To ascertain the efficacy of systemic cyclosporin therapy in the management of scleritis we performed an open, uncontrolled study of the use of this drug in severe refractory disease. Five of seven patients whose disease had previously been poorly controlled with a combination of corticosteroids and immunosuppressive drugs responded to cyclosporin therapy (10 mg/kg/day). Systemic side effects occurred in all but one patient, with tremor, hirsutism, hypertension, and raised serum creatinine being common. In the present study, we compared hemolytic activities of C1, C4, C2, C3, C5, C6 and C7 in the anterior and posterior sclera. Additionally, we used radial immunodiffusion to measure levels of Factor B, IgG, IgA and albumin in the anterior and posterior sclera. Acute angle-closure glaucoma complicating ciliochoroidal detachment developed in eight eyes of six patients. The prevalence and pattern of mucosal involvement in 121 patients with lupus erythematosus (LE) was investigated. Fifty-three patients had systemic LE (SLE) and 68 patients had chronic cutaneous LE (CCLE). Twenty-one per cent (11/53) of patients with SLE and 24% (16/68) of patients with CCLE had signs of mucosal involvement, but the pattern of involvement differed in the two groups. Posterior scleritis is a granulomatous inflammation occurring either within the scleral tissue or in the vascular networks adjacent to it. One hundred and seven patients of definite rheumatoid arthritis were screened for the presence of ocular manifestations of which 21 (19.63%) were observed to have ocular changes. Of these keratoconjunctivitis sicca in 19 (17.7%) patients and episcleritis in one (0.93%) were attributable to rheumatoid arthritis. Bilateral retinal haemorrhage in one patient was due to grade 3 hypertensive retinopathy. Absence of posterior subcapsular cataract in 106 (99.06%) patients who did not receive corticosteroids supports the hypothesis that steroids are the aetiologic agents and not rheumatoid arthritis.

Superior limbic keratoconjunctivitis (SLK) is a disease characterised by inflammation of the upper palpebral and superior bulbar conjunctiva, keratinisation of the superior limbus and corneal and conjunctival filaments. It may be associated with other diseases such as keratoconjunctivitis sicca, hyperthyroidism and hyperparathyroidism. Although its exact aetiology is unknown, a mechanical hypothesis seems most attractive. Treatment is aimed at altering the abnormal mechanical interaction of the upper lid and superior corneal limbus.

Pterygium excision was complicated by bacterial corneoscleritis and endophthalmitis in two patients, one who had received postoperative beta irradiation and the other topical thiotepa. These complications followed surgery by two and six weeks. Causative organisms were Streptococcus pneumoniae and Pseudomonas aeruginosa, respectively. Despite aggressive therapy, visual outcome was poor in both cases. These cases demonstrate that, although infrequent, complications of pterygium excision can be destructive and visually disabling.

We reviewed the records of all patients with rhegmatogenous retinal detachments examined and treated by a single surgeon (B.P.C.) at the University of Virginia, Charlottesville, between 1978 and 1984. Of the 607 eyes that satisfied the selection criteria, a preliminary chart review of outcomes found that 65 (10.7%) had proliferative vitreoretinopathy and 34 had macular puckers. To measure retinal nerve fiber layer (NFL) thickness, three sets of simultaneous stereophotographs of the optic disc, taken with the Donaldson fundus camera, were measured at 5 degrees intervals in a stereoplotter (Kern PG2) at the surface of the internal limiting membrane (ILM) and the retinal pigment epithelium (RPE) along the disc margin. Necrotizing scleritis with inflammation of the right eye developed after bilateral eye muscle surgery for thyroid ophthalmopathy. Debilitating pain, delay in onset, and involvement of the sclera distinguish this condition from anterior segment ischemia. The surgery may have acted as a nonspecific trigger in an eye at risk for scleritis. Necrotizing scleritis has occurred infrequently after other types of eye surgery but, to our knowledge, has not been previously reported as a complication of eye muscle surgery.
