Lymphatic filariasis (LF) remains a significant public health challenge in many tropical regions where the disease is endemic. In Malaysia, LF is found in small pockets across the country. Asymptomatic carriers play a critical role in transmission but are often undetected. This report details an investigation of an asymptomatic filariasis reported by local health authorities involving an 83-year-old female patient residing in the Bako area, Sarawak. Despite being immobile due to a stroke, routine screening identified an infection with Brugia malayi through microscopy and a rapid diagnostic tes

BACKGROUND: Lymphatic filariasis (LF) is a significant neglected tropical disease in the Democratic Republic of the Congo (DRC). Although progress has been made toward interrupting transmission through mass drug administration, LF-related morbidities-particularly hydrocele, lymphoedema, and recurrent acute dermatolymphangioadenitis-continue to cause substantial disability, comorbid infections, and reduced quality of life. National evidence on the burden of LF morbidities and factors associated with access to morbidity management remains limited.
METHODS: We conducted a national retrospective

BACKGROUND: Onchocerciasis and loiasis are co-endemic filarial neglected tropical diseases in Central and West Africa. While ivermectin-based mass drug administration has reduced onchocerciasis burden, it can trigger severe neurological adverse events in individuals with high Loa loa microfilaraemia. This limitation highlights the urgent need for safe macrofilaricidal therapies and appropriate preclinical models.
METHODOLOGY/PRINCIPAL FINDINGS: We conducted a systematic review following PRISMA guidelines to evaluate animal models used for preclinical drug screening in onchocerciasis and loias

The small Pacific island nation of Niue was validated by the WHO in 2016 as having eliminated lymphatic filariasis (LF) as a public health problem; however, no post-validation surveillance (PVS) has been conducted since. In 2025, LF-PVS was integrated into a near-census national WHO STEPwise non-communicable disease (NCD) risk factor survey, a cost-effective approach for estimating LF prevalence and assessing whether elimination had been sustained. Finger-prick blood samples were tested for LF antigen and antibodies; antigen-positive samples were screened for microfilariae. One participant was

Filarial parasites such as Brugia malayi and Dirofilaria immitis cause serious diseases in humans and animals, yet effective treatments that kill adult worms are limited and often toxic. We tested a new strategy that targets parasite mitochondria-the cell's energy source-using modified versions of two existing drugs, atovaquone (ATO) and honokiol (HNK). By attaching a mitochondrial targeting group, we created Mito-ATO and Mito-HNK, which enter parasite mitochondria more efficiently. These compounds were 10-50 times more potent than the original drugs, strongly inhibiting parasite movement and

In regions where Loa loa is coendemic with lymphatic filariasis and onchocerciasis, mass drug administration of ivermectin has posed a risk for serious adverse reactions in individuals with high microfilarial loads. To overcome this problem, easy-to-use point-of-care diagnostic tools are crucial for identifying individuals at risk in population-based control programs. The NTDscope is a device that uses smartphone components for video microscopy and may support decentralized L. loa microfilariae screening under field conditions. This study evaluated the usability and operational feasibility of

BACKGROUND: Lymphatic filariasis (LF) is a neglected tropical disease in some parts of India and results in chronic lymphedema, hydrocele and acute inflammatory episodes. Even outreach to communities often yields undiagnosed cases of chronic cases needing confirmatory testing and tied-in care. This case illustrates the confluence between chronic infectious and metabolic diseases in rural India, and the requirement of comprehensive primary care.
CASE REPORT: A 75-year-old South Asian male farmer with type-2 diabetes mellitus detected at a rural camp in the community from rural Kayar, Yavatmal

Mansonella streptocerca is a species of neglected skin-dwelling filarial nematode parasite with scarce epidemiologic data from Central Africa. We conducted a cross-sectional survey of 1,007 adults from 51 rural and semiurban communities in Gabon to update prevalence estimates and identify risk factors. Molecular analyses by quantitative PCR detected filarial DNA in 18.3% of skin snips; M. streptocerca predominated (14.2%), and Onchocerca volvulus (3.4%) occurred focally in a single rural area. Blood-dwelling parasite species such as Loa loa, M. perstans, and Mansonella sp. "DEUX" were rarely d

Dirofilaria nematodes, a common cause of canine filarial disease, are increasingly recognized as emerging human pathogens. We report a case of human pulmonary dirofilariasis in the lung of a man from Northern Australia with pulmonary adenocarcinoma. This case highlights the risk for zoonotic transmission in regions with high canine heartworm prevalence.

The Amazon region faces persistent structural limitations for the diagnosis of filarial diseases. This study aimed to develop and evaluate an artificial intelligence model based on convolutional neural networks to classify microscopic images according to the presence or absence of microfilariae. This was a technological, quantitative, and applied study in which blood samples from 43 dogs were collected in rural areas of Manaus, prepared on stained slides, and digitized using a webcam coupled to a microscope, generating 500 original images. The images were preprocessed, organized into binary cl

This study aimed to achieve consensus on the case definitions and clinical aspects of loiasis, a filarial parasite infection caused by Loa loa, in order to support the development of guidelines for diagnosis and clinical management and to provide harmonised definitions for future research and scientific communication. We applied the Delphi method. Statements on the definition of eyeworm, Calabar swelling, possible/probable/confirmed loiasis, occult loiasis, and the classification of loiasis clinical manifestations were submitted individually to a panel of 18 experts participating in the guidel

BACKGROUND: Dirofilaria spp. are parasitic filarial nematodes endemic worldwide that infect dogs but are also zoonotic. Traveling dogs may serve as carriers of zoonotic parasites, including Dirofilaria immitis, introducing them to new regions. However, the risks of importation from endemic areas are unknown.
METHODS: To study seroprevalence in travelling dogs, we first established the signal-to-positive (S/P) cutoffs for the DiroCHEK™ ELISA assay (Zoetis) by spectrophotometry at three wavelengths (490, 620, or 650 nm). We also validated the assay for testing serum pools using known D. immitis

BACKGROUND: Medicine donation programs have been central to neglected tropical disease (NTD) control, particularly for soil-transmitted helminths (STH), but growing concerns about sustainability, supply continuity, and country ownership highlight the need for new access models. A novel fixed-dose combination (FDC) of ivermectin and albendazole, recently endorsed by the European Medicines Agency, offers programmatic advantages over monotherapy. Unlike some existing treatments, the FDC may not be donated. This paper examines how the current drug donation landscape for STH shapes the prospects fo

A 79-year-old woman was found to have a slowly enlarging pulmonary nodule in the right upper lobe, and partial lung resection was performed for diagnostic and therapeutic purposes. Histopathological examination revealed fragments of a filarial worm within the nodule. The parasite showed features compatible with Dirofilaria immitis, including a thick multilayered cuticle, abundant somatic musculature, and prominent internal longitudinal ridges projecting into the lateral cords. However, the external cuticular surface showed localized ridge-like elevations, making distinction from Dirofilaria ur

BACKGROUND: Loiasis is a filarial disease caused by Loa loa, endemic to Central and West Africa. Cases are observed in migrants and occasionally travellers. Its diagnosis may be difficult due to the unspecific clinical picture and the high percentage of people who do not have microscopically detectable circulating microfilariae. We performed a landscape analysis of the laboratory-based diagnostic techniques available for loiasis and of their estimated sensitivity and specificity.
METHODS: We performed a systematic review of cross-sectional, cohort, case-control, diagnostic accuracy, and clini

Belitung district, Indonesia, is endemic for lymphatic filariasis (LF) caused by zoophilic Brugia malayi. The district received 5 rounds of mass drug administration (MDA) with diethycarbamazine and albendazole (DA) between 2006 and 2010 and passed three transmission assessment surveys (TAS). Post-TAS-3 surveillance in 2021 revealed a prevalence of microfilariae (Mf) of 2.1%. This led to district-wide resumption of MDA with co-administered ivermectin, diethylcarbamazine, and albendazole (IDA). The impact of two rounds of IDA MDA was assessed using a modified IDA Impact Survey design. At baselin

BACKGROUND: Previously, antifilarial antibody testing in the evaluation of returned travelers and immigrants to North America has relied on IgG- and IgG4-specific responses to crude filarial extracts (BmA), tests that suffer from significant cross-reactivity with Strongyloides and cannot distinguish among the infecting filarial species. Nevertheless, between 1992 and 2016 we have tested >11000 samples from providers across North America in whom a filarial infection was being considered; close to 20% were able to be definitively diagnosed with a filarial infection (based on IgG4 anti-BmA reacti

During an investigation of animals as reservoirs for the filarial parasite Brugia malayi, three molecularly undescribed filarial species were co-detected. Individual microfilariae (Mf) were isolated and analyzed from blood samples of crab-eating macaques (Macaca fascicularis) from Belitung, Indonesia, and from pet dogs and cats in Sabah, Malaysia. Among 163 macaques, 33 (20.2%) were positive for large Mf (mean length 498.9 µm) similar to Dirofilaria ('Belitung I'). One macaque was infected with small Mf (mean length 150.4 µm) ('Belitung II'), with a high density of 17,150 Mf/mL. In two cats co

Lymphatic filariasis is a vector-borne infectious disease, most commonly caused by Wuchereria bancrofti, that affects 120 million people. It invades the lymphatic system of humans, who are the definitive hosts. We compared the administration of doxycycline with intensified hygiene measures alone in patients diagnosed with bancroftian filariasis. We searched PubMed, Scopus, and Embase and included 12 randomized controlled trials. A random-effects model was used to calculate the mean difference (MD)/risk ratio (RR) and I2 for heterogeneity. The risk of bias and GRADE evaluated the certainty of e

The anthelmintics ivermectin, albendazole, and diethylcarbamazine are the backbone of mass drug administration (MDA) campaigns targeting human filariasis, yet their direct effects on parasites are still not fully defined or understood. The clinical effects of these drugs are stage dependent, resulting in effective clearance of circulating microfilariae but only limited activity against adult worms, a pattern that complicates disease surveillance and elimination efforts. Although molecular targets have been identified or proposed for some antifilarial drugs, their precise modes of action remain
