Evidence of ocular involvement was found in seven of eleven patients with histologically confirmed Wegener's granulomatosis. Pain (7/7), redness (6/7), scleritis (5/7), reduced visual acuity (5/7), episcleritis (5/7), uveitis (3/7), proptosis, sclerokeratitis and conjunctivitis (2/7) each were the major ophthalmological features. One patient each had bilateral corneoscleral ulcers, eyelid infiltration, scleromalacia perforans and epiphora. Corneal, scleral and uveal tract involvement was observed more frequently than has been described in the literature. These complications responded favourabl

2'-Deoxycoformycin, a potent inhibitor of adenosine deaminase, was administered to three patients with cutaneous T cell lymphoma refractory to multiple treatment modalities. Patient 1, who received 5 mg/m2/day for 3 days at 35- to 71-day intervals, has achieved a complete remission greater than 16 months in duration. Patient 2 had progressive disease despite two courses of 2'-deoxycoformycin at a dose of 5 mg/m2/day for 3 days at 28-day intervals. The third patient, who was treated with 4 mg/m2 2'-deoxycoformycin weekly to biweekly, had an initial response, but the disease progressed after eig

Sinus histiocytosis with massive lymphadenopathy is a non-neoplastic disease of unknown cause observed mainly in children with markedly enlarged cervical or other lymph nodes. We have reported two very atypical cases with extranodal manifestations that required ophthalmological consultation. Neither patient had significant lymphadenopathy. One patient, who had tumors of all four eyelids as well as lesions of the nose and larynx, was 68 years old when first examined, and 70 at the time of his last surgical procedure. The other patient, a 13-month-old child, had a unique corneoscleral lesion as

Carbon dioxide (CO2) laser treatment was applied to the cornea and conjunctiva of 16 eyes from eight young pigs at power levels of 2.5 to 20.0 W, an exposure time of 0.1 seconds and different numbers of overlapping burns. Four animals were euthanatized at 30 minutes, one at 36 hours and three from 18 to 21 days after the treatment. Nitrous oxide (N2O) cryotherapy also was applied in a double freeze-thaw fashion to three of the eyes to compare the effects of the CO2 laser and cryotherapy at each of the time intervals. Epithelial destruction was achieved with CO2 laser power levels of 5.0 W and

Two similar patient populations with previously unoperated, atraumatic, rhegmatogenous retinal separations were surgically treated using the same technique with encircling elements in one series (290 eyes) and without encircling elements in the other (463 eyes). Nine percent of the encircled group remained detached compared with 7.6% in the other group. The reason for ultimate surgical failure in both groups was proliferative vitreoretinopathy. Encirclement offered no advantage with respect to surgical success.

We used split-thickness dermal grafts for the surgical treatment of corneal and scleral perforations in two patients and obtained excellent results. Patient 1 had severe lye burns and bilateral corneal perforations and Patient 2 had scleromalacia perforans. The dermal graft self-epithelializes and, thus, does not need to be covered by conjunctiva; is supple, without the bulkiness of other materials, particularly cartilage and periosteum; is hearty and flourishes on avascular surfaces such as cornea and sclera; has good tensile strength; and is autogenous.

Scleritis is an important, severely destructive, chronic inflammatory disorder affecting the eye wall. It presents a difficult management problem, often requiring high-dose systemic corticosteroid therapy or other immunosuppressive regimens to control the inflammatory response. A quantitative scleritis scoring system has been developed and its application to the assessment and management of scleral disease is discussed. This paper reviews current concepts in the management of scleral disease with emphasis on newer treatment modalities, such as pulse therapy with intravenously administered meth

We describe a 56-year-old woman with benign hypergammaglobulinemic purpura in whom scleritis, acute stromal keratitis and peripheral corneal furrowing developed. Initial therapy with topical antibiotics and steroids failed to control the scleritis and keratitis. Subsequent treatment with plasmapheresis led to a decrease in circulating immune complex and serum immunoglobulin levels, with resolution of the ocular inflammation. After 8 years of follow-up the serum IgG and IgM levels were again elevated; however, the corneal disease remained quiescent. Plasmapheresis may have a role in the therapy

Seventy-six consecutive patients with total rhegmatogenous retinal detachments and severe proliferative vitreoretinopathy underwent combined pars plana vitrectomy, lensectomy, panretinal photocoagulation, perfluoropropane gas (C3F8)/fluid exchange, and scleral buckling. Sixty-two (82%) of the patients had successful, sustained (greater than 12 months) posterior retinal reattachments at last examination. Of these 62 patients, 40 (65%) had complete retinal attachment, with no evidence of regrowth of periretinal membranes or redetachments following the initial procedure. In the remaining 22 cases

An 11-year-old boy and a 28-year-old woman each had a painless, slowly enlarging, elevated, yellow lesion of the inferior corneoscleral limbus and adjacent cornea and conjunctiva of the left eye. Histopathologic examination of both lesions disclosed involvement of the conjunctival substantia propria and the adjacent limbal and peripheral corneal stroma by a nonencapsulated, dense infiltrate composed of elongated fibrocytic cells, plump histiocytic cells occasionally with a vacuolated cytoplasm, some multinucleated giant cells, and scattered lymphocytes. The diagnosis in each case was fibrous h

A number of cases of necrotic sclerokeratitis following eye surgery have been reported in recently published literature. The condition was presumably triggered by surgical inflammation and caused by localized occlusive vasculitis: in one case deposits of immune complexes in vessel walls were demonstrated. The authors report on three cases of necrotic sclerokeratitis which developed five to 16 months after uncomplicated cataract extraction. Clinical examination showed disappearance of vessels in affected sclera, together with tissue necrosis. None of the patients had any systemic immune disease

Although toxoplasmosis is the most common infectious cause of posterior intraocular inflammation, it is rarely described in association with scleritis. The authors present five cases of toxoplasmosis with scleritis. Two of the five cases were diagnosed clinically and serologically as having toxoplasmosis. Their retinochoroiditis and scleritis responded well to medical therapy. Retinochroiditis and scleritis that was refractory to treatment developed in the other three patients, two of whom had been receiving immunosuppressive therapy for systemic diseases. Their therapeutic regimens did not in

A 23-year old male presented for evaluation of skin coloured, non-scaly, asymptomatic papulonodules of sizes varying from 0.5 cm to 2 cm of 4 years duration distributed all over the body including the ears. The plaques present on the face gave the appearance of a 'leonine facies'. Clinically mistaken for lepromatous leprosy in reaction the patient was treated with antileprosy and anti-inflammatory drugs in 3 other centres for months with no improvement. Systemic involvement included painful swelling of both knee joints, pericardial effusion episcleritis and enlarged liver. Negative slit smears

Scleral inflammatory disease, a relatively common disease of the eye, is demonstrable on CT. Because of its unilateral and sometimes focal involvement, it may be mistaken for more serious diseases, both clinically and by CT. In particular, the clinical difficulties in diagnosing posterior scleritis have led to enucleations when such entities as melanoma and metastasis were suspected. We report the CT appearance and differential diagnosis of scleritis and correlate them with its clinical manifestations, both within the eye and systemically. We found CT to be reliable in differentiating scleriti

We present the case history of a 50-year-old man with seropositive erosive rheumatoid arthritis of 30-years standing who developed polychondritis simultaneously with several extra-articular rheumatoid manifestations, such as anaemia, subcutaneous nodules, pericarditis and episcleritis. The relevant literature is reviewed. Gradually, all symptoms and signs disappeared after start of treatment with 30 mg prednisone and 100 mg azathioprine daily. We suggest that the polychondritis in this patient was also an extra-articular manifestation of rheumatoid arthritis.

We screened patients with episcleritis or scleritis to detect syphilis by using either fluorescent treponemal antibody absorption testing or microhemagglutination assay for antibodies to Treponema pallidum. Four patients, two with nodular episcleritis and two with scleritis, had ocular involvement as the initial manifestation of late syphilis. Topical or oral corticosteroid therapy provided minimal initial clinical benefit, but ocular inflammation resolved within one to three weeks after administration of systemic penicillin.

We studied six patients with early failure after Watson's trabeculectomy done due to chronic simple, neovascular, or chronic angle-closure glaucoma. After operation the eyes showed a maximum intraocular pressure (IOP) between 50 mmHg and 66 mmHg and no filtering bleb. Reoperation on the 5th to 14th post-operative day showed incarceration of ciliary body into the trabeculectomy fistula in all cases. The uveal protrusion was replaced and a deep corneoscleral block was removed in front of the scleral spur in three cases, and electrocoagulation of the anterior edges of the trabeculectomy fistula w

In 10 eyes a direct refixation of the ciliary muscle to the scleral sporn with many 10 X 0 nylon sutures very close to each other has been performed after a traumatic cyclodialysis. In all 10 eyes the intraocular pressure could be normalized between 12 and 18 mm Hg, the visual acuity recovered in all cases and the signs of ocular hypotony disappeared at the anterior and posterior segment. The follow-up ranged from 6 months to 4 1/2 years. The extent of cyclodialysis ranged from 30 to 120 degrees. In 5 eyes the direct cyclopexy was performed within 10 days after the traumatic cyclodialysis and
