INTRODUCTION: Pyoderma gangrenosum is a rare form of neutrophilic dermatosis, with ocular involvement being atypical. We present a rare case of ulcerative colitis (UC) complicated by nodular scleritis and pyoderma gangrenosum, both occurring almost simultaneously.
CASE PRESENTATION: A 55-year-old man with active UC initially presented to our hospital with anterior diffuse scleritis and a peripheral corneal ulcer in his left eye. The condition rapidly progressed to bilateral nodular scleritis. Concurrently, multiple painful abscesses developed on his trunk and head. A skin biopsy confirmed the

A woman in her fifties was presented to the emergency room with eye pain, swelling, redness, and reduced vision. The examination revealed periorbital swelling, limited extraocular motility, tenderness, and conjunctival chemosis, with thinning observed nasally at the site of prior pterygium repair. The anterior chamber was quiet, and B-scan (ultrasonography) revealed retino-choroidal thickening. Computed tomography (CT) scan results indicated septal cellulitis with clear sinuses. Laboratory workups were negative. The patient was diagnosed with orbital cellulitis and was admitted and treated acc

PURPOSE: The purpose of this study was to evaluate the role of anterior segment optical coherence tomography (ASOCT) in the diagnosis and management of anterior scleritis.
PATIENTS AND METHODS: In this retrospective study, we examined 58 eyes of 44 patients with anterior scleritis. The unaffected eye served as a control. In bilateral cases, the less affected eye was taken as the control. ASOCT image over the inflamed area and over the corresponding same area in the control eye was taken. The images were analysed for the presence or absence of hyporeflective areas, and the mean total scleral t

This study aims to provide an update on infectious scleritis (IS). A comprehensive search was conducted using Google Scholar, Scopus, and PubMed. IS is a rare but critical condition that presents significant diagnostic challenges and often leads to poor visual outcomes. It accounts for approximately 5%-10% of scleritis cases and warrants significant attention due to its delayed diagnosis. Ocular surgery is the primary contributor to IS, accounting for a substantial proportion of cases ranging from 58% to 83%, followed by trauma contributing to approximately 10% of cases. IS can be caused by va

A wide range of systemic and ocular symptoms have been associated with 2019 novel coronavirus (COVID-19) infection, including long-term alterations in inflammatory pathways. We describe a novel case of an 11-year-old female presenting with posterior scleritis and associated serous retinal detachment, occurring one year after confirmed COVID-19 infection. The patient presented with a one-day history of central scotoma following three days of progressive right eye pain, photophobia, conjunctival injection, and blurred vision. She reported two prior episodes of right eye pain, the first of which

PURPOSE: To evaluate outcomes of and risk factors for cataract surgery in patients with scleritis.
METHODS: This retrospective cohort study evaluated demographic and ocular features of a surgical cohort of IRIS Registry (Intelligent Research in Sight) patients with scleritis undergoing cataract surgery between 2013 and 2019. Multiple logistic regression was used to identify factors associated with poor postoperative best visual acuity (BVA) in the surgical cohort and to identify risk factors for undergoing cataract surgery by comparing the surgical cohort with a nonsurgical scleritis cohort.

PURPOSE: To report a case of chronic unilateral nodular anterior scleritis as a rare, isolated presentation of IgG4-related ophthalmic disease.
CASE: A fifty-two-year-old patient was evaluated for painful, nodular scleral injection of the right eye, diagnosed as nodular anterior scleritis. There was only a partial response to topical corticosteroid and systemic immunomodulatory therapy. This, in combination with a sharp delineation between normal and abnormal sclera lead to the decision to perform a diagnostic biopsy of the lesion. Histopathology showed an IgG4 lymphoplasmacytic infiltration

Scleritis is a rare, potentially sight-threatening, painful eye disease. Based on its anatomical involvement, it can be categorized as anterior and posterior scleritis. There are several possible causes, among which infectious and noninfectious origins should be considered. From the therapeutic aspect, it is important to clarify the infectious origin, to provide target treatment, or to identify the possible underlying autoimmune disease. Corticosteroid therapy is considered to be the basis for the stepwise treatment of scleritis. In this article, we describe the management of three patients (i

PURPOSE: Seasonal Hyperacute Panuveitis (SHAPU) is a severe, rapid-onset panuveitis primarily affecting children, often linked to the setae released in the air or by contact with female moths of the genus Gazalina (Lepidoptera, Notodontidae), or with their egg masses laid on various substrates. This study aims to report a rare case of SHAPU from the higher altitude of Nepal with an alpine climate, with concurrence of necrotising scleritis. To the best of our knowledge, this is the first report of SHAPU at high elevation.
CASE REPORT: A 27-year-trekker male from an alpine climatic region of ea

Scleral necrosis is a rare but severe complication caused by various etiologies. The main therapeutic approach is topical and systemic medical treatment. Surgical interventions may be indicated in unresponsive cases. These approaches, however, may fail to control the scleral necrosis. In addition, both medical and surgical treatment may lead to a number of ocular and systemic side effects, calling for noninvasive but effective treatment for the management of scleral necrosis. This review aims to summarize current studies investigating the role of topical erythropoietin in the treatment of scle

Choroidal metastasis from occult cutaneous melanoma is rare and can masquerade as ocular inflammation. A 70‑year‑old man with sectoral anterior scleritis was found on multimodal imaging to have a solitary choroidal mass with mild periscleral fluid, prompting systemic evaluation that uncovered colonic polyps that, on histopathology, contained metastatic melanoma, a scalp primary, and widespread visceral, nodal, and intracranial metastases. Tumour cells stained HMB‑45, Melan‑A, and SOX10 positive, AE1/AE3 negative, and carried an NRAS‑Q61 mutation with wild‑type BRAF, confirming cutaneous origin

PurposeTo report a case of fungal sclerouveitis caused by Alternaria doliconidium, misdiagnosed as autoimmune scleritis and incorrectly treated with immunosuppressive therapy.ObservationsA 69-year-old man presented with progressive chronic necrotizing scleritis in his right eye following ocular trauma. Initial treatment with antimicrobial and steroid therapy showed no improvement, and a scleral biopsy yielded inconclusive results. Autoimmune scleritis was suspected, leading to treatment with systemic steroids and immunosuppressants. Upon further examination, nodular necrotizing scleritis with
