VEXAS syndrome with eosinophilia and pathologically mimicking histiocytosis: a case report.

Vacuoles, E1 enzyme, X-linked, Autoinflammatory, Somatic (VEXAS) syndrome is a recently identified autoinflammatory disorder caused by somatic mutations in the UBA1 gene. This report describes the case of a 54-year-old Japanese man with VEXAS syndrome exhibiting atypical features of eosinophilia and histiocytoid changes that mimic histiocytosis. Initially, the patient presented with recurrent fever, eosinophilia, lymphadenopathy, polyarthritis, and a skin rash. Histopathological examination of the skin and lymph node biopsies revealed the infiltration of CD68-positive histiocytes, raising susp

Granulomatosis With Polyangiitis Presenting as a Rapidly Enlarging Choroidal Mass: A Case Report.

Granulomatosis with polyangiitis (GPA) is a necrotizing vasculitis that can involve virtually any organ system. Among ocular manifestations, choroidal involvement is exceedingly rare and may mimic intraocular tumors. We report the case of a 68-year-old woman who presented with floaters and a rapidly enlarging choroidal mass. Imaging with contrast-enhanced CT revealed a choroidal lesion, tracheal wall thickening, pulmonary nodules, and maxillary sinus mucosal hypertrophy. FDG-PET demonstrated uptake in these lesions and in the nasal septum. Biopsies of the nasal septum and bronchus showed plasm

Clinical profile and outcome of patients with ANCA-associated vasculitis with special reference to vasculitis damage index: a longitudinal observational study.

Antineutrophil cytoplasmic autoantibody-associated vasculitis (AAV) is the most commonly encountered primary small-vessel vasculitis, with an annual incidence of 20 per million. The data on AAV from the Southeast Asian ethnicity is scarce. The present longitudinal observational study was undertaken to determine the clinical profile and outcomes in fifty-eight consecutive patients of AAV attending the Department of Rheumatology in a tertiary care centre in India. The majority (39, 67.2%) were females with a mean (± SD) age at baseline of 39.2 (± 15.2) years. Granulomatosis with polyangiitis (GP

Posterior Scleritis in a Pediatric Patient with IgA Vasculitis.

PURPOSE: To report a boy with IgA vasculitis (IgAV), who suffered two episodes of Posterior Scleritis (PS). This is the first reported case of this association. METHODS: Case report based on medical record review. RESULTS: A 13-year-old boy with IgAV presented with left eye (LE) pain. Although the eye examination was unremarkable, Magnetic resonance imaging (MRI) revealed PS; appropriate treatment was initiated, resulting in an excellent response. Later, a similar episode occurred in the right eye (RE) accompanied by a violaceus papule on the tip of the nose. CONCLUSION: PS may be associate

An Episcleral Lesion in a Patient With Neurofibromatosis Type 1.

Neurofibroma can present in a myriad of body tissues, including, very rarely, in the sclera; a unique presentation that is described herein. We describe a case of a 21-year-old man with neurofibromatosis type 1, who presented with 2 weeks of a blue-gray scleral discoloration overlying a protrusion of the sclera. His presentation was initially concerning for scleritis; however, histopathological analysis revealed an epibulbar neurofibroma. Recognition of rare manifestations of neurofibromatosis is essential to properly diagnose and manage ocular findings in this condition.

Posterior scleritis in an 18-year-old female with primary familial brain calcification syndrome: Incidental finding or association?

Primary familial brain calcification (PFBC) is an idiopathic neurological disease associated with intracranial calcification. An 18-year-old female, a known case of uncontrolled Type 1 diabetes mellitus, presented with unilateral progressive diminution of vision in her left eye. Based on her clinical examination, a diagnosis of left eye posterior scleritis was made. Computed tomography of the brain revealed bilateral caudate and lentiform nuclei patchy calcifications suggestive of Fahr's syndrome or PFBC. In our patient, age and posterior scleritis were two uncommon presentations to be associa

Long-term remission of severe refractory scleritis associated with ulcerative colitis after proctocolectomy: a case report and literature review.

Extraintestinal manifestations occur in up to 50% of patients with inflammatory bowel disease and can affect various organs, including the joints, skin, and eyes. Scleritis, one such extraintestinal manifestation, is associated with severe eye pain, vision impairment, and potential permanent vision loss. However, it is rare, and detailed descriptions of its characteristics and management are limited. We report the case of a 55-year-old man initially diagnosed with proctitis ulcerative colitis who simultaneously developed scleritis as the disease progressed to left-sided colitis. Treatment with

Dramatic Inflammatory Regression of Choroidal Metastases from Renal Cell Carcinoma following Ipilimumab and Nivolumab Immunotherapy: A Case Series.

INTRODUCTION: Renal cell carcinoma (RCC) is a rare cause of ophthalmic metastasis. Immune checkpoint blockers (ICBs) such as ipilimumab and nivolumab (ipi/nivo) are first-line therapies for advanced RCC. There are limited efficacy reports of ICBs for RCC choroidal metastases (CMs). CASE ONE: A 43-year-old male with metastatic (lung) clear cell RCC presented with left eye scleritis and a 3.4 mm choroidal mass. One week after starting ipi/nivo, the lesion rapidly expanded to 11.9 mm with vitritis, subtotal exudative retinal detachment (ERD), and features of necrosis (heterogenous echogenicity).

VEXAS syndrome with cochleovestibular involvement.

We report a case of VEXAS syndrome in a 71-year-old man presenting with auricular chondritis, scleritis, hearing loss, and vestibular dysfunction. Although the clinical picture initially suggested relapsing polychondritis, hematologic findings and UBA1 variant confirmed VEXAS syndrome. Comprehensive audiovestibular testing revealed bilateral sensorineural hearing loss and global vestibular impairment. Steroid therapy led to partial improvement. This case highlights the potential for inner ear involvement in VEXAS syndrome, underscoring the importance of systematic audiovestibular evaluation an

Concordance between presenting features and relapse in granulomatosis with polyangiitis: implications for risk assessment and counselling.

OBJECTIVE: To investigate the concordance between organ involvement at diagnosis and relapse in granulomatosis with polyangiitis and factors associated with new disease features at relapse. METHODS: Data from a national database of newly diagnosed patients was analysed. Clinical features were recorded at diagnosis and relapse, grouped by organ system. ORs and HRs were used to assess associations between baseline features and first relapse. Factors independently associated with new organ involvement at relapse were identified using multivariable logistic regression. RESULTS: Among 795 patient

Non-necrotising Scleritis and Spherophakia in Marfan Syndrome: A Rare Situation Adhering to Hickam's Dictum.

Marfan syndrome is an autosomal dominant, connective tissue disorder that affects various systems of the body including the eyes. We present a case of a 22 year old man with Marfan syndrome presented with a painful red left eye. On examination he was diagnosed to have non-necrotising scleritis and microspherophakia. Non-necrotising scleritis with microspherophakia is a rare and arduous situation. This is an interesting case where Hickam's dictum prevails over its counter argument of Occam's razor principle.
