Symptom overlap in chikungunya and dengue: A diagnostic challenge during outbreaks.

Distinguishing dengue from chikungunya at presentation is difficult in co-circulation settings due to substantial symptom overlap. In November 2023, São José do Rio Preto, São Paulo, Brazil, a dengue-endemic city, faced its first recognized chikungunya outbreak, challenging frontline triage and surveillance. We conducted a hospital-based retrospective cross-sectional study (November 2023-August 2024), integrating clinical variables with RT-qPCR confirmation to compare syndromic profiles and severity markers between dengue and chikungunya. Neurological cases were captured through active surveil

SEC31A promotes gastric cancer progression by inducing the PI3K/AKT/mTOR pathway and enhancing the CCL5‑mediated M2 polarization and recruitment of tumor‑associated macrophages.

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is characterized by respiratory and extrarespiratory components referring both to systemic complications of COPD, like skeletal muscle myopathy, weight loss and others, and frequently associated comorbidities, interesting various organs and systems (cardiovascular diseases, malignancies, osteoporosis, diabetes, etc.). These comorbidities may increase the rate of hospitalization of COPD patients and have a huge effect on the outcomes of the respiratory disease. Inhalation therapy of COPD with bronchodilators and steroid is primary driven

Post-chikungunya retinitis leading to optic atrophy in a toddler: A rare cause of acute vision loss.

Chikungunya fever is a mosquito-borne viral illness that is increasingly recognized for its atypical and multi-organ involvement. Although self-limiting, it can affect multiple organ systems, including the nervous and ocular systems. Ocular manifestations, particularly retinitis with retinal detachment, are vision-threatening. Recent outbreaks in endemic regions have demonstrated an expanding clinical spectrum, with increasing reports of severe and atypical complications. We report a pediatric case presented with chikungunya infection and concurrent UTI with septicemia. Visual impairment was n

Rift Valley fever: An emerging zoonotic threat to global health and agriculture.

Rift Valley fever (RVF) is an acute, mosquito-borne zoonotic disease caused by the RVF virus (RVFV), which belongs to the Phlebovirus genus of the Bunyaviridae family. The infection mainly targets domestic ruminants such as sheep, goats, cattle, and camels, with sheep showing the highest vulnerability. The virus is spread predominantly through the bites of Aedes and Culex mosquitoes, although humans may also become infected by direct exposure to blood, organs, or secretions of diseased animals. In livestock, the illness is marked by sudden fever, ocular and nasal discharges, vomiting, and swol

Herpetic panophthalmitis in a patient with a history of neonatal herpetic encephalitis.

Herpetic acute retinal necrosis (ARN) with orbital inflammation is a rare form of panophthalmitis. We report the case of a 15-year-old boy with hypoxic ischemic encephalopathy (HIE) secondary to neonatal herpetic encephalitis who developed unilateral ocular inflammation and proptosis. Imaging and aqueous PCR confirmed orbital inflammation and active HSV infection. Given the patient's comorbidities, aggressive surgical treatment was not pursued. Treatment aimed at protecting the contralateral eye from involvement requires life-long antiviral prophylaxis.

Evolution-guided prioritization identifies a tissue-specific phosphorylation switch on herpes simplex virus 1 UL7 regulating viral replication and pathogenicity.

UNLABELLED: Although numerous phosphorylation sites on herpes simplex virus 1 (HSV-1) proteins have been identified through phosphoproteomic analyses, their virological significance remains largely unclear. Here, we developed an evolution-guided prioritization strategy that integrates phosphoproteomic data with residue-level conservation across the genus Simplexvirus to identify phosphorylation sites as candidates for functional relevance. Applying this strategy to a previously reported phosphoproteomic data set, we identified 46 phosphorylation sites with a conservation level of 88% or higher

Highly pathogenic avian influenza A(H5N1) in domestic cats: epidemiology, clinical manifestations, and One Health implication.

Historically considered accidental hosts of influenza A viruses, domestic cats (Felis catus) have gained attention during the global expansion of highly pathogenic avian influenza (HPAI) H5N1 clade 2.3.4.4b. In addition to traditional exposure via predation on infected wild birds, recent outbreaks have highlighted novel transmission routes, including contaminated commercial raw poultry products and unpasteurised milk. The susceptibility of cats has been linked to the abundant distribution of α-2,3-linked sialic acid receptors in both the respiratory tract and the central nervous system, while

Multisystem Burden of Paediatric Systemic Lupus Erythematosus: A Systematic Review and Meta-Analysis of Neuropsychiatric, Renal, Ocular, and Immunological Outcomes.

BACKGROUND: Paediatric systemic lupus erythematosus (pSLE) is associated with more aggressive disease and higher rates of organ involvement than adult-onset disease. However, the relationship between disease activity and multisystem manifestations has not been comprehensively quantified. METHODS: A systematic review and meta-analysis were conducted following PRISMA 2020 guidelines. Five databases (PubMed, Embase, Scopus, Web of Science, and Cochrane Library) were searched for observational studies published between 2015 and 2024. Central nervous system involvement of hantaviruses: clinical presentations, neuroimaging findings, and current implications.

Because of their involvement in outbreaks, hantaviruses as emerging zoonotic diseases have been regarded a public health issue. Recent reports have identified CNS symptoms of hantavirus infection, although the data are still relatively sparse and inconsistent, especially for Andes orthohantavirus (ANDV) and to a lesser extent Puumala virus (PUUV). Reported neurological features include headache, dizziness, altered mental state, seizures, focal deficits, and peripheral neuropathy, possibly accompanied by ocular symptoms such as impaired vision and photophobia. The results of neuroimaging have b

Nipah Virus: A Neurotropic Zoonosis-From Molecular Biology to Clinical Manifestations and Translational Advances.

Nipah virus (NiV) is a single-stranded negative-sense RNA virus of the genus Henipavirus, family Paramyxoviridae. The World Health Organization has declared NiV a priority pathogen with a great public health risk due to its epidemic potential and lack of sufficient countermeasures. In humans, the virus causes Nipah viral disease (NVD), characterized by acute respiratory failure and/or encephalitis. Long-term assessments of NVD survivors have revealed persistent neurological dysfunction, including encephalopathy, cerebral atrophy, hyperintensities, ocular motor palsies, and dystonia. The entry

Congenital HSV-2 infection with atypical skin lesions, chorioretinitis, and neurodevelopmental sequelae: a case report.

BACKGROUND: Neonatal herpes simplex virus (HSV) infection, primarily caused by HSV-2, carries a significant risk of severe morbidity, including neurological impairment and developmental delays, without timely diagnosis and intervention. The early identification of atypical cutaneous manifestations at birth and the prompt empirical initiation of high-dose intravenous acyclovir in suspected cases are crucial to prevent systemic dissemination and improve outcomes. Nevertheless, due to the unique neonatal immune response and limitations of current therapies, significant long-term sequelae such as

Ocular ultrasound lowers threshold for lumbar puncture in varicella zoster virus meningitis: A case report.

Headache is a common presenting concern among patients presenting to the emergency department, with etiologies ranging from benign primary headaches to potentially life-threatening intracranial infection, hemorrhage, or mass lesions. Ocular ultrasound can rapidly and noninvasively detect papilledema, making it a valuable first-line tool for guiding diagnosis and management decisions. We describe a case of an otherwise healthy 37-year-old female who presented to the emergency department with a five-day history of headache. She had previously been evaluated at two other emergency departments, wh

mTORC2-dependent autophagy inhibition regulates the replication of HSV-1 and adenovirus in viral keratitis & conjunctivitis.

Viral keratitis & conjunctivitis result in multiple ophthalmic symptoms and even progress to vision loss without timely intervention. Although multitudinous pathogens can cause ocular infections, the regulatory mechanisms underlying virus-host interactions remain incompletely defined. Our clinical and mechanistic investigations identify the co-infection of herpes simplex virus type 1 (HSV-1) and adenovirus as a predominant etiology of viral keratoconjunctivitis in Shenzhen, China (2024). The viral co-infection causes both severe symptoms and inflammations in clinical cases and in vitro. Mechan

Accuracy and completeness of large language models in Epidemic keratoconjunctivitis Queries: A Comparative study.

BACKGROUND: Large language models (LLMs) are increasingly applied in clinical contexts, yet their reliability in disease-specific ophthalmic domains remains insufficiently characterized. Epidemic keratoconjunctivitis (EKC), a highly contagious adenoviral ocular infection, represents a critical area where accurate diagnostic and therapeutic information is essential. This study aimed to assess and compare the accuracy and completeness of responses generated by two widely used LLMs, namely ChatGPT and Meta AI, to standardized ophthalmology questions related to EKC. METHODS: This cross-sectional

High pathogenicity of human adenovirus type 55 in adult males: a meta-analysis and phylogenetic study of HAdV-55 epidemics worldwide over the past six decades.

Human adenovirus type 55 (HAdV-55), formerly designated as HAdV-11a, is a notable recombinant pathogen linked to severe respiratory infections. Since its reclassification as HAdV-55 in 2010, numerous outbreaks of pneumonia associated with it have been documented over the past decade. We conducted a systematic review of 111 peer-reviewed articles from PubMed, Web of Science, and China National Knowledge Infrastructure (CNKI) spanning the 60 years (1965-2024). Furthermore, we performed phylogenetic analysis on 87 complete HAdV-55 genomes available in GenBank. Bayesian methods were employed to es

AdamiForge: a modular reverse genetics tool for human adenovirus type 12 (HAdV-A12).

Human adenoviruses (HAdVs) are a large family of viruses comprising more than 100 genotypes distributed across seven species (A to G). Depending on the genotype, they cause respiratory, ocular, urinary tract, or gastrointestinal infections. Despite the considerable diversity within the human adenovirus genus, most of our current knowledge comes from studies on HAdV-C5. The availability of experimental tools, including antibodies and reverse genetics systems, for the study of adenoviruses other than the C5 strain remains very limited. Here, we describe a reverse genetics tool to engineer HAdV-A

Applications of genome editing technologies in the treatment of human diseases.

Genome editing has progressed from a laboratory capability for targeted DNA manipulation to a clinically relevant strategy for correcting, silencing, or regulating genes implicated in human disease. In this Review, we synthesize the mechanisms, capabilities, and constraints of the principal programmable platforms-zinc-finger nucleases (ZFNs), transcription activator-like effector nucleases (TALENs), and CRISPR-Cas systems-and highlight how base editors, prime editors, and epigenetic editors expand the range of achievable outcomes beyond double-strand break-dependent repair to precise nucleotid
