Imaging Features of Herpetic Interstitial Keratitis by Anterior Segment Optical Coherence Tomography.

PURPOSE: Herpes simplex virus (HSV) and varicella zoster virus (VZV) are known causes of chronic and recurrent interstitial keratitis. Determination of active corneal inflammation is important for appropriate management. This study aimed to investigate features of clinically active herpetic interstitial keratitis (HIK) by anterior segment optical coherence tomography (AS-OCT). METHODS: Twenty-seven patients with active HIK (17 with HSV, 10 with VZV) and AS-OCT imaging were retrospectively identified. Five patients also had stromal scarring (SS), presumably from prior HIK episodes. An addition

Herpes Zoster Ophthalmicus Complicated by Optic Neuritis: Diagnostic and Therapeutic Challenges.

Herpes zoster ophthalmicus (HZO) is caused by reactivation of the varicella-zoster virus (VZV) in the ophthalmic branch of the trigeminal nerve and can result in a range of ocular complications. One of the rare but serious sequelae of HZO is optic neuritis, which can lead to significant visual impairment within a few days without timely management. We present a case of an immunocompetent adolescent female who developed right-sided retrobulbar optic neuritis following an episode of HZO. Her condition was confirmed with magnetic resonance imaging (MRI), which showed optic nerve involvement. Mana

Multidermatomal Herpes Zoster Involving All Three Branches of the Trigeminal Nerve in an Immunocompetent Adult: A Case Report.

Background and Clinical Significance: Herpes zoster (HZ), caused by the reactivation of the latent Varicella-Zoster virus (VZV), typically is confined to a single dermatome in immunocompetent individuals. Thus, multidermatomal involvement, particularly simultaneous reactivation across all three branches of the trigeminal nerve, is exceedingly rare without history of immunosuppression. Case Presentation: We present the case of a 60-year-old immunocompetent male who presented to the Emergency Department with a two-day history of a rapidly progressive, painful vesicular eruption over the entire l

Performance of the Interleukin Score for IntraOcular Lymphoma Diagnosis Score in the Diagnosis of Vitreoretinal Lymphoma in Clinical Practice.

PURPOSE: To evaluate the real-world diagnostic performance of the Interleukin Score for IntraOcular Lymphoma Diagnosis (ISOLD) score-a probabilistic formula based on absolute intraocular interleukin-10 (IL-10) and IL-6 levels-and to compare it with absolute cytokine thresholds and the IL-10/IL-6 ratio for vitreoretinal lymphoma (VRL) diagnosis. Because the comparison group consists predominantly of patients who underwent anterior chamber paracentesis (ACP), this study primarily assesses ISOLD performance on aqueous humor (AH) samples while providing additional data on vitreous specimens. Ocular involvement, fever, and holocranial headache predict intracranial infection in immunocompetent patients with cranial herpes zoster: a multicenter propensity score-matched study.

BACKGROUND: Varicella-zoster virus (VZV) reactivation in the head and facial region can cause severe intracranial complications. Early identification of central nervous system (CNS) involvement is critical, yet specific bedside predictors to guide prompt antiviral escalation are lacking, frequently leading to diagnostic delays and poor neurological prognosis. OBJECTIVES: To develop and internally validate a clinical risk-stratification scoring system based on ocular involvement, systemic inflammation, and headache patterns to identify intracranial infection in head and facial herpes zoster (H

Case Report: Presumed ocular tuberculosis masquerading as acute retinal necrosis: the diagnostic and therapeutic journey of an elderly female.

BACKGROUND: Acute retinal necrosis (ARN) is typically caused by varicella-zoster or herpes simplex virus, but Mycobacterium tuberculosis can present an identical clinical picture. Distinguishing tuberculous uveitis (TBU) from ARN is challenging when etiological tests are unavailable. CASE PRESENTATION: A 78-year-old woman presented with rapid visual loss (hand motion) in the left eye. Examination revealed elevated intraocular pressure, mutton-fat keratic precipitates, severe anterior chamber and vitreous inflammation, retinal arteritis, and peripheral necrotic lesions, which met the 2015 Japa

Total Ophthalmoplegia as an Early Manifestation of Herpes Zoster Ophthalmicus in an Elderly Patient With Asthma: A Case Report.

Herpes zoster ophthalmicus (HZO) may cause ocular surface disease, but ocular motor cranial neuropathy and total ophthalmoplegia are rare. These neurological deficits generally develop after the cutaneous eruption, making their presence at initial presentation unusual. Asthma has recently gained attention as a risk factor for herpes zoster, potentially through Th2-skewed inflammation, relative Th1-mediated antiviral immune insufficiency, and impaired cellular control of latent varicella-zoster virus (VZV). We report a 71-year-old man with poorly controlled asthma who presented with a one-day h

Foveal-Onset Acute Retinal Necrosis Associated With Varicella-Zoster Virus and Delayed Peripheral Retinitis: A Case Report.

Acute retinal necrosis (ARN) usually begins in the peripheral retina, but atypical cases may initially involve the posterior pole. A 63-year-old man with no evidence of immunodeficiency presented with decreased vision and ocular hypertension in the left eye. The earliest optical coherence tomography (OCT) image showed a lesion centered on the fovea. At a subsequent examination, a whitish foveal lesion was associated with marked hyperreflectivity involving nearly the entire inner retina, posterior shadowing, and ellipsoid zone disruption. Dilated peripheral retinal examination and widefield fun

Adjunctive Varicella-Zoster Virus-Specific Immunoglobulin for Complicated Varicella-Zoster Virus Infection in Kidney Transplant Recipients: Case Series.

BACKGROUND: Kidney transplant recipients are at increased risk of severe varicella-zoster virus (VZV) infection due to chronic immunosuppression. While antiviral therapy is the mainstay of treatment, the role of adjunctive VZV-specific intravenous immunoglobulin (VZV-IVIG) in complicated disease remains insufficiently defined. METHODS: We present a case series of 3 kidney transplant recipients with complicated VZV infection treated with VZV-IVIG in addition to standard antiviral therapy at a tertiary transplant center between 2024 and 2025. Clinical presentation, virological course, immunosup

Reactivation of Varicella-Zoster Virus Keratitis after Recombinant Herpes Zoster Vaccination: A Case Report and Literature Review.

INTRODUCTION: Varicella-zoster virus (VZV) keratitis following recombinant herpes zoster (HZ) vaccination is rare, and its underlying mechanisms remain poorly understood. Sporadic cases of ocular inflammatory reactivation have been reported after vaccination, particularly in patients with a history of HZ infection. CASE PRESENTATION: A 68-year-old woman with a history of trigeminal HZ without prior corneal involvement developed VZV keratitis 2 months after receiving the recombinant HZ vaccine (Shingrix®). The condition progressed to neurotrophic keratitis, requiring advanced ocular surface th

Recurrent unilateral anterior uveitis in an HLA-B27-positive ankylosing spondylitis patient: varicella-zoster virus etiology case report.

BACKGROUND: A 28-year-old Chinese woman with human leukocyte antigen (HLA)-B27-positive ankylosing spondylitis (AS) for 5 years experienced recurrent left-eye blurred vision and redness for 4 years (2-3 episodes/year). This was previously attributed to AS without etiologic testing. Her father and brother had AS but without ocular involvement. The patient received no systemic disease-modifying therapy for AS throughout her clinical course. METHODS: Ophthalmic examinations [visual acuity, intraocular pressure (IOP), slit-lamp, and dilated fundus] and 100-µL left-eye aqueous humor testing [via p

Severe Eczema Herpeticum With Ocular Involvement Leading to Orbital Cellulitis: A Pediatric Case Report.

Eczema herpeticum (EH) is a superimposed cutaneous viral infection that occurs in the context of preexisting dermatoses, most commonly atopic dermatitis (AD). It typically involves the face, neck, and upper trunk. Although ocular involvement is relatively uncommon, it may present diagnostic and therapeutic challenges and pose a significant risk to vision. We report a pediatric case of EH with severe ocular involvement in a young child with underlying AD. Despite early oral antiviral and antibiotic therapy, the condition progressed rapidly to orbital cellulitis. Clinical improvement was observe

Identification of pathogenic fungi causing ocular infections using full rRNA operon sequencing with Oxford Nanopore Technologies.

Although fungal eye infections are a major cause of visual impairment worldwide, standard clinical laboratory methods remain slow, insensitive, and limited in their taxonomic resolution. Sequencing of the full ribosomal RNA (rRNA) operon provides a comprehensive marker for fungal identification. In this study, twenty fungal isolates associated with ocular infections were obtained from Srinagarind Hospital, Thailand, and characterized using four identification approaches. Initial hospital-based routine identification relied on conventional morphological methods and matrix-assisted laser desorpt

Beyond the Kidney and Lung: Cutaneous and Mucosal Clues to Human Hantavirus Disease.

Hantaviruses are rodent-borne viruses that cause two overlapping vascular syndromes in humans: hemorrhagic fever with renal syndrome, including nephropathia epidemica, and hantavirus pulmonary/cardiopulmonary syndrome. Although dermatologists are rarely central to hantavirus care, early visible abnormalities may occur on the skin, mucosa, and ocular surface. This narrative review synthesizes the dermatologic and mucocutaneous manifestations of human hantavirus disease, with emphasis on signs that should prompt diagnostic suspicion in dermatology, emergency, travel-medicine, and infectious-dise
