Sclerokeratitis after keratoplasty in atopy.

The authors report a series of five markedly atopic patients in whom a severe sclerokeratitis developed within 1 to 4 weeks of keratoplasty. The onset was acute with discomfort, photophobia, hyperemia, and mucus production. This resulted in early loosening of sutures and was associated with microbial keratitis in two cases and graft rejection in one. The inflammatory reaction was controlled with high-dose oral steroids and did not recur when the treatment was terminated. Serum IgE levels were elevated in all these patients (range, 421-8434 kU/l). Binding of this IgE onto the surface of mast ce

[Serum immunosuppressive acidic protein (IAP) in inflammatory ocular disease].

The titer of serum immunosuppressive acidic protein (IAP) was determined by the method of single radial immunodiffusion (Matsuda et al) in Behçet's disease, sarcoidosis, tuberculosis, toxoplasmosis, etiology-unknown anterior uveitis (granulomatous, non-granulomatous), allergic conjunctivitis and scleritis. The titer of IAP in these diseases except allergic conjunctivitis was significantly higher than in normal controls. In all the diseases studied, the titer during the active stage was significantly higher than that during the inactive stage. The titer in Behçet's disease was significantly hig

Subretinal fluid in the diagnosis of posterior scleritis.

The etiology of subretinal mass lesions is very difficult to establish. A 28-year-old man presented to us with an area of choroiditis, which progressed, despite corticosteroid and antibiotic therapy, to an exudative retinal detachment, secondary glaucoma and a painful blind eye. To develop a means of identifying the cause of such lesions, we did a subretinal fluid tap prior to enucleation. Cytopathology showed only inflammatory cells and the final histopathological diagnosis was that of a granulomatous scleritis.

Role of testing for anticytoplasmic autoantibodies in the differential diagnosis of scleritis and orbital pseudotumor.

Wegener's granulomatosis is a multisystem disease often with protean manifestations. Eye signs and symptoms can be prominent and may be the patient's initial complaint. For a definitive diagnosis of Wegener's granulomatosis, a tissue biopsy specimen must show vasculitis and necrotizing granuloma. The presence of anticytoplasmic autoantibodies in the serum of patients has been found to be highly specific for Wegener's granulomatosis and can considerably facilitate early diagnosis and be used to monitor disease activity. In two illustrative cases, the utility of this laboratory test in the diffe

Ocular syphilis.

The ability of syphilis to mimic different ocular disorders can lead to misdiagnosis and delay in appropriate antimicrobial therapy. The authors describe their experience over the past 5 years with the ocular manifestations of syphilis in 25 patients who comprised 2.45% of 1020 new patients. Uveitis was the most common ocular manifestation seen. All patients had positive results from FTA-ABS tests, whereas only 68% had reactive serum VDRLs. Two of five patients tested for human immunodeficiency virus (HIV) antibody were reactive. The authors recommend routine FTA-ABS and VDRL screening in pati

[Intraocular malignant lymphoma].

The paper in devoted to a rare condition difficult to diagnose, intraocular lymphoma. Five patients with a disease history of 2 to 6 years were examined. The authors describe clinical features of the condition and histologic and electron microscopic findings. The clinical pattern of malignant intraocular lymphoma in polymorphous, therefore virtually every case with a sluggish uveitis or posterior scleritis resistant to common therapy has to be verified. A hematologist should take part in examination of such patients. Since the involvement is local, as our findings evidence, a course of polyche

[Postoperative necrotizing sclerokeratitis].

Two cases of necrotizing sclerokeratitis following uncomplicated extracapsular cataract extraction are reported. Enucleation became necessary in the first case despite initially successful immunosuppressive treatment. In the second case, a stable condition was achieved by covering the affected area with a patch of lyophilized dura. Two years later, however, phthisis bulbi developed.

Long-term follow-up study of scleroplasty using autogenous periosteum.

Autogenous periosteum was used to reinforce scleral thinning, perforation, or corneoscleral wound dehiscence in four eyes of three patients with necrotizing scleritis or peripheral ulcerative keratitis associated with advanced rheumatoid arthritis. All grafts have remained intact during an average follow-up interval of 36 months (range, 19-52 months). The postoperative visual acuity ranged between 20/30-20/60. Two eyes exhibited age-related macular degeneration, and one eye had an epiretinal membrane postoperatively. No systemic complications occurred following surgery.

[A case of necrotizing scleritis with angiitis of papilla].

A case of a 66-year-old woman suffering from polymyalgia rheumatica with necrotizing scleritis followed central retinal artery occlusion in the left eye was studied clinico-pathologically. The left eye was enucleated because of severe pain in spite of the steroid therapy, and was examined histopathologically. The necrotic granulomatous changes of the deep sclera in the ciliary body and peripheral fundus were observed. Lymphocytes, plasma cells and epitheloid cells infiltrated in the central retinal artery at the papilla. It seemed that subconjunctival injection of steroid had induced scleral n

Scleritis in relapsing polychondritis. Response to therapy.

The authors reviewed the records of 11 patients with relapsing polychondritis associated with active scleritis and analyzed the immunopathologic characteristics of ocular tissue from three of these. Seven patients (63%) required cytotoxic drugs, alone or in combination with low-dose oral corticosteroids. Only one patient was treated successfully with systemic corticosteroids alone. Two patients were controlled with dapsone and one with an oral nonsteroidal anti-inflammatory drug. Dapsone, which has been reported to be effective in the treatment of relapsing polychondritis, did not control the

Relapsing polychondritis in pregnancy.

A 25-year-old patient presented at 14 weeks of her 3rd pregnancy with chondritis affecting the cartilaginous portion of her right ear. The clinical syndrome of recurrent chondritis, scleritis, iritis and arthritis supported a diagnosis of relapsing polychondritis. Fetal and maternal outcomes were favorable despite steroid dependent active disease during the period of fetal organogenesis. Chondritis was limited to the 3rd pregnancy, ocular inflammation occurring in the 2nd and 3rd pregnancies. A 4th pregnancy was uneventful. This may represent the first case of relapsing polychondritis in pregn

[Early ocular symptoms in Wegener's granulomatosis].

Wegener's granulomatosis is characterized by necrotizing granulomatous vasculitis that initially occurs in a localized form, disseminates in various degrees and particularly involves the respiratory tract and kidneys. Eye manifestations are common (40 to 60%). Both clinical and histopathological findings are described in eight patients with Wegener's granulomatosis. The initial ocular symptoms included conjunctivitis, episcleritis, scleritis, corneoscleral ulceration, uveitis and ocular proptosis. In one patient histological examination of the orbital tissue was consistent with limited Wegener

Complications of malpositioned Jones tubes.

A Jones tube is an effective lacrimal drainage bypass device often associated with post-operative problems. Unsuspected migration of the tube may cause signs and symptoms that are not immediately identified with the prosthesis. Malposition of Jones tubes in three patients resulted in the following complications. The first patient had persistent episcleritis and atypical facial pain and numbness caused by migration of the tube against the globe and into the nasal septum. Bilateral tubes in the second patient were improperly oriented, causing severe scleral erosion and ulceration. The third pati

Sweet's syndrome.

Two women are reported both of whom exhibited, after an episode of an upper respiratory tract infection, painful, erythematous, sharply demarcated plaque-like lesions on the extremities, in combination with fever. In addition, one woman also had acute polyarthritis and episcleritis. The histological features of the lesions showed a perivascular neutrophilic infiltration in the dermis without signs of vasculitis, pathognomonic of Sweet's syndrome. One patient was successfully treated, initially with aspirin and later prednisone, the other with potassium iodide. Recognition of this syndrome is i

External ocular findings in lupus erythematosus: a clinical and immunopathological study.

A selected group of 18 patients with systemic lupus erythematosus (SLE) and 30 patients with chronic cutaneous lupus erythematosus (CCLE) showed an unexpectedly high incidence of problems involving the globe or eyelids. Five SLE patients had recurrent episcleritis, two CCLE patients had lower tarsal plaques, and two further CCLE patients had erosion of the lower lid margins associated with conjunctival scarring and symblepharon. This association has not previously been reported. There was an unexpectedly high incidence of deposition of immunoreactants in a linear pattern at the basement membra

[A case of Cogan's syndrome associated with multiple cranial neuropathy].

A 52-year-old woman was admitted to our hospital because of disturbance of right visual acuity and double vision. At 38-year-old she became deaf bilaterally and experienced many vertigo attacks. She was diagnosed as Ménière disease. At 45-year-old vertigo attacks disappeared. At 47-year-old right peripheral facial nerve palsy developed transiently with interstitial keratitis and episcleritis of the both eyes. Oral adrenocorticosteroid therapy produced an improvement of interstitial keratitis and episcleritis. On admission, ophthalmological examination revealed bilateral interstitial keratitis

[Ophthalmological aspects of Reiter's disease].

Clinical picture and course of eye involvement were studied in 254 patients with Reiter's disease. Altogether 130 patients suffered from conjunctivitis of varying severity, 23 from keratitis, 21 from episcleritis, 20 from uveitis, 2 from detachment of the retina, and 5 from secondary glaucoma. Chlamydia were detected in one third of conjunctivitis patients in scrapings off the conjunctiva. The eyes may be accidentally infected by agents from the urogenital organs if the patient does not observe the hygienic rules. Uveitis developed as a rule after a prolonged course of Reiter's disease. They w
