirofilaria repens, zoonotic vector-borne helminths of the family Onchocercidae, primarily cause cardiopulmonary and subcutaneous dirofilariosis in dogs, respectively. In the Western Amazon region of Brazil, information on the prevalence of Dirofilaria spp. remains limited, despite environmental conditions such as deforestation, high humidity, and a growing population of stray dogs and cats that may favor vector expansion and parasite transmission. However, the underuse of molecular tools may contribute to both low detection and misidentification of filariid species. Th

A preliminary survey of feline filarial parasites in Kerala State, India.

Feline filarial parasites remain among the least investigated in India, despite the well-established endemicity of filariosis in canine and human populations. This study aimed to assess filarial infections in domestic cats in Kerala using both microscopy and molecular techniques, with a focus on evaluating their potential role as reservoir hosts. From September 2023 to February 2024, 300 owned cats presented to the University Veterinary Hospital, Mannuthy, were screened for circulating microfilariae using wet blood film examination. Four cats (1.33%) tested positive, of which three harbored sh

Current issues in heartworm chemotherapy.

Several issues that affect prevention and treatment of heartworm infections require more intensive research. The incidence of heartworm infection in the USA is increasing, but the factors that underlie this trend remain incompletely understood. The contributions of climate change, vector range expansion, client compliance and resistance to macrocyclic lactones (ML) are likely interrelated and require investigation. Molecular-level research has not yet identified the causative mechanisms underlying ML resistance (MLR), but surveys of genomic markers associated with the trait reveal worrying tre

Cross-sectional study to assess filarial infection among the never treated individuals in selected districts in India: a study protocol.

INTRODUCTION: With the global lymphatic filariasis (LF) elimination goal set to 2030, it is necessary to address challenges hindering the last-mile efforts. Never treated individuals are those who self-report that they have never taken the drugs for LF during any mass drug administration (MDA) rounds. Hence, it is necessary to identify these individuals and assess if they can be potential reservoirs of infection and understand the reasons for non-compliance. METHODS AND ANALYSIS: This mixed method study, proposed for a period of 2 years, will assess the filarial infection status of never trea

Whole-genome sequence of a female Loa loa adult worm from Cameroon.

OBJECTIVE: Although loiasis affects more than 20 million people and that its associated clinical complications can cause multiple organ failures, this disease remains understudied. Consequently, the elimination of loiasis is not on the agenda of public health services. Defining an effective strategy to tackle this disease is challenging and requires a better understanding of the parasite's biology, including its genetic aspects. Loa loa adult worms provide large amounts of biological tissues for genetic sequencing, but are very difficult to collect. Therefore, limited genomic data are availabl

Illuminating the cytological intricacies of isolated filarial epitrochlear lymphadenitis: a case series of a seldom-seen clinicopathological occurrence with a review of the literature.

BACKGROUND: Lymphatic filariasis is a major parasitic disease caused by Wuchereria bancrofti, Brugia malayi, and Brugia timori, with India contributing a significant proportion of global cases. Although lymphadenitis commonly involves inguinal, axillary, or cervical nodes, isolated epitrochlear lymphadenopathy is exceptionally rare and can be mistaken for other infectious or neoplastic causes. Fine-needle aspiration cytology (FNAC) offers a rapid, cost-effective method for diagnosing filarial infections, especially when peripheral blood smears are negative.
