Episcleritis, conjunctivitis, and keratitis as ocular manifestations of Lyme disease.

A 35-year-old woman presented with a bilateral palpebral follicular conjunctivitis. Subsequently, she developed a bilateral keratitis and, on a separate occasion, an episcleritis that was associated with a recrudescence of Lyme disease and poor compliance with the antibiotic regimen. Both the keratitis and episcleritis cleared completely after topical corticosteroid therapy and reinstitution of appropriate antibiotic treatment. This report emphasizes the importance of collaboration between internal medicine and ophthalmologic specialists during the long-term management of Lyme disease.

Angle-closure glaucoma as initial presentation of myelodysplastic syndrome.

A 57-year-old man presented with proptosis of the right eye and findings consistent with bilateral angle-closure glaucoma. Subsequent evaluation revealed severe bilateral uveal effusions and associated nonrhegmatogenous retinal detachments, which were felt to be related to orbital pseudotumour and associated scleritis. Hematologic studies were consistent with a diagnosis of myelodysplastic syndrome. Although myelodysplastic syndrome has been reported in one patient with orbital inflammation and myositis, to our knowledge ciliochoroidal effusion and secondary angle closure have not previously b

Ocular manifestations of patients with circulating antineutrophil cytoplasmic antibodies.

Antineutrophil cytoplasmic antibodies are seen in patients with systemic vasculitides, especially Wegener's granulomatosis. Antineutrophil cytoplasmic antibodies are helpful laboratory markers for these disease. We report on the ocular findings of six patients with systemic vasculitis who had antineutrophil cytoplasmic antibodies. Four patients had systemic Wegener's granulomatosis, one had microscopic polyarteritis, and in one a specific histopathologic diagnosis could not be made. Two patients were first evaluated for systemic vasculitis because of their ocular manifestations. Ocular finding

Topical flurbiprofen: an effective treatment for episcleritis?

Topical flurbiprofen was compared with placebo and prednisolone 0.3% in the treatment of episcleritis by a randomised double-blind trial. Seventy-seven eyes were included in the trial. Rapid spontaneous improvement in symptoms and signs was noted in the majority of cases. There was no significant difference between the cure-rates of patients treated with flurbiprofen and placebo over a 3-week trial period. The proportion of patients treated with prednisolone 0.3% who were cured was significantly higher than in either of the other groups after 3 weeks.

The causes of glaucoma in cats.

The cause of glaucoma in 131 enucleated eyes from 128 cats was determined in a retrospective histologic study. Obliteration of the ciliary cleft by diffuse iridal melanoma (38 eyes), or other neoplasms (14 eyes), or by the presence of idiopathic lymphocytic-plasmacytic anterior uveitis (53 eyes) were the most frequent lesions likely to explain the development of glaucoma. Secondary changes of inner retinal atrophy, optic disc cupping, scleral thinning with megaglobus, and atrophy of ciliary processes were similar to those described in dogs and human beings with chronic glaucoma. In light of th

Acute scleritis in porphyria cutanea tarda.

We examined three patients who had dermatologic and biochemical manifestations of porphyria cutanea tarda and localized thinning or excavation in the sun-exposed interpalpebral sclera, adjacent to the cornea. All three patients had signs of acute scleritis. The acute scleritis responded to oral indomethacin in one patient, but systemic corticosteroids were required to control the inflammation in the other two patients. Phlebotomy, protection from sunlight, and refraining from alcohol played an important part in the treatment of the patients.

Lymphomatoid granulomatosis presenting as sudden blindness.

Lymphomatoid granulomatosis is an uncommon clinical problem and even more rarely presents to the ophthalmologist. A case is described in which a 60-year-old Fijian woman presented with sudden, sequential, complete visual loss in association with signs of scleritis. Direct eyewall biopsy and nasal mucosal biopsy confirmed the diagnosis of lymphomatoid granulomatosis. Despite intensive high-dose oral prednisone therapy her vision remained at no light perception and she developed recrudescence of disease necessitating the use of radiotherapy. All systemic investigations failed to show any other o

Unusual mucopolysaccharide disorder with corneal and scleral involvement.

A 68-year-old man and a 66-year-old woman had diffuse corneal stromal deposits that stained with alcian blue and colloidal iron but did not react with periodic acid-Schiff stain and lipid stains. Similar deposits were found within postmortem sclera in one case, but not in other ocular or extraocular tissues. The abnormal material was sensitive to testicular hyaluronidase and chondroitinase. The material reacted with monoclonal antibody 9-A-2 after digestion by chondroitinase AC in one case and ABC in both cases, which is consistent with the identification of the glycosaminoglycans chondroitin

Sterile corneal ulceration after cataract extraction in patients with collagen vascular disease.

We report the occurrence of sterile corneal ulceration in 11 eyes of eight patients with collagen vascular diseases and dry eyes after cataract extraction with intraocular lens implantation. Keratolysis occurred after both extracapsular and intracapsular cataract extraction and appeared unrelated to the type of intraocular lens. Despite aggressive lubrication and other medical treatment, including systemic immunosuppressive agents, penetrating keratoplasty was often required. Although all eyes were saved, visual outcome was usually poor. The histopathologic finding of polymorphonuclear leukocy

Antilens antibodies in cataract and inflammatory eye disease: an evaluation of a new technique.

A new technique measuring serum antoantibodies to lens proteins, employing antigen linked to magnetisable cellulose particles and fluorescent end-point detection, was used to examine patients with senile cataract and inflammatory eye disease. 40% of patients with senile cataract had antibodies to lens proteins, as did 28% of patients with heterochromic cyclitis and 20% of patients with uveitis, while there were no positive sera from patients with scleritis. The single patient with lens-induced uveitis had a high titre, but so did some patients with senile cataract. With the incorporation of a

Scleritis as the presenting manifestation of procainamide-induced lupus.

Scleritis developed in a patient using procainamide as part of a drug-induced lupus syndrome. Systemic findings, which developed after the onset of ocular signs and symptoms, included arthralgias, myalgias, weight loss, and markedly elevated antinuclear antibody (ANA) titers and antihistone titers. The clinical picture and laboratory abnormalities improved after discontinuation of the drug. Although systemic findings secondary to drug-induced lupus have been well described in the medical literature, this case represents the first detailed ophthalmologic documentation of scleritis as the presen

[Involvement of the eyes in yersiniosis].

The results of clinical examinations of 15 patients with yersiniasis are analyzed. Besides the involvement of the anterior segment of the eye, known before, the authors have detected for the first time 3 cases of the fundus oculi involvement; the impairments resemble multiplaquoid epitheliopathy. The clinical description is supplied with fluorescent angiograms. Yersiniasis is regarded as a possible etiologic factor responsible for multiplaquoid epitheliopathy; this extends the scope of our knowledge on the etiological structure of the fundus oculi impairments.

Scleral grafting for necrotizing scleritis.

Although systemic immunosuppressive chemotherapy is effective in halting progressive necrotizing scleritis, the onset of its action may be too slow to prevent profound scleral thinning and/or traumatic or spontaneous perforation. Scleral homografts may be used to maintain the integrity of the globe until immunosuppressive drugs can take effect. The authors reviewed their experience with scleral homografts in 12 patients with progressive necrotizing scleritis; eight (all with autoimmune disease) had concomitant chemotherapy and four (two with autoimmune disease) did not. Grafts remained stable

Tear stix tests for leucocyte-esterase, nitrite, haemoglobin, and albumin in normals and in a clinical series.

Tears are absorbed by a tuft of cotton and subjected to stix test for leucocyte-esterase (L), nitrite (N), haemoglobin (H), and albumin (A). Testing of 84 cases of infectious conjunctivitis and 282 normals revealed nosographic sensitivity to L in 89% and a specificity of 98%. By including N (only 26% positive with infectious conjunctivitis) and H the sensitivity rose to 98% while the specificity fell to 95%. A was generally raised in cases of infectious conjunctivitis. An additional number of 607 stix tests were carried out on a clinical series. The reaction was controlled before, during, and

[Increased episcleral venous pressure in uveal effusion].

This paper reports on two patients with bilateral, spontaneous choroidal and retinal detachment. One patient had scleritis, while in the other no underlying eye disease was found. No signs of orbital venous congestion were detected. Although the pathogenesis of this disease is not fully known, the findings of (1) dilated episcleral veins and (2) elevated episcleral venous pressure suggest an obstruction of venous outflow from the uveal tract. The authors believe that these two findings may have been caused by shunting of venous blood anteriorly from the uveal tract into the episcleral veins du
